Description

  • Die vorliegende Erfindung bezieht sich allgemein auf therapeutische expandierbare Stentprothesen zur Implantation in Körperhohlräume, beispielsweise vaskulare Implantation, und insbesondere auf ein Verfahren zur Bereitstellung biostabiler, elastomerer Beschichtungen auf diesen Stents, die biologisch aktive Spezies mit kontrollierten Freisetzungseigenschaften direkt in die Beschichtungsstruktur einbringen.
  • Bei chirurgischen oder anderen verwandten invasiven medizinischen Verfahren sind die Einführung und die Expansion von Stentvorrichtungen in Blutgefäße, Harnwege oder andere schwer zugängliche Stellen um Restenose zu verhindern, um Gefäß- oder Hohlraumwandstützung oder -verstärkung bereitzustellen und für andere therapeutische oder wiederherstellende Funktionen eine allgemeine Form der Langzeitbehandlung geworden. Typischerweise werden diese Prothesen an der Stelle von Interesse unter Verwendung eines vaskulären Katheters oder einer ähnlichen transluminalen Vorrichtung angewendet, um den Stent zu der Stelle von Interesse zu tragen, wo er danach freigesetzt wird, um zu expandieren oder in situ expandiert zu werden. Diese Vorrichtungen sind im Allgemeinen als Dauerimplantate konstruiert, die in Gefäße oder anderes Gewebe, welches sie bei der Implantation kontaktieren, eingeführt werden können.
  • Ein Typ eines selbstexpandierenden Stents weist einen flexiblen Röhrenkörper auf, der aus mehreren einzelnen, flexiblen Fadenelementen gebildet ist, wobei sich jeder in einer Helixkonfiguration mit der Mittellinie des Körpers, die als eine gewöhnliche Achse dient, erstreckt. Die Elemente sind in normaler Richtung gewunden, aber sind axial in Bezug zueinander verschoben und treffen sich unter Vernetzung einer ähnlichen Anzahl von Elementen, die ebenso axial verschoben sind, aber die entgegengesetzte Richtung der Windung aufweisen. Diese Konfiguration stellt eine elastische, geflochtene Röhrenstruktur bereit, die stabile Dimensionen bei der Relaxation annimmt. Die axiale Spannung erzeugt Dehnung und entsprechende Durchmesserkontraktion, was die Befestigung des Stents an einer Kathetervorrichtung und die Beförderung durch das Gefäßsystem als eine enge in die Länge gezogene Vorrichtung ermöglicht. Wenn die Spannung einmal in situ relaxiert, kehrt die Vorrichtung zumindest weitestgehend zu ihrer Ursprungsform zurück. Die Prothesen der Klasse, die einen geflochtenen, flexiblen Röhrenkörper einschließen, werden in den US-Patenten 4,655,771 und 4,954,126 von Wallsten und 5,061,275 von Wallsten et al. dargestellt und beschrieben.
  • Implantierte Stents sind ebenso verwendet worden, um Medikamente wie thrombolytische Mittel, zu tragen. US-Patent 5,163,952 von Froix offenbart eine thermische, speichernde, expandierende Kunststoffstentvorrichtung, die formuliert werden kann, um ein Medikament unter Verwendung des Materials des Stents selbst als einen inerten polymeren Arzneimittelträger zu tragen. Pinchuk offenbart in US-Patent 5,092,877 ein Stent aus einem polymeren Material, welches mit einer Beschichtung, die mit der Abgabe der Arzneimittel verbunden ist, eingesetzt werden kann. Andere Patente, die sich auf Vorrichtungen der Klasse unter Verwendung von biologisch abbaubaren oder biosorbierbaren Polymeren richten, umfassen Tang et al., US-Patent 4,916,193 , und MacGregor, US-Patent 4,994,071 . Sahatjian offenbart in US-Patent Nr. 5,304,121 eine Beschichtung, die auf ein Stent aufgetragen wird, der aus einem Hydrogelpolymer und einem vorausgewählten Arzneimittel besteht; mögliche Arzneimittel umfassen Zellwachstumsinhibitoren und Heparin. Ein weiteres Verfahren zur Herstellung eines beschichteten intravaskularen Stents, der ein therapeutisches Mittel trägt, bei dem eine Polymerbeschichtung in einem Lösungsmittel gelöst wird, und das therapeutische Material in dem Lösungsmittel dispergiert wird, und das Lösungsmittel danach eindampft, wird in Berg et al., US-Patent 5,464,650 , erteilt am 5. November 1995, das der Europäischen Patentanmeldung 0 623 354 A1 , veröffentlicht am 9. November 1994, entspricht, beschrieben.
