Description

  • Die Erfindung bezieht sich allgemein auf eine Vorrichtung zur Einführung eines Implantats und insbesondere auf eine Vorrichtung zur Einführung von Implantaten um die Fusion von benachbarten Knochenstrukturen zu vereinfachen, welche insbesondere einen Körper und einen Handgriffabschnitt umfasst. Der wesentlichste Stand der Technik ist das Dokument EP-A-0 716 840, welches unter Artikel 54(3) EPÜ fällt. Dieses Dokument zeigt keinen Randgriffabschnitt, der in lösbarer Weise in einen Körperabschnitt eingreift.
  • Eine grosse Anzahl von orthapädischen Operationen umfasst das Einfügen von entweder natürlichen oder prothetischen Implantaten in Knochen oder zugeordneten Geweben. Diese verfahren umfassen beispielsweise die Ligamentreparatur, die Gelenkreparatur oder den Gelenkersatz, nicht heilenden Knochenbrüche, die Gesichtsrekonstruktion, die Wirbelstabilisierung und die Wirbelfusion. Bei einer typischen Operation wird ein Einsatz, ein Zapfen oder eine Schraube in eine vorbereitete Bohrung eingeführt, die in dem Knochen oder den Geweben ausgebildet worden ist, um die Reparatur und die Heilung zu unterstützen. Siehe beispielsweise die US-Patente US 5,470,334 für Ross et al.; US 5,454,811 für Hübner, US 5,480,403 für Lee at al.; US 5,40,805 für Warren, US 5,358,511 für Gatturna et al.; und US 4,877,020 für Vich.
  • Einige Implantate sind insbesondere mit Öffnungen und Bohrungen ausgestaltet, um das Einwachsen von Knochen zu vereinfachen und das Verankern des Implantats am Einführungsort zu verbessern. Siehe beispielsweise die US-Patente US 4,328,593 für Sutter et al.; US 4,936,851 für Fox et al.; und US 4,878,915 für Branti gan. Implantate in Gestalt von Fusionskäfigen, die innere Hohlräume haben, um Knochenwachstumstimulierungsmaterialen wie Knochenspäne und Fragmente aufzunehmen, sind beispielsweise offenbart in US 4,501,269 für Bagby; US 4,961,740 für Ray et al.; US 5,015,247 für Michaelson; und US 5,489,307 für Kuslich et al.. Diese Arten von Implantaten sind insbesondere gut geeignet für intervertebrale Wirbelfusionsoperationen, die bei Verletzungen, Krankheiten und einigen degenerativen Störungen der Bandscheiben erforderlich sind. Nachfolgend kann eine progressive fortschreitende Degeneration zu mechanischer Instabilität zwischen benachbarten Wirbeln führen, was eine direkten Fusion der Wirbel erfordert, während ein vordefinierter Zwischenwirbelabstand erhalten bleiben muss. Diese Fusion kann durch das Einführen von einem oder mehreren speziellen Implantaten ausgeführt werden, wie es oben beschrieben worden ist und auch in dem Patent der Anmelderin US 5,025,373 .
  • Beide anteriore (transabdominale) und posteriore Operationsvorgänge werden für die Zwischenkörper-Fusionen der lumbalen Wirbel eingesetzt. Fusionen im zervikalen Bereich der Wirbelsäule werden überwiegend durch eine anteriore Vorgehensweise behandelt. Typischerweise wird ein Implantat wie ein Stopfen, Stift, Prothese oder Käfig in einen vorausgeformten Hohlraum innerhalb des Zwischenkörper-Zwischenscheibenwirbelraumes eingesetzt. Da es in diesem Verfahren wünschenswert ist, eine "Knochen-zu-Knochen" Brücke auszubilden, wird verbindendes Gewebematerial und mindestens ein Abschnitt des Bandscheibengewebes entfernt. vorzugsweise werden relativ tiefe Einschnitte in benachbarten Knochen ausgeführt, um in den weicheren besser durchbluteten spongiösen Bereich einzudringen, um das Knochenwachstum über das Implantat zu vereinfachen.
