A possible mode of operation and application proceeds as follows: The medical instrument is inserted into a body cavity (for example, into the gallbladder) by a doctor with the small bowel which has been retracted into the tube-like sheath and folded in a space-saving manner with the distal tip of the instrument from which a stone, for example, should be removed. Subsequently, the
Transmission element by means of the handling element in the tube-like sheath distally (ie forward) moved to push out the basket partially or completely from the distal end of the sheath to a desired extent, wherein the basket due to its execution according to partially or completely unfolded under spatial Belas- free spaces between its jetties. Subsequently, the basket is placed with its webs around the stone to be removed and then the transmission element in the tubular casing pulled back until the webs of the basket close to the stone to be removed, so that it is trapped in the basket. In this state, the medical instrument can be withdrawn backwards out of the body cavity with the stone held on its distal tip, thus safely removing the stone. Despite numerous applications, including the ability to introduce such instruments through a so-called working channel of an endoscope in the body cavity, the exact positioning of the basket at the treatment site in the body requires considerable skill. To push the basket forward, it can be exerted on it by means of the handling element and the transmission element, a thrust; to retract the basket to the proximal, it can be transmitted by handling and transmission element, a tensile force.
In order to rotate the basket about its geometrical longitudinal center axis, a torsion element can be transferred thereon by means of the handling element (for example a handle) and of the transmission element. The tensile or shear forces and torsional moments to be transmitted, depending on the application, could be of considerable size. It is essential that the transmission element under its loads in the appropriate directions as little or not deformed, otherwise the induced by a doctor on the handling element movement can not be appropriately transferred to the basket accordingly. Therefore, in order to achieve a high rigidity, such instruments conventionally have transmission elements formed of a wire or wire mesh of solid cross-section, as compared to the webs of the basket. On the other hand, there are requirements to be observed with respect to the permissible thickness of the sheath, so that the instrument can easily move back and forth in the working channel of an endoscope. This also leads to the difficulty that, depending on the cross-sectional thickness of the transmission element no guide wire for positioning of the basket can be included in the sheath with. Against this background, the invention has the object of developing a medical instrument of the type mentioned advantageous, so that in particular the limitations and disadvantages described above can be avoided as much as possible. The object is achieved according to a first aspect of the present invention initially and essentially in conjunction with the features that the transmission element has at least one in its interior in its longitudinal hollow channel, ie a longitudinally leading cavity, and that provided either a guide wire is that extends through the hollow channel, or that the medical instrument can be supplemented by a guide wire, which can be passed through the hollow channel. The cross-section of the guide wire may, for example, have approximately the same size or preferably smaller, in particular only slightly smaller, dimensions than the hollow cross-section in the hollow channel for achieving a mobility. This initially allows a space-saving recording of the guide wire inside the transmission element, so that the guide wire must not be performed outside the transmission element and the cross-section of the transmission element with respect to its outer dimensions, the hollow cross-section in a shell can exploit to the extent that just barely the required mobility exists. According to the invention, the advantage of a comparatively tension / compression and torsion-resistant transmission element can thus be combined with the possibility of guiding a guide wire, which facilitates the positioning of the instrument, in particular of the basket, in a space-saving manner through the jacket of the instrument. It is understood that depending on the requirements For example, a thin catheter or the like could be passed through the transmission element instead of a guide wire.