  • Ein Artikel von Michael N. Helmus (ein Miterfinder der vorliegenden Erfindung) mit dem Titel ”Medical Device Design - A Systems Approach: Central Venous Catheters”, 22.   • Die obige querverwiesene übergeordnete Stammanmeldung liefert einen Ansatz, der die Langzeitarzneimittelfreisetzung bereitstellt, d. h. über einen Zeitraum von Tagen oder sogar Monaten, eingeschlossen in einem kontrollierten Freisetzungssystem. Die Stammanmeldung und vorliegende Erfindung stellen ein Verfahren zur Beschichtung dieser Stents bereit, einschließlich Techniken, die die anfängliche Explosionswirkung der Arzneimittelfreisetzung kontrollieren und die Modifikation des kinetischen Arzneimittelfreisetzungsprofils, das mit der therapeutischen Langzeitwirkung verbunden ist, ermöglichen können.
  • Metallstents von ähnlicher Dicke und Flechtung weisen im Allgemeinen bessere mechanische Eigenschaften als polymere Stents auf. Metallische Gefäßstents, die aus sogar relativ feinen Metallfäden geflochten sind, können eine große Festigkeit bereitstellen, um dem nach innen gerichteten Umfangsdruck in Blutgefäßen standzuhalten. Um einem Polymermaterial vergleichbare Festigkeitseigenschaften zu geben, ist eine viel dickwandigere Struktur oder ein schwereres, dichteres Fadengeflecht erforderlich. Dies verringert wiederum den Querschnitt, der für den Fluss durch den Stent verfügbar ist, und/oder verringert die relative Menge an offenem Raum, der in der Struktur verfügbar ist. Wenn überhaupt anwendbar, ist es normalerweise schwieriger, polymere Stents unter Verwendung von vaskularen Katheterabgabesystemen einzubringen und abzugeben.
  • Es wird jedoch angemerkt, dass während bestimmte Typen von Stents, wie geflochtene Metallstents, besser als anderen für einige Anwendungen sein können, das erfindungsgemäße Verfahren in dieser Hinsicht nicht eingeschränkt ist, und verwendet werden kann, um eine breite Vielzahl von Vorrichtungen zu beschichten. Die vorliegende Erfindung trifft ebenso beispielsweise auf die Klasse von Stents zu, die nicht selbst expandierend sind, einschließlich denen, die beispielsweise mit einem Ballon expandiert werden können. Polymere Stents von allen Arten können unter Verwendung des Verfahrens beschichtet werden. Daher wird trotz speziell ausführlicher Ausführungsformen die Verwendung der Erfindung in Bezug auf entweder die Stentkonstruktion oder die Konstruktionsmaterialen nicht eingeschränkt oder soll eingeschränkt werden. Außerdem kann die vorliegende Erfindung mit anderen Typen von Implantatprothesen verwendet werden.
  • Folglich ist es ein primärer Gegenstand der vorliegenden Erfindung, beschichtete, implantierbare Prothesen bereitzustellen, wobei die Beschichtung zur Langzeitabgabe von biologisch aktiven Materialien fähig ist.
  • Ein weiterer Gegenstand der vorliegenden Erfindung ist, die Aspekte der zeitlich abgestimmten oder zeitvariablen Arzneimittelabgabe von einer Stentbeschichtung durch Kontrollieren der durchschnittlichen Teilchengröße in den biologisch aktiven Spezies zu kontrollieren oder zu modifizieren.
  • Andere Gegenstande und Vorteile der vorliegenden Erfindung werden dem Fachmann aus der Bekanntmachung mit der Beschreibung und den anhängenden Ansprüchen offensichtlich sein.