  • Wenn diese speziellen Implantate eingesetzt werden, wird ein Einführungswerkzeug eingesetzt, um das Implantat in dem gewünschten Zwischenwirbelraum anzuordnen. Siehe beispielsweise US 3,848,601 für Ma et al.; US 4,501,259 für Bagby; US 4,877,020 für Vich; und US 4,878,915 für Brantigan. Einmal in Position wird das Einführungswerkzeug Entfernt und, wenn es die Implantatstruktur gestattet, werden Knochenspäne und andere das Knochenwachstum induzierende Substanzen in vivo in das Implantat gepackt. Nachfolgend wird eine Endkappe oder eine andere abdichtende Struktur positioniert, um das Implantat zu schliessen. Siehe beispielsweise die der Anmelderin übertragenen US 4,961,740 für Ray et al..
  • Übliche Einführungswerkzeuge benutzen entweder eine einfache in das Implantat eingreifende Struktur oder höchstens zwei in das, Implantat eingreifende Strukturen, um das Positionieren des Implantats zu vereinfachen. Beispielsweise werden in US 4,501,269 für Bagby Zinken oder Stifte eingesetzt, um in das Implantat einzugreifen. Im US-Patent US 4,878,915 für Brantigan und US 5,015,247 für Michaelson wird ein gewindeter Stab und Schlitz eingesetzt, um in das Implantat einzugreifen, In den US-Patenten US 4,961,740 für Ray et al.; US 5,489,308 und US 5,489,307 , beide für Kuslich et al.; und in US 4,936,838 für Bagby wird ein einzelner zentraler Schaft eingesetzt. Bei all diesen Einführungswerkzeugen ist keine Struktur vorgesehen, um es dem Einführungswerkzeug zu erlauben, an einem äusseren peripheren Abschnitt des Implantats zu gelangen, weder in vitro noch in vivo. Weiterhin liefern diese Werkzeuge keine Struktur, die in getrennter Weise sowohl in das Implantat als auch in den Implantatsverschluss eingreift, beispielsweise eine Endkappe.
  • Demgemäss besteht ein Bedürfnis für ein Einführungswerkzeug, welches fähig ist, entweder ein mit Knochenspänen vorgefülltes Implantat einzuführen, etc.. Solch eine in vitro Füllfähigkeit vereinfacht das chirurgische Verfahren, weil es oft zeitraubend und relativ schwierig ist, insbesondere im Beispiel bei zervikalen Anwendungen, den Käfig in vivo zu füllen. Es wäre auch vorteilhaft, falls solch ein Einföhrungswerkzeug zusätzlich eingesetzt werden könnte, um ein leeres Implantat für nachfolgende in vivo Füllung und Verschliessung einzuführen und zu positionieren.
  • Die vorliegende Erfindung wird durch den unten stehenden Anspruch 1 definiert. Die abhängigen Ansprüche sind auf optionale oder bevorzugte Merkmale ausgerichtet. Die Vorrichtung in Übereinstimmung mit der Erfindung umfasst einen Handgriffabschnitt und einen Körperabschnitt, der mit dem Handgriffabschnitt befestigt ist und eine longitudinale Achse definiert. Der Körperabschnitt umfasst ein äusseres röhrenförmiges Element, welches relativ zum Handgriffabschnitt für die Drehung mit diesem um die longitudinale Achse fixiert ist. Das äussere röhrenförmige Element weist eine erste in das Implantat eingreifende Struktur auf, die dem distalen Ende benachbart ist. Ein inneres röhrenförmiges Element ist mindestens teilweise innerhalb des äusseren röhrenförmigen Elementes angeordnet und für eine longitudinale Bewegung relativ zum äusseren röhrenförmigen Element montiert. Die zweite in das Implantat eingreifende Struktur ist zu einem distalen Ende des inneren röhrenförmigen Elementes benachbart positioniert. Der Körperabschnitt umfasst weiterhin einen inneren Schaft, der koaxial mit mindestens einem Teil innerhalb des inneren röhrenförmigen Elementes für eine unabhängige Rotation relativ zu den inneren und äusseren röhrenförmigen Elementen angeordnet ist, wobei der innere Schaft eine dritte in das Implantat eingreifende Struktur benachbart zu einem distalen Ende auf weist.
  • Die Vorrichtung ist nützlich bei einem Verfahren zum Einsetzen eines Implantats, welches einen hohlen Abschnitt mit einem geschlossenen distalen Ende und eine entfernbare Kappe aufweist, wobei die ersten, zweiten und dritten in das Implantat eingreifende Strukturen mit dem Implantat verbunden sind, wobei mindestens eine der eingreifenden Strukturen an der entfernbaren Kappe befestigt ist und eine andere eingreifende Struktur an dem hohlen Abschnitt befestigt ist. Das Implantat ist vorzugsweise mit Knochenspänen vorgefüllt und/oder mit das Knochenwachstum induzierenden Substanzen, bevor es befestigt wird. Danach wird das vorgefüllte Implantat in den gewünschten Operationsraum eingeführt.