Gemäß einem weiteren Aspekt, der im Rahmen der Erfindung sowohl als be- vorzugte Weiterbildung als auch eigenständig von Bedeutung sein kann, besteht die Möglichkeit, dass das Übertragungselement eine Mehrzahl von Drähten umfasst, dass die Drahtquerschnitte in dem zur Längsrichtung des Übertragungselements senkrechten Querschnitt entlang des Umfangs des Hohlkanals angeordnet sind und dass die Drähte mit zueinander gleicher Steigung in zu- mindest einer mehrgängig wendeiförmigen Anordnung um den mittigenAccording to a further aspect, which may be of importance in the context of the invention both as a preferred development and independently, there is the possibility that the transmission element comprises a plurality of wires, that the wire cross sections in the cross section perpendicular to the longitudinal direction of the transmission element along the Circumference of the hollow channel are arranged and that the wires with mutually equal pitch in at least one multi-turn helical arrangement around the central
Hollow channel are guided around. In the manufacture of the transmission element, the wires are wound accordingly and then remain bent in this shape. Each individual wire forms a helical arrangement with a gear. The majority of the wires belonging to a transmission element are wound together during manufacture, so that the respective single-turn helical arrangements of the individual wires intermesh to form the overall multi-turn helical arrangement. Experiments have shown that such a transmission element, which is also referred to as a strand in the context of the invention, high tensile and shear forces, as well as high torsional moments with high accuracy, ie with only slight deformation-induced deviation transmitted. The wires covered the hollow channel. For example, twelve wires which are preferably identical to one another can be used to form a transmission element. It is understood, however, that a different number of wires, for example, between ten and twenty (if necessary, but also a smaller or a higher number), could be selected. The number is, preferably also depending on the diameter of the individual wires and the further embodiment, preferably chosen so that the hollow cross-section of the central hollow channel is so large that a conventional guide wire, for example. A metallic bare or shrink-tube related guidewire, in the hollow channel in the Longitudinal direction can be easily moved. The length of the transmission element can be adapted to the requirements prescribed for the medical instrument and, if necessary, for example, be 1.5 m, 2 m or longer. Even shorter lengths are of course possible. The described helical arrangement of the wires can also be referred to as a helical spring-wound or bent structure or else as a helical structure, wherein a multiple helix is present overall.
In this respect, the invention according to the second independent aspect relates to a medical instrument according to the preamble mentioned above, wherein the invention proposes that the transmission element at least one in its interior in the longitudinal direction continuous hollow channel, ie a longitudinal leading cavity, that the transmission element has a A plurality of wires comprises that the wire cross-sections are arranged in the cross-section perpendicular to the longitudinal direction of the transmission element along the circumference of the hollow channel and that the wires are guided with mutually equal pitch in at least one multi-turn helical arrangement around the central hollow channel. In the following, various possibilities for further development preferred for an instrument according to the first aspect of the invention and for an instrument according to the second aspect of the invention will be described. First, it is preferred that the wire cross-sections along the circumference of the hollow channel, preferably along a circular line, evenly distributed, ie evenly spaced from each other, are arranged. In this case, an expedient embodiment is seen in that each wire bears against the wires adjacent to it in the cross section along the circumference on both sides. This means that each individual wire rests against the two wires which are laterally adjacent to its direction of travel, so that the wires together in the longitudinal and circumferential direction of the transmission element at least in the Form substantially or completely closed coat. This can be achieved by "winding" the individual wires in the manufacture of the transmission element, and it is also preferred that the number of wires corresponds to the number of turns of the multi-turn helical arrangement. For example, if the transmission element comprises twelve individual wires It is preferred that the helixes of the helical wire winding be about equal to or greater than 40 °, but other helix angles are possible.
However, there is also the possibility in a particular manner that the transmission element comprises two or more multi-turn helical arrangements comprising a plurality of wires, wherein at least one helical arrangement is disposed concentrically in a helical arrangement of larger cross-section, and wherein the Wires of a helical arrangement in
Compared to the wires of the other helical arrangement have a respect to the pitch sign opposite, but preferably equal in magnitude, slope of the helical course. Within each individual arrangement, the pitch of all wires is preferably the same. Such a transmission element is characterized by a further increased rigidity or by an even higher deformation resistance to tensile / compressive forces and torsional moments and thus by a further increased transmission accuracy of movements. In such a preferred embodiment, the medical instrument is also particularly suitable for the removal and destruction of particularly hard stones, such as eg gallstones. While with the use of transmission elements in which the one or more wires are wound in only one pitch direction (ie also with the same slope sign), a so-called "wave formation" can occur at high alternating pressure and tensile loads of the transmission element, especially in the proximal range This is reduced or avoided altogether in the preferred, so far opposite winding of wires with opposite slope sign.