  • ZUSAMMENFASSUNG DER ERFINDUNG
  • Die vorliegende Erfindung stellt eine beschichtete, implantierbare Prothese wie in den Ansprüchen definiert bereit.
  • Das elastomere Material der ersten Schicht ist bevorzugt hydrophob, und das biologisch aktive Material wird darin für dessen zeitlich abgestimmte Abgabe verteilt.
  • Die bevorzugte Prothese ist eine selbstexpandierende Röhrenstentprothese mit offenem Ende. Obwohl andere Materialien, einschließlich Polymermaterialien, verwendet werden können, wird in der bevorzugten Ausführungsform der Röhrenkörper aus einem offenen Geflecht aus feinem Einzel- oder Mehrfadenmetalldraht geformt, der sich ohne Zusammenfallen biegt, und ohne Weiteres axial zu einer länglichen Form zur transluminalen Insertion über einen vaskularen Katheter verformt. Der Stent versucht federnd, die vorbestimmten stabilen Dimensionen bei der Relaxation in situ wieder anzunehmen. Die erste Deckschicht wird bevorzugt als ein homogenes Gemisch, eine Lösung oder Suspension aus polymerem Material und fein verteilten, biologisch aktiven Spezies, die in einem organischen Vehikel oder einer homogenen Lösung verteilt sind, oder Teillösung aus diesen Spezies in einem Lösungsmittel oder Vehikel für das Polymer und/oder die biologisch aktive Spezies aufgetragen. Die durchschnittliche Teilchengröße der biologisch aktiven Spezies beträgt weniger oder gleich 15 μm. Für die Zwecke dieser Anmeldung bedeutet der Ausdruck ”fein verteilt” jede(n) Typ oder Größe innerhalb dieses Bereiches von eingeschlossenem Material von gelösten Molekülen bis Suspensionen, Kolloiden und partikularen Gemischen. Das aktive Material wird in einem Trägermaterial, das das Polymer, ein Lösungsmittel oder beides sein kann, verteilt. Die Beschichtung wird bevorzugt als eine Vielzahl von relativ dünnen Schichten im Wesentlichen in relativ schneller Reihenfolge aufgetragen, und wird bevorzugt mit dem Stent in einem radial expandierten Zustand angewandt. In einigen Anwendungen kann die Beschichtung weiter als eine zusammengesetzte anfängliche Bindeschicht oder Grundschicht und eine zusammengesetzte Deckschicht charakterisiert werden. Das Beschichtungsdickenverhältnis der Deckschicht zu der Grundschicht kann mit der gewünschten Wirkung und/oder dem Elutionssystem variieren. Typischerweise sind diese von unterschiedlichen Formulierungen.
  • Die Beschichtung kann durch Eintauchen oder Sprühen unter Verwendung von Verdampfungslösungsmittelmaterialien mit relativ hohem Dampfdruck aufgetragen werden, um die gewünschte Viskosität herzustellen und die Deckschichtdicken schnell herzustellen. Das bevorzugte Verfahren basiert auf gegenseitigem Spritzen eines rotierenden, radial expandierten Stents unter Einsatz einer Feinspritzvorrichtung. Das Beschichtungsverfahren ermöglicht die haftende Anpassung des Materials, und deckt die gesamte Oberfläche der Faden der offenen Struktur des Stents ab, aber in einer Weise, so dass die offene Gitterbeschaffenheit der Struktur des geflochtenen oder anderen Musters in der beschichteten Vorrichtung bewahrt wird.
  • Die Beschichtung wird der Belüftung bei Raumtemperatur für eine vorbestimmte Zeit (möglicherweise eine Stunde oder mehr) zur Lösungsmittelvehikelverdampfung ausgesetzt. Danach wird das polymere Präkursormaterial bei Raumtemperatur oder erhöhten Temperaturen gehärtet, oder das Lösungsmittel verdampft je nachdem aus dem gelösten Polymer. Das Härten wird als das Verfahren zur Umwandlung des elastomeren oder polymeren Materials in den fertigen oder nützlichen Zustand durch die Anwendung von Wanne und/oder Chemikalien, die physikalisch-chemische Ladungen umfassen, definiert. Wo beispielsweise thermoplastische Polyurethanelastomere verwendet werden, kann die Lösungsmittelverdampfung bei Raumtemperatur auftreten, was das polymere Material für die kontrollierte Arzneimittelfreisetzung ohne weiteres Härten nützlich macht. Nicht-einschränkende Beispiele des Härtens gemäß dieser Definition umfassen die Anwendung von Wärme und/oder Chemikalien und die Verdampfung des Lösungsmittels, welches physikalische und/oder chemische Veränderungen induzieren kann.