  • Kurze Beschreibung der Zeichnungen

  • Ausführliche Beschreibung von bevorzugten Ausführungsbeispielen
  • Die bevorzugten Ausführungsbeispiele der Vorrichtung, die hier beschrieben sind, werden in Bezug auf die orthopädische Wirbelsäulenfusion und die entsprechenden Verfahren und Vorrichtungen erläutert. Es ist auch angedacht, dass die Vorrichtung für eine grosse Breite von Verfahren anwendbar ist, die die Ligamentreparatur, die Gelenkreparatur oder den Gelenkersatz, die nicht heilende Brüche, die Gesichtsrekonstruktion und die Wirbelsäulenstabilisierung umfassen, aber nicht auf diese begrenzt sind. Zusätzlich wird angenommen, dass die vorliegende Vorrichtung in sowohl offenen als auch minimal invasiven Verfahren angewendet werden kann, inklusive der endoskopischen und arthroskopischen Verfahren, wobei der Zugriff auf den Operationsort durch eine Kanüle oder einen kleinen Einschnitt erreicht wird.
  • In der folgenden Beschreibung bezieht sich der Begriff "proximal" wie üblich auf den Teil der Struktur, welche näher zum Operateur ist, während sich der Begriff "distal" auf den Teil bezieht, der weiter entfernt vom Operateur liegt.
  • Unter Bezug auf die Zeichnungen, in denen gleiche Bezugszeichen ähnliche oder identische Elemente bezeichnen, ist ein bevorzugtes Ausführungsbeispiel der Implantatseinführungsvorrichtung in den 1 und 2 dargestellt und allgemein durch das Bezugszeichen 100 gekennzeichnet. Die Implantatseinführungsvorrichtung 100 umfasst einen entfernbaren Handgriffabschnitt 102 und einen Körperabschnitt 104. Der Handgriffabschnitt 102 verfügt über einen T-förmigen Handgriff 105, der an einem proximalen Ende angeordnet ist, und über eine lösbare Eingriffstruktur 108 an einem distalen Ende davon.
  • Der Körperabschnitt 104 definiert eine longitudinale Achse "L" und umfasst ein äusseres röhrenförmiges Element 110, welches an einem Gehäuse 112 für eine longitudinale Drehung mit ihm fixiert ist. Die erste in das Implantat eingreifende Struktur 114 ist benachbart zu einem distalen Ende des äusseren röhrenförmigen Elementes positioniert. Das äussere röhrenförmige Element 110 umfasst vorzugsweise einen entfernbaren distalen Endabschnitt 158, der weiter unten beschrieben wird. In dem dargestellten Ausführungsbeispiel ist die in das Implantat eingreifende Struktur ein paar von sich distal erstreckenden Reitern 116, die sich von dem distalen Ende des äusseren röhrenförmigen Elementes 110 wegerstrecken.
  • Der Körperabschnitt 104 umfasst weiterhin ein inneres röhrenförmiges Element 118, das mindestens teilweise innerhalb des äusseren röhrenförmigen Elementes 110 angeordnet ist. Das innere röhrenförmige Element 118 ist für eine begrenzte longitudinale Bewegung relativ zum äusseren röhrenförmigen Element 130 angeordnet und ist in unabhängiger Weise relativ zum äusseren röhrenförmigen Element 110 durch das Rad 120 drehbar. In dem dargestellten Ausführungsbeispiel verankert die Schraube 122 das Rad 120 in dem inneren röhrenförmigen Element 118. Ein erster Hohlraum 124 ist in dem Gehäuse 112 definiert und ist so dimensioniert, um die relative longitudinale Bewegung des inneren röhrenförmigen Elementes 118 durch das Eingrenzen des Rades 120 zwischen den Wänden 112A und 112B zu begrenzen. Wie in der 1 dargestellt, ist die Umgebung des Rades 120 mit Erhebungen 126 versehen, um die Reibungscharakteristika zu verbessern.