It is also preferable that the individual wires have a round, preferably a circular wire cross-section. There is the possibility that the round wire cross-section is flattened on the side facing away from the hollow channel outside compared to the circular shape, so that a total of substantially circular outer contour of the transmission element is formed. It is preferably thought that the individual wires made of stainless or stainless material, preferably made of stainless steel. Particularly suitable are stainless steels with the material designations 1.4310, 1.4301 or 1.4401 as well as, for example, alloys containing nickel and titanium (so-called nitinol alloys). As already stated, it is preferred that the instrument has a hose-like casing in which the transmission element is displaceably received in its longitudinal direction and rotatable about its geometrical longitudinal central axis, wherein the basket by means of the handling element and the transmission element selectively move into the casing or from the Sheath can be pushed out. As appropriate, it is considered that the basket has a plurality of flexible webs which extend from its proximal end connected to the transmission element to its distal end and distributed on the circumference of the basket, preferably evenly distributed on the circumference. The said webs can, when the basket in its proximal displacement position inside the Umman- is located in a stretched position. On the other hand, the webs can cause, in particular due to a certain embossed predetermined shape, that the basket unfold when pushed out of the sheath from its space-saving extended position to a spatial structure, the basket cross-section is then greater than the cross-section of the sheath and the distance between the webs clearly larger than in the stretched position. In particular, webs of so-called shape-memory material, in particular alloys containing nickel and titanium, in which certain shapes can be impressed well for the unfolded state of use of the basket, can be used to support these properties. For the medical instrument according to the invention a wide variety of baskets can be used. By way of example, the basket may be composed of a plurality of wires or of a plurality of wire strands braided in each case from wire filaments, for example being welded (in particular laser-welded). Alternatively, a basket made in sections or in one piece may be used. For example, it can be a tube produced in regions or in one piece by means of a laser cutting method from a tube, in particular from a nickel-titanium alloy. In particular, such cups produced as nickel-titanium alloys when pushed out of the shell have a high expansion force, ie require high Zugbzw. Shearing forces for retraction and extension and therefore require a rigid transmission element. Especially in this context is therefore advantageous that, if necessary, even a guide wire can be accommodated to save space in the interior of the transmission element. Particularly in this connection, it is also preferred that the guide wire extends not only through the transmission element but also through a passage opening of the handling element and / or through the basket, preferably through an opening formed at the proximal end of the basket and / or. or by one at the distal End of the cup trained opening. The guidewire may thus project lengthwise beyond the proximal end of the handle and, if necessary, beyond the distal tip of the cup, and is longitudinally slidably and rotatably received within the medical instrument. In order to connect the transmission element with the handling element or with the basket either directly or indirectly, ie by means of intermediary further elements, there is the possibility that the front ends of the transmission element are, for example, flattened by grinding or brought into a planar shape. The connection itself may, for example, be a welded connection (in particular produced by means of the laser welding method), an adhesive connection or the like.
Preferably, there is the possibility that the transmission element, preferably over its entire length, is sheathed by a flexible sheath or sheath which fits tightly against it and / or adheres, which may preferably be a hose, such as a heat-shrinkable tube , This can advantageously be prevented from the outside acting pollution of the transmission element. In particular, this also opens up the possibility of passing a liquid, such as a contrast agent, through the cavity formed between the tube-like outer casing and the transfer element, without this liquid penetrating into the transfer element formed of wires. Preferably, an envelope is contemplated which, due to its close-fitting and / or adhering attachment relative to the transmission element is not or only with difficulty displaceable and / or not or only with difficulty rotatable. On the other hand, it is provided that the transmission element, together with its enclosure fixed thereto, is easily displaceable and / or rotatable relative to the outer tubular casing. Likewise, it is provided with regard to an existing guide wire that it can easily be displaced and / or rotated in the longitudinal direction through the interior of the transmission element. at The guidewire itself may also have a sheathing which is relatively immovable to it, for example a heat-shrinkable tube fixed on it. It is preferably provided that the flexible covering of the transmission element, which is preferably shrink tubing, extends in length continuously from the handling element to the distal end of the transmission element or beyond. In this way, a lengthy complete protection of the transmission element and also possibly further existing transmission elements (such as, for example, a tube) against contamination can be achieved. The contrast agent supplied in the area of the proximal longitudinal end of the outer sheath is to be transported within the outer sheath to its distal longitudinal end and exit there. In order to prevent the fluid or the contrast agent from reaching the proximal direction from the supply cross-section and possibly exiting the outer sheath proximally, there is the possibility that in the region of the proximal longitudinal end of the outer tube-like sheath or in one adjoining housing piece at a relative to the feed cross-section proximal position, a seal is arranged, which cooperates sealingly with the sheath, preferably with the shrink tube, the transmission element at the cross-sectional circumference.