  • Die Belüftungszeit und -temperatur zur Härtung werden durch das spezielle einbezogene Polymer und die speziellen verwendeten Arzneimittel bestimmt. Beispielsweise sind Silikon oder Polysiloxanmaterialien (wie Polydimethylsiloxan) erfolgreich verwendet worden. Diese Materialien werden als Vorpolymer in der Beschichtungszusammensetzung verwendet und müssen danach gehärtet werden. Die bevorzugten Spezies weisen relativ niedrige Härtungstemperaturen auf, und sind als bei Raumtemperatur vulkanisierende (RTV) Materialien bekannt. Einige Polydimethylsiloxanmaterialien können beispielsweise durch Luftaussetzung bei etwa 90°C für einen Zeitraum von 16 Stunden gehärtet werden. Ein Härtungsschritt kann sowohl nach der Auftragung einer bestimmten Anzahl von unteren Grundschichten als auch Deckschichten oder einem einzelnen Härtungsschritt, der verwendet wird, nachdem das Beschichten beendet ist, durchgeführt werden.
  • Die beschichteten Stents können danach einem Nachhärtungssterilisationsverfahren unterzogen werden, welches eine Inertgasplasmabehandlung umfasst, und dann kann das Aussetzen der Gammastrahlung, dem Elektronenstrahl, Ethylenoxid (ETO) oder der Dampfsterilisation ebenso eingesetzt werden.
  • Bei der Plasmabehandlung werden unerzwungen beschichtete Stents in einer Reaktorkammer platziert und das System wird mit Stickstoff gespült, und ein Vakuum auf etwa 20 bis 50 m Torr angelegt. Danach wird Inertgas (Argon, Helium oder Gemisch aus diesen) der Reaktorkammer für die Plasmabehandlung zugeführt. Ein stark bevorzugtes Betriebsverfahren besteht in der Verwendung von Argongas, welches bei einem Leistungsbereich von 200 bis 400 Watt, einer Fließgeschwindigkeit von 150 bis 650 Standard ml pro Minute, die zu etwa 100 bis 450 m Torr äquivalent ist, und einer Exponierungszeit von 30 Sekunden bis etwa 5 Minuten arbeitet. Die Stents können direkt nach der Plasmabehandlung entfernt werden, oder verbleiben in der Argonatmosphäre für einen zusätzlichen Zeitraum, typischerweise fünf Minuten.
  • Nach der Argonplasmavorbehandlung werden die beschichteten und gehärteten Stents der Gammastrahlungssterilisation nominal bei 2,5 bis 3,5 Mrad unterzogen. Die Stents verfügen über vollständige Elastizität nach der Bestrahlung, egal ob sie in einem erzwungenen oder nicht erzwungenen Zustand exponiert wurden. Es ist herausgefunden worden, dass erzwungene Stents, die der Gammasterilisation ohne Verwendung der Argonplasmavorbehandlung unterzogen wurden, Elastizität verlieren, und sich bei einer ausreichenden oder geeigneten Rate nicht wieder herstellen.
  • Das elastomere Material, das einen Hauptbestandteil der Stentbeschichtung bildet, sollte bestimmte Eigenschaften besitzen. Es ist bevorzugt ein geeignetes hydrophobes, biostabiles, elastomeres Material, welches sich nicht abbaut, und das die Gewebeabstoßung und Gewebeentzündung minimiert, und eines, das der Einkapselung durch Gewebe unterliegt, welches neben der Stentimplantationsstelle liegt. Polymere, die für solche Beschichtungen geeignet sind, umfassen Silikone (z. B. Polysiloxane und substituierte Polysiloxane), Polyurethane (einschließlich Polycarbonaturethane), thermoplastische Elastomere im Allgemeinen, Ethylenvinylacetatcopolymere, Polyolefinelastomere, EPDM-Kautschuke und Polyamidelastomere. Die obengenannten Materialien werden in Bezug auf die beabsichtigte Umgebung der Erfindung als hydrophob betrachtet.