  • Das innere röhrenförmige Element 118 ist normalerweise an einer distal entferntesten longitudinalen Position relativ zum äusseren röhrenförmigen Element 110 wie in 2 dargestellt vorgespannt. Die Springfeder 127, die im Gehäuse 112 montiert ist, stösst gegen ein proximales Ende des inneren röhrenförmigen Elementes 118 und spannt das innere röhrenförmige Element distal vor. Die zweite in das Implantat eingreifende Struktur 128 ist benachbart zu einem distalen Ende des inneren röhrenförmigen Elementes 118 ausgebildet. In dem dargestellten Ausführungsbeispiel ist die zweite in das Implantat eingreifende Struktur 128 eine hohle polygonale Struktur mit einer Vielzahl von flachen Seiten 130, die sich von dem distalen Ende des inneren röhrenförmigen Elementes 118 weg erstrecken.
  • Der Körperabschnitt 104 umfasst auch einen inneren Schaft 132, der in koaxialer Weise mindestens teilweise innerhalb des inne ren röhrenförmigen Elementes 118 aufgebaut ist. Der innere Schaft 132 ist durch das Rad 134 in longitudinaler Weise relativ zum inneren röhrenförmigen Element 118 und zum äusseren röhrenförmigen Element 110 drehbar. Wie dargestellt, verbindet die Schraube 136 das Rad 134 mit einem distalen Ende des inneren Schaftes 132. Bei einem bevorzugten Ausführungsbeispiel ist der äussere Umkreis des Rades 134 mit einer die Reibung verbessernden Oberfläche wie Erhebungen 138 versehen.
  • Ein zweiter Hohlraum 140 ist in dem Gehäuse 112 proximal zum ersten Hohlraum 124 definiert und dient dazu, ein Rad 134 einzugrenzen und damit den inneren Schaft 132, um die longitudinale Bewegung relativ zu den äusseren und inneren röhrenförmigen Elementen 110 und 118 jeweils zwischen den Wänden 112c und 112d zu begrenzen. Die Räder 120 und 134 erstrecken sich vorteilhafterweise radial über mindestens eine Umkreisoberfläche des Gehäuses 112, um die Betätigung durch die Finger eines Benutzers zu vereinfachen.
  • Der innere Schaft 132 verfügt über dritte in das Implantat eingreifende Strukturen 142, die benachbart zu einem distalen Ende positioniert sind (siehe 2 und 6A). Bei dem dargestellten Ausführungsbeispiel umfasst die Struktur Gewinde 144, die auf einem distalen Ende des inneren Schaftes 132 ausgebildet sind.
  • Unter Bezugnahme auf die 2 und 2A umfassen lösbare Eingriffstrukturen 108 des dargestellten Ausführungsbeispiels der Implantatseinführungsvorrichtung 100 eine distale Hülse 146 mit einer inneren Oberfläche, die so ausgestaltet ist, um den hexagonalen Ansatz 148 zu haben, der an einem proximalen Ende des Gehäuses 112 ausgebildet ist.
  • Die proximale Hülse 150 ist in operativer weise mit der distalen Hülse 146 verbunden und umfasst ein inneres federvorgespanntes Kugelsystem 152, welches dazu ausgelegt ist, in lösbarer Weise in einen ringförmigen Kanal 154 einzugreifen, der sich proximal gegenüber dem hexagonalen Vorsprung 148 erstreckt. Die proximale Hülse 150 ist auf einem distalen Ende des Handgriffabschnittes 102 für die relative longitudinale Bewegung zwischen einer verriegelten Position (2) und einer nicht verriegelten Position (2A) zu bewegen. In der verriegelten Position ist das Kugelsystem 152 radial nach innen in den ringförmigen Kanal 154 gezwungen. Die Feder 156 spannt normalerweise die proximale Hülse 150 in der verriegelten Position vor. Wie in der 2A dargestellt ist, in der nichtverriegelten Position die proximale Hülse 150 zurückgezogen, um das Kugelsystem 152 aus dem Ringkanal 154 freizugeben. Dies ermöglicht das Entfernen des Handgriffabschnittes 102 von dem Körperabschnitt 104, so dass der Handgriffabschnitt mit anderen Instrumentarien verbunden werden kann, die notwendig sind, um die Operation durchzuführen.