Fig. 3). 10 shows a fiction, in accordance with a medical instrument according to a fourth preferred embodiment in a comparable to FIG. 1 side view, partially broken away and shown in section, and Fig. 11 is a detail enlargement of detail XI in Fig. 10, With reference to FIGS. 1 to 4 and 7, first of all a medical instrument 1 according to the invention is presented according to a first preferred embodiment. This comprises a basket 2, a handling element 3 and an elongated transmission element 4, by means of which either tensile or shear forces and torsional moments, which are directed around the geometric longitudinal center axis, can be transmitted from the handling element 3 to the basket 2. In the example chosen, the transmission element 4 at its distal end 5 at its end face with a sleeve 6 and this at its opposite longitudinal end with the webs formed of wire 7 of the basket 2 fixedly connected, which in the example by laser welding joints, which in the figures are not shown in detail is reached. The transmission element 4 is thus indirectly connected to the basket 2 or attached thereto (see Fig. 3). The proximal longitudinal end 8 of the transmission element 4 is likewise fixedly connected to a tube 9 by means of a laser welding connection (not shown), which is connected at its opposite, proximal longitudinal end to the distal end face of the handling element 3 in a manner not further shown in the figures (cf. also Fig. 2). In this way, the transmission element 4 is indirectly fixedly connected to the handling element 3, which serves as a handle. As shown in particular the enlarged detail in Figure 1 and Figures 3, 4 clearly extends in the interior of the transmission element 4 in the longitudinal direction L continuously a hollow channel 10. In the continuous extension extends through the tube 9, a through hole 11 and through the handling element On the distal side extends in extension of the hollow channel 10 through the sleeve 6, which is located at the proximal tip of the basket 2, a through opening 13 and, in particular aligned, by a at the distal tip of the basket second 1 shows that through the hollow channel 10, through the Durchgangsöffnun- gene 11, 12 and through the likewise through openings 13, 14 continuously extending a guide wire 16 so that its proximal end 17 protrudes proximally over the handling element 3 and its distal longitudinal end 18 protrudes distally beyond the distal tip 19 of the sleeve 15. The transmission element 4 has a hose-like design and is manufactured in the example from twelve thin individual wires 20. 4 shows that the wire cross sections of the wires 20 are arranged in the cross section perpendicular to the longitudinal direction L of the transmission element 4 (corresponding to the plane of the drawing of FIG. 4) along the circumference U of the hollow channel 10, one being distributed uniformly along the circumference. spaced arrangement is provided along a circular line. It is provided that each wire 20 bears against the adjacent to him in Figure 4 on its two sides wires 20 touching. In particular, FIG. 3 illustrates that the wires 20 are guided around the central hollow channel 10 in this lateral arrangement with mutually equal pitch or with an equal pitch angle .alpha. In a twelve-way helical arrangement. For this purpose, one of the twelve wires is exemplarily provided in FIG. 3 with the reference numeral 20 'and this reference symbol is attached to two windings of this wire which are adjacent in the longitudinal direction. The wire 20 ', taken alone, forms a catchy helical arrangement, just as the other eleven wires 20 (which are not distinguished from one another by reference numerals) each form a single-turn helical arrangement. The helical arrangements of the individual wires 20 are wound into one another in such a way that the twelve-turn helical arrangement shown in FIG. 3, which is designated overall by the reference numeral 21, arises. FIG. 3 shows that in the selected configuration, due to the twelve existing wires, a twelve-way helical arrangement 21 is produced, ie that the number of wires 20 corresponds to the number of turns of the arrangement 21. In the example, the pitch angle α indicated in FIG. 3 is approximately 40 °. It is understood, however, that this is described here only by way of example. FIG. 4 illustrates that the wire cross sections of the multi-turn helical assembly 21 associated wires 20 along the circumference U of the hollow channel 9 in one layer, ie only in a circumferentially curved following row, are arranged. In this example chosen, that is not necessary, the outer diameter D of the transmission element 4 is about 1.5 mm and its inner diameter d 0.95 mm, while the diameter b of the wires 20 is 0.28 mm in each case. In this way, the hollow cross-section of the hollow channel 10 is dimensioned so large that a guide wire 16 with a diameter of 0.89 mm (corresponding to 0.035 inch) can be easily moved back and forth in the longitudinal direction L or the guide wire can slide well in this direction , Incidentally, the illustration is not true to scale.