  • Mittel, die zur Einführung geeignet sind, umfassen Antithrombotika, Antikoagulationsmittel, Antithrombozytenmittel, thrombolytische Mittel, antiproliferative Mittel, Entzündungshemmer, Mittel, die Hyperplasie und insbesondere Restenose inhibieren, Glattmuskelzelleninhibitoren, Antibiotika, Wachstumsfaktoren, Wachstumsfaktorinhibitoren, Zelladhäsionsinhibitoren, Zelladhäsionspromotoren und Arzneimittel, die die Bildung von gesundem Neointimalgewebe, einschließlich der Endothelzellregeneration, verbessern können. Die positive Wirkung kann aus der Inhibierung der speziellen Zellen (beispielsweise Glattmuskelzellen) oder Gewebebildung (beispielsweise fibromuskuläres Gewebe) während der Unterstützung unterschiedlicher Zellmigration (z. B. Endothel) und Gewebebildung (Neointimalgewebe) hervorgehen.
  • Die bevorzugten Materialien zur Herstellung des geflochtenen Stents umfassen Edelstahl, Tantal, Titanlegierungen, einschließlich Nitinol (ein Nickeltitan, thermogespeichertes Legierungsmaterial), und bestimmte Kobaltlegierungen, einschließlich Kobalt-Chrom-Nickel-Legierungen, wie Elgiloy® und Phynox®. Weitere Einzelheiten bezüglich der Herstellung und Einzelheiten von anderen Aspekten der Stents selbst können aus den obengenannten US-Patenten 4,655,771 und 4,954,126 von Wallsten und 5,061,275 von Wallsten et al. zusammengetragen werden.
  • Verschiedene Kombinationen von Polymerbeschichtungsmaterialien können mit biologisch aktiven Spezies von Interesse koordiniert werden, um die gewünschten Wirkungen zu erzeugen, wenn sie auf Stents beschichtet werden, die gemäß der Erfindung implantiert werden sollen. Die Ladungen von therapeutischen Materialien können variieren. Der Mechanismus der Einführung von biologisch aktiven Spezies in die Oberflächenbeschichtung und der Ausgangsmechanismus hängen sowohl von der Beschaffenheit des Oberflächenbeschichtungspolymers als auch von dem einzuführenden Material ab. Der Mechanismus der Freisetzung hängt ebenso von der Einführungsweise ab. Das Material kann über Zwischenwege eluieren oder über Transport oder Diffusion durch das Einkapselungsmaterial selbst verabreicht werden.
  • Für die Zwecke dieser Beschreibung wird die ”Elution” als irgendein Freisetzungsverfahren definiert, welches die Extraktion oder Freisetzung durch direkten Kontakt des Materials mit Körperflüssigkeiten durch die Zwischenwege, die mit dem Äußeren der Beschichtung verbunden sind, umfasst. Der ”Transport” oder die ”Diffusion” sollen einen Freisetzungsmechanismus umfassen, bei dem ein freigesetztes Material ein anderes Material durchquert.
  • Das gewünschte Freisetzungsratenprofil kann durch Variieren der Beschichtungsdicke, radialen Verteilung (Schicht um Schicht) der bioaktiven Materialien, des Mischverfahrens, der Menge an bioaktivem Material, der Kombination von unterschiedlichen Matrixpolymermaterialien bei unterschiedlichen Schichten und der Vernetzungsdichte des polymeren Materials abgestimmt werden. Die Vernetzungsdichte bezieht sich auf die Menge an Vernetzung, die stattfindet, und ebenso die relative Dichtheit der Matrix, die durch das spezielle verwendete Vernetzungsmittel erzeugt wird. Dies bestimmt während des Härtungsverfahrens die Menge an Vernetzung und so die Vernetzungsdichte des Polymermaterials. Für bioaktive Materialien, die aus der vernetzten Matrix freigesetzt wurden, wie Heparin, wird eine Vernetzungsstruktur von größerer Dichte die Freisetzungszeit erhöhen und die Explosionswirkung verringern.