  • Die 2B und 2C unter Berücksichtigung der 1 zeigen ein breit anwendbares Merkmal des dargestellten Ausführungsbeispiels. Bei diesem bevorzugten Ausführungsbeispiel ist ein distaler Endabschnitt 158 des äusseren röhrenförmigen Elementes 110, welcher die erste in das Implantat eingreifende Struktur 114 umfasst, in austauschbarer Weise über einen Reibschluss befestigt. Dies gestattet es dem Benutzer, einen anderen distalen Endabschnitt 160 zu positionieren, wie es in der 2C gezeigt ist, und entweder ein Implantat von unterschiedlicher Grösse (beispielsweise im Durchmesser) zu montieren oder die Konfiguration der in das Implantat eingreifenden Struktur für dieselbe Implantatseinführungsvorrichtung zu ändern. Daher kann die Einführungsvorrichtung in einfacher Weise für das Einsetzen von verschiedenen Implantaten angepasst werden.
  • Ein Implantat, welches für den Einsatz in Wirbelfusionsverfahren nach 3 und 5 ausgestaltet ist, wird im Allgemeinen mit dem Bezugszeichen 200 gekennzeichnet. Dieses Implantat wird allgemein als "Fusionskäfig" bezeichnet und ist in diesem Ausführungsbeispiel speziell für einen posterioren Zugang bei der Wirbelsäulenfusionsoperation ausgelegt. Die nachfolgende Diskussion in Bezug auf einen beispielhaften Einsatz des Implantatseinführungswerkzeuges 100 wird auf diese posteriore Wirbelfusionsoperation fokussiert, bei der der Fusionskäfig 200 eingeführt wird. Es wird jedoch festgehalten, dass das beschriebene Implantatseinführungswerkzeug eine breite Anwendung bei einer grossen Vielzahl von Implantatseinführungsverfahren jenseits von der anterioren oder posterioren Wirbelfusion aufweist.
  • Der Fusionskäfig 240 umfasst einen Körperabschnitt 202 mit einem geschlossenen distalen Ende 204 und einem proximalen Ende 206. Das distale Ende 204 ist abgerundet oder weist eine runde Nase auf, um die Einführung des Fusionskäfigs 200 in Bezug auf eine oder mehrere Knochenstrukturen zu vereinfachen. Das proximale Ende 206 begrenzt eine Öffnung 248 (6), die mit einem inneren Hohlraum 210 (5) kommuniziert, welche durch den Fusionskäfig 200 definiert ist. In dem dargestellten Ausführungsbeispiel ist die Öffnung 208 mit einem Gewinde versehen, um eine Endkappe 212 aufzunehmen. Diese Endkappe 212 wird eingesetzt, um das proximale Ende 206 zu schliessen und um die das Knochenwachstum induzierenden Substanzen zurückzuhalten, wie beispielsweise Knochenspäne 214 (6), die hier eingefüllt sind.
  • Unter Bezugnahme auf die 4 definiert die Endkappe 212 eine gewindete Bohrung 215, die so ausgestaltet ist, um eine dritte in das Implantat eingreifende Struktur 142 des inneren Schaftes 132 aufzunehmen, wie im weiteren Detail unten beschrieben wird.
  • Die Endkappe 212 definiert auch eine im wesentlichen rechteckige Vertiefung 218, die koaxial mit der gewindeten Bohrung 216 ist und so ausgestaltet ist, um die zweite in das Implantat eingreifende Struktur 128 auf dem inneren röhrenförmigen Element 118 aufzunehmen.
  • Das proximale Ende 206 definiert weiterhin erste und zweite Umkreisausnehmungen 220, 222, die um die transversale Achse (T) zentriert sind. Diese umkreisförmigen Ausnehmungen 220, 222 sind so ausgestaltet, um die erste in das Implantat eingreifende Struktur 114, in diesem Fall Reiter 116, aufzunehmen. Diese Ausnehmungen können auch eingesetzt werden, um den Fusionskäfig 200 für die richtige Einführung und Anordnung zwischen den benachbarten Wirbelstrukturen auszurichten.
  • Eine Gewindehelix 224 ist auf der äusseren Umkreisoberfläche des Fusionskäfiges 200 ausgebildet. Eine Vielzahl von Öffnungen 226 sind über und durch den Fusionskäfig 200 definiert. In dem dar gestellten Fusionskäfig 200 sind die Schlitze 226 durch ausgedornte Nuten 228 (5) in der inneren Oberfläche 230 des inneren Hohlraumes 210 ausgebildet. Diese Technik entfernt Material aus den Tälern zwischen den Gängen des Gewindes 224 und so werden die Öffnungen 226 definiert, um in vorteilhafter Weise den direkten Kontakt zwischen dem Wirbelkörper und den Knochenspänen zu liefern, die innerhalb des Käfigs angeordnet sind, wenn der Käfig in dem Körper positioniert wird.