Bei der in Figur 4 gezeigten Ausführung und bei der in Figur 5 gezeigten Vari- ante besitzen die Drähte 20 im Ausgangszustand jeweils einen kreisrundenIn the embodiment shown in FIG. 4 and in the variant shown in FIG. 5, the wires 20 each have a circular one in the initial state
Drahtquerschnitt. Wire cross section. In contrast to Figure 4 is provided in Figure 5 that the round wire cross-section is flattened on the side facing away from the hollow channel 10 outside 22. The instrument 1 according to the invention has a tubular casing 23, in which the transmission element 4 is accommodated in its longitudinal direction L so as to be displaceable and rotatable about its geometrical longitudinal central axis. In the region of the tube 9, the sheath 23 merges into a section 23 'which, in the outer diameter, progresses stepwise towards the proximal end. A comparison of Figures 1 and 2 shows that the basket 2 by means of the handling element 3 and the transmission element 4 optionally according to Figure 1 can be pushed out of the casing 23 or move into the casing 23 according to Figure 2. The section 23 'widens up to a housing piece 23 ", which closes off the outer casing 23 proximally and which, for example, is grasped by a surgeon with one hand can, while the relatively movable handling element 3, for example. With the other hand can be taken. The formed in the sheath 23, in the direction of continuous cavity 29 continues in the longitudinal direction through the section 23 'and the housing piece 23 "continues or expands, possibly in the execution of several stages, up to a distal threaded bore 30 (see. 10, 11), in which a clamping sleeve 31 with its distal threaded pin 27 is screwed in. The screwing depth can be changed by engaging the clamping sleeve 31 on its grip part 28 and rotating it relative to the housing piece 23 ". Depending on the selected embodiment (see also FIGS. 10, 11), a cylindrical bore section 32 for receiving an annular seal 33 can be connected to the bottom of the threaded bore 30. The continuous cavity 29 also extends continuously through the clamping sleeve 31 and an optionally present seal 33. FIG. 1 shows a state of use of the instrument 1, in which the handling element 34 with its distal front end until it engages positively against the proximal end face The clamping sleeve 31, ie as far as possible, was approximated to the housing piece 23 ", whereby the basket 2 was pushed out and unfolded distally from the sheath 23. To retract the basket 2 into the sheath 23 and to save space therein, the handling element 3 became in the proximal direction of the clamping sleeve 31 and of the housing piece 23 "retracted, wherein in FIG. 2 the length of the tube 9 which has emerged from the clamping sleeve 31 is shown shortened in FIG.
FIGS. 1-3 also show that the basket 2 has several, in the example, four flexible webs 7. The webs 7 extend from the proximal end 24 of the basket 2 to its distal end 19 and are evenly distributed or spaced apart on the circumference of the basket 2. By means of the hollow spaces formed between the webs 7, the Basket 2 around a stone to be removed, for example, be placed around a gallstone. For the application of the described instrument 1, for example, an endoscope, which is not shown in the figures, can be introduced with its distal end up to a treatment site in a body. The guidewire 16 may then be passed into a body through a free working channel of the endoscope and optionally further to a treatment site, and then the instrument 1 advanced along the guidewire 16 through the working channel along the guidewire to the treatment site in the body can connect the treatment described above at the beginning.
FIG. 6 shows a modification of the embodiment shown in FIG. In this case, the transmission element 4 has two helical arrangements 21, 21 ', each of which comprises 12 wires 20 in each case. Each of the two arrangements 21, 21 'has been wound or bent in the manner already described above from in each case twelve wires 20 each having a twelve-fold helical structure. The inner diameter d and the outer diameter D of the inner assembly 21 are adapted to the inner and outer diameter of the sleeve 6. The inner diameter of the outer arrangement 21 'substantially corresponds to the outer diameter of the inner arrangement 21. As FIG. 6 shows in the lower image section, the arrangements 21 and 21', apart from their different diameters, have an opposite slope sign with respect to their wires 20. but the slopes of the helical course have the same slope amount.
In the embodiment described above, each web 7 of the basket 2 is formed by a respective separate wire section. The webs 7 were then respectively welded to the sleeves 6 and 15, so that the cup structure shown in the figures was formed. FIG. 8 shows a further preferred exemplary embodiment, which corresponds to the example from FIG. 1 with the exception of the configuration of the basket 2. In the example shown in Figure 8 go from the webs 7 at branching points 25 side webs 26, which are spot welded to the webs 7 at the branching points 25 and which were also spot welded to the sleeve 15. Furthermore, also the webs 7 at its proximal end with the sleeve 6 and at its distal end with the sleeve 15 were spot-welded, so that also the basket shown in Figure 8 was composed of several components.