  • Unter Bezugnahme auf die 6, 6A und 7 werden zwei Verfahren des Verschliessens der Endkappe 212 in der Öffnung 208 des proximalen Endes 206 dargestellt. In der 6 werden Knochenspäne 214 in dem inneren Hohlraum 210 unter Einsatz von Klemmen abgelagert. Daraufhin wird die Endkappe 212 in manueller Weise in die Öffnung 208 eingeschraubt, entweder per Hand oder mit ei nem schraubenschlüsselähnlichen Instrument. In einer alternativen Ausgestaltung kann die Endkappe 212 im Eingriff mit der zweiten und der dritten in das Implantat eingreifenden Struktur 128, 142 der Implantatseinführungsvorrichtung 100 ausgerichtet werden. Dies wird ausgeführt durch Positionierung der flachen Seiten 130 der zweiten in das Implantat eingreifenden Struktur 128 in eine rechteckige Ausnehmung 218 der Endkappe 212. Das distale Ende des inneren Schafts 132 wird dann in die Bohrung 216 der Endkappe 212 durch ein sich drehendes Rad 134 (1) eingeschraubt. Die Endkappe 212 wird dann in sicherer Weise durch die zweite und dritte in das Implantat Eingreifende Struktur 128, 142 gesichert. Die Implantatseinführungsvorrichtung 100 wird mit der im Eingriff stehenden Endkappe 212 in übereinstimmender axialer Ausrichtung mit der Öffnung 208 in dem proximalen Ende 206 des Fusionskäfigs 200 positioniert. Die Drehung des Rades 120 schraubt die Endkappe 212 in den Fusionskäfig 200 ein. Wie dargestellt wird in beiden Verfahren die Füllung des Käfigs ausserhalb des Körpers ausgeführt. Dies vereinfacht das Einführen von Knochenspänen, da die Späne in individueller Weise mit einer Klammer gegriffen und in den Käfig leicht angedrückt (das heisst kompaktiert) werden. Dies ist insbesondere vorteilhaft, wenn der Zugang zum Käfig, sobald er implantiert ist, begrenzt ist oder bei kleiner Käfigen wie bei zervikalen Anwendungen.
  • Das Montieren eines gefüllten Fusionskäfigs (7) auf die Einführungsvorrichtung 100 und die nachfolgende Einführung in einen Zwischenwirbelraum wird nun unter Bezugnahme auf die 812 beschrieben. In den 8A und 8B ist der gefüllte Fusionskäfig in axialer Ausrichtung mit dem proximalen Ende des Fusionskäfigs 200 positioniert; ausrichtende Reiter 116 mit Ausnehmungen 220, 222, flache Seiten 130 mit einer rechteckigen Ausnehmung 218; und Gewinde 144 mit gewindeten Bohrungen 216.
  • Unter Bezugnahme auf die 9A, 9B, 10A und 10B wird der Fusionskäfig 200 ursprünglich in den Eingriff mit dem inneren röhrenförmigen Element 118 bewegt, so dass die flachen Seiten 130 in der rechteckigen Ausnehmung 218 der Endkappe 212 angeordnet sind. Eine weitere proximale Bewegung (durch die Pfeile in den 9A und 9B dargestellt) des inneren Schaftes 118 relativ zum inneren Schaft 132 durch Pressen des Käfigs 200 gegen die Vorrichtung oder das proximale Bewegen des Rades 120 bringt die Gewinde 144 in den Eingriff mit der gewindeten Bohrung 216 und die Reiter 116 des äusseren röhrenförmigen Elementes 110 in gleichzeitigen Eingriff mit den Ausnehmungen 220 und 222 (10A). Das Rad 120 kann leicht gedreht werden, um die Ausrichtung der Reiter 116 und der Ausnehmungen 220, 222 zu gewährleisten. Dann wird das Rad 134 gedreht, so dass bewirkt wird, dass der gewindete innere Schaft in den Fusionskäfig 200 über die Endkappe 212 eingreift und so in sicherer weise den Fusionskäfig 200 auf dem distalen Ende der Implantatseinführungsvorrichtung 100 montiert, solange der Käfig 200 in proximaler Weise über den Eingriff der Gewinde gezogen wird.
  • Danach wird die Implantatseinführungsvorrichtung 100 benachbart zum Implantatsort (11) positioniert, was in üblicher Weise eine Gewindebohrung umfasst, die in einem Zwischenwirbelraum zwischen den zwei benachbarten wirbeln ausgebildet wird. (Alternativ kann der Fusionskäfig auch selbstschneidend sein). Die Implantatseinführungsvorrichtung 100 kann in die richtige Position unter Einsatz einer Kanüle oder eines C-Retractors 300 geführt werden, um die korrekte Einführung des Fusionskäfigs 200 zu vereinfachen. Der T-Handgriff 106 wird dann gedreht, um das äussere röhrenförmige Element 110 zu drehen, dass dieses in die Gewinde 224 des Fusionskäfigs 200 (strichliniert dargestellt) in dem Zwischenwirbelraum 302 eingreift.
  • Sobald der Fusionskäfig 200 in der richtigen Position ist, wird das Rad 134 gedreht, um die Gewinde 144 von der gewindeten Bohrung 216 zu entfernen. Dies befreit die Implantatseinführungsvorrichtung 100 von dem implantierten Fusionskäfig 200 (12).
  • Es ist festzuhalten, dass der Fusionskäfig 200 durch die Vorrichtung 100 von ihrem offenen Ende ergriffen und eingeführt wird, dass sobald er sich im Körper befindet, die Endkappe 212 entfernt werden kann, falls der Benutzer es wünscht, die Knochenspäne in dem inneren Hohlraum 210 zu sehen oder auf diese Zugriff zu haben.
  • Die Implantatseinführungsvorrichtung 100 kann auch. eingesetzt werden, um ein leeres Implantat wie einen Fusionskäfig 200 in den Wirbelzwischenraum einzuführen und nachfolgend den Fusionskäfig nach der Auffüllung des Fusionskäfigs mit Knochenwachstum induzierende Substanzen in vivo zu füllen. Bei diesem Verfahren wird der leere Fusionskäfig mit den ersten, zweiten und dritten in das Implantat eingreifenden Strukturen 114, 128 und 142 in Eingriff gebracht und in derselben Art und Weise wie oben beschrieben eingeführt. Sobald dieser im Körper an Ort und Stelle ist, wird das Rad 120 gedreht, um die Endkappe 212 aus dem Körperabschnitt 202 zu entfernen, und die Implantatseinführungsvorrichtung mit der befestigten Endkappe 212 (6A) wird von dem Ort entfernt.
  • Danach kann der Fusionskäfig 200 gefüllt werden und das Verfahren wird umgekehrt, um die Endkappe 212 in sicherer weise zurück auf den Körperabschnitt 202 zu schrauben und unter Einsatz der vorrichtung 100 wie oben beschrieben ohne die relative Position des Körperabschnitts 202 an dem Ort zu beeinflussen. Dies würde die Notwendigkeit für ein getrenntes Kappeneinführungswerkzeug vermeiden, da die Vorrichtung 100 die doppelte Funktion des Einführens von Käfig und Befestigen der Endkappe ausfüllen könnte.
  • Es ist wohl verstanden, dass eine vielzahl von Änderungen an den Ausführungsbeispielen der hier beschriebenen Vorrichtung gemacht werden können. Beispielsweise können die ersten, zweiten und/oder dritten in das Implantat eingreifenden Strukturen modifiziert werden, um den Eingriff mit einer grossen Anzahl von Implantaten, sowohl prothetischer als auch natürlicher Art zu vereinfachen. Auch können endoskopische, arthroskopische und perkutane Verfahren des Einsatzes in einfacher Weise angepasst werden. Daher sollte die obige Beschreibung nicht als limitierend angesehen werden, sondern nur als Darstellung der bevorzugten Ausführungsbeispiele.