FIG. 9 shows a further preferred exemplary embodiment of an instrument 1 according to the invention. This differs only by the embodiment of the basket 2 of the embodiments described above. The basket 2 shown in FIG. 9 also has webs 7 and secondary webs 26. The special feature is that all webs 7 and secondary webs 26 of the basket 2 are made in one piece with respect to one another. For this purpose, a tube made of a nickel-titanium alloy (so-called shape memory material) was selected as a semi-finished product and the contours of the webs 7 and secondary webs 26 were cut out by means of the laser cutting method. The webs 7 thus enter into the tube 6 at its proximal end in one piece and into the tube 15 at its distal end in the same way as the side webs 26. The branch webs 26 extend integrally from the webs 7 at the branching points 25. In the embodiment chosen in the example, it is provided that each web 7 splits at its branch point 25 in a total of three web branches, which is about the web branch passing straight through the branching point 25 and two branch branches branched off from it. With reference to Figures 10 and 11, a fourth preferred embodiment of a medical instrument 1 according to the invention will be described, again for the same or functionally corresponding features for a better overview, the reference numerals have been used for the previous Ausführungsbeispie- le used. While FIG. 11 shows a detail enlargement of detail XI in FIG. 10, two further detail enlargements have been taken directly in FIG. 10 for better illustration. Incidentally, while the transmission element is only shown in simplified form, the enlargement of section XII shows that the transmission element 4 again has the embodiment described with reference to FIG. 6 with two nested helical arrangements 21, 21 'whose wires 20 have mutually opposite slopes have. The basket 2 corresponds to the embodiment described with reference to FIG. 9, ie it was produced in one piece from a thin-walled tube of shape-memory material (for example from nitinol) by means of laser cutting. FIG. 11 also illustrates the internal configuration of the housing piece 23 "and of the clamping sleeve 31. In the housing piece 32, that of the central cavity 29 in the proximal direction widens first to form a cylindrical bore section 32 and then further to a threaded bore 30. In the example, the seal was made of silicone, and at both end faces rises an annular bead 34 running through the circumference, in each case, into the threaded bore 30 At the maximum depth of engagement, the seal 33 is slightly compressed by the threaded pin 27 in the direction of engagement, so that a resulting transverse deformation also causes a reduction in diameter at the central seal opening 35. If the screw-in depth is reduced again, the deformation of the seal is formed g 33 back, so that the seal opening again assumes its original size. FIG. 11 also shows that in the housing 23 "into the channel-like cavity 29, a side bore 36 opens, which leads to a hose connection 37 and serves from there to introduce a liquid, in particular contrast medium, into the hose-like casing 23. In Figures 10, 11 is the liquid transport within the outer casing 23 is shown only schematically by the arrows 38 indicating a liquid and its direction of movement. The liquid can thus be introduced into the instrument 1 by means of the hose connection 37 and passes through the side bore 36 first into the cavity in the region of the housing piece 23 " 29 and therein to the distal end 39 of the casing 23, where, as shown in the enlargement of section XII, the liquid can escape from the instrument 1 again. The cross-section of the channel-like cavity 29 depends on the course of the heat-sealed tubing 40 enveloping the transmission element 4 in the outer sheath 23. In the middle course of the cavity 29 has the shape of an annular gap. In order to prevent liquid from penetrating into the transmission element formed from twisted wires 20, in the exemplary embodiment shown in FIGS. 10, 11 the transmission element 4 is sheathed throughout its entire length with a shrinkage tube 40 which is non-displaceable in the longitudinal direction Transmission element 4 has shrunk. The shrink tube 40 extends in its extension also continuously over the tube 9 and ends, as Figure 11 shows in the upper part, only on the front side of the handling element 3. At the distal longitudinal end of the transmission element 4, the shrink tube 40 extends in extension to about one Proximal portion of the sleeve 6 of the basket 2. The diameter of the seal opening 35 is chosen so large that at any time, that is, even if the seal 33 is not compressed, a all-round sealing contact with the shrink tube 40 is ensured. Thus, it is prevented that liquid introduced through the side bore 36 can escape in the cavity 29 in the proximal direction. If the clamping sleeve 31 is screwed far into the threaded hole 30, so that it is compressed When the seal 33 arrives, it exerts a clamping force on the tube 9 sheathed with shrink tubing 40 so that it is fixed in a previously set axial relative position to the housing piece 23 " the compression of the seal 33, so that an axial relative movement between the fixedly connected to the tube 9 handling element 3 and the housing piece 23 "is possible while the sealing effect of the seal 33 is maintained.