Claims (12)

  1. Vorrichtung zum Einsetzen eines Implantats: – mit einem Handgriffabschnitt (108), und – mit einem Körperabschnitt (104), der mit dem Handgriffabschnitt verbunden ist und eine longitudinale Achse definiert, wobei der Körperabschnitt ein äusseres röhrenförmiges Element (110) umfasst, welches relativ zum Handgriffabschnitt für eine Drehung mit diesem um die longitudinale Achse fixiert ist, wobei das äussere röhrenförmige Element eine erste in ein Implantat eingreifende Struktur (114) aufweist, die zu einem distalen Ende benachbart ist, wobei ein inneres röhrenförmiges Element (118) zumindest teilweise innerhalb des äusseren röhrenförmigen Elementes angeordnet ist und für eine longitudinale Bewegung relativ zu dem äusseren röhrenförmigen Element angeordnet ist, wobei das innere röhrenförmige Element unabhängig von dem äusseren röhrenförmigen Element rotierbar ist und eine zweite in ein Implantat eingreifende Struktur (128) aufweist, welche benachbart zu einem distalen Ende und einer inneren Welle (132) ist, die koaxial mindestens teilweise innerhalb des inneren röhrenförmigen Elementes für eine unabhängige Drehung relativ zu den inneren und äusseren röhrenförmigen Elementen angeordnet ist, wobei die innere Welle die dritte in ein Implantat eingreifende Struktur (142) aufweist, welche zu einem distalen Ende benachbart ist, wobei der Handgriffabschnitt in lösbarer Weise an dem Körperabschnitt befestigt ist und von dem Körperabschnitt durch das Mittel einer lösbaren Eingriffsstruktur (108, 146, 148, 152, 154) entfernbar ist, welche zwischen einer verriegelten Position (2) und einer nichtverriegelten Position (2A) bewegbar ist.
  2. Vorrichtung nach Anspruch 1, bei der die Eingriffsstruktur einen ringförmigen Kanal (154) umfasst, der in dem Körperabschnitt definiert ist, und ein federgespanntes Kugelsystem (152) umfasst, welches von dem Handgriffabschnitt getragen wird und so ausgestaltet ist, um in lösbarer Weise in der verriegelten Position in den besagten ringförmigen Kanal einzugreifen.
  3. Vorrichtung nach einem der vorstehenden Ansprüche, welche weiterhin ein Rad (120) umfasst, welches auf dem inneren röhrenförmigen Element für eine unabhängige Drehung von diesem relativ zu dem äusseren röhrenförmigen Element angeordnet ist.
  4. Vorrichtung nach einem der vorstehenden Ansprüche, welche weiterhin ein Rad (134) umfasst, welches auf der inneren welle für eine unabhängige Drehung von dieser relativ zu den äusseren und inneren röhrenförmigen Elementen angeordnet ist.
  5. Vorrichtung nach einem der vorstehenden Ansprüche, bei der die erste in ein Implantat eingreifende Struktur ein paar von Ansätzen (116) umfasst, die von dem distalen Ende des äusseren röhrenförmigen Elementes ausgehen.
  6. Vorrichtung nach einem der vorstehenden Ansprüche, bei der die zweite in ein Implantat eingreifende Struktur eine polygonale Struktur (130) umfasst, die sich von dem distalen Ende des inneren röhrenförmigen Elementes wegerstreckt.
  7. Vorrichtung nach einem der vorstehenden Ansprüche, bei der die dritte in ein Implantat eingreifende Struktur einen gewindeten Abschnitt (144) umfasst, der auf dem distalen Ende der inneren Welle ausgebildet ist.
  8. Vorrichtung nach einem der vorstehenden Ansprüche, bei der die erste in ein Implantat eingreifende Struktur in entfernbarer Weise auf dem Körperabschnitt angeordnet ist.
  9. Vorrichtung nach einem der vorstehenden Ansprüche, bei der der Handgriffabschnitt T-förmig ist.
  10. Vorrichtung nach Anspruch 5, bei der die Fortsätze auf radial gegenüberliegenden Seiten der longitudinalen Achse und so ausgestaltet sind, um in eine äussere Umkreiswand eines Implantats einzugreifen.
  11. Vorrichtung nach Anspruch 6, bei der die polygonale Struktur auf dem distalen Ende des inneren röhrenförmigen Elementes angeordnet ist, um in eine proximale Endkappe eines Prothesenimplantats einzugreifen.
  12. Vorrichtung nach einem der vorstehenden Ansprüche, bei der der gewindete Abschnitt, der auf dem distalen Ende der inneren Welle ausgebildet ist, so ausgestaltet ist, um in Verschraubbarerweise in eine proximale Endkappe eines Prothesenimplantats einzugreifen.