  • Außerdem können bei der Elution der Materialien, wie Heparin, die Freisetzungskinetiken, speziell die anfängliche Arzneimittelfreisetzungsrate, durch Variieren der verteilten durchschnittlichen Teilchengröße beeinflusst werden. Die beobachtete anfängliche Freisetzungsrate oder Explosionswirkung wird im Wesentlichen durch die Verwendung kleinerer Teilchen verringert, und zwar wenn die Teilchengröße auf kleiner als 15 μm kontrolliert wird, und die Wirkung in dem Teilchengrößenbereich von weniger als oder gleich 10 μm sogar signifikanter ist, speziell wenn die Beschichtungsdicke nicht mehr als etwa 50 μm beträgt und die Arzneimittelladung etwa 25 bis 45 Gew.-% beträgt.
  • Es wird ebenso eingeschätzt, dass eine nicht medikamentös behandelte dünne Silikondeckschicht eine vorteilhafte Arzneimittel-enthaltende Deckschicht bereitstellt. Ihre Oberfläche weist eine begrenzte Porosität auf, und ist im Allgemeinen glatt, was weniger thrombogen sein kann und die Chance zur Entwicklung von Kalzifizierung, die meistens auf der porösen Oberflache vorkommt, verringern kann.
  • KURZE BESCHREIBUNG DER ZEICHNUNGEN
  • In den Zeichnungen, worin gleiche Zahlen überall gleiche Teile bezeichnen:
  • ist 1 ein schematisches Fließdiagram, welches die Schritte zum Herstellen einer Prothese der Erfindung darstellt;

  • AUSFÜHRLICHE BESCHREIBUNG
  • Gemäß der vorliegenden Erfindung werden die Stentbeschichtungen, die biologisch aktive Materialien zur zeitlich abgestimmten Abgabe in situ in einen Körperhohlraum von Interesse umfassen, bevorzugt in vielen dünnen Schichten aus hergestellten Beschichtungslösungen oder -suspensionen aufgesprüht. Die Schritte des Verfahrens werden allgemein in 1 dargestellt. Die Beschichtungslösungen oder -suspension en werden bei 10 hergestellt, wie später beschrieben. Die gewünschte Menge an Vernetzungsmittel wird zu der Suspension/Lösung bei 12 zugegeben, und das Material wird dann bewegt oder gerührt, um eine homogene Beschichtungszusammensetzung bei 14 herzustellen, die danach in einen Auftragungsbehälter oder eine -vorrichtung, die ein Behälter zum Farbspritzen sein kann, bei 16 übertragen wird. Typische exemplarische Präparate von Beschichtungslösungen, die für Heparin und Dexamethason verwendet wurden, werden als nächstes dargestellt, obwohl letzteres, das nicht partikulär ist, wenn es auf den Stent aufgetragen wird, nicht erfindungsgemäß ist.
  • Allgemeine Herstellung der Heparinbeschichtungszusammensetzung
  • Silikon wurde aus einem Polymerpräkursor in einem Lösungsmittelgemisch (Xylol) erhalten. Beispielsweise wurde ein Silikon mit einem Feststoffgehalt von 35 Gew.-% in Xylol von Applied Silikon, Part #40.000 beschafft. Zunächst wurde das Silikon-Xylol-Gemisch gewogen. Der Silikon-Feststoffgehalt wurde gemäß der Analyse des Verkäufers bestimmt. Vorberechnete Mengen an fein verteiltem Heparin (2 bis 6 μm) wurden zu dem Silikon zugegeben, dann wurde Tetrahydrofuran (THF) HPCL-Qualität (Aldrich oder EM) zugegeben. Für eine 37,5%ige Heparinbeschichtung beispielsweise: W(Silikon) = 5 g; Feststoffgehalt in Prozent = 35%; W(Hep.) = 5 × 0,35 × 0,375/(0,625) = 1,05 g. Die Menge an benötigtem THF (44 ml) in der Beschichtungslösung wurde unter Verwendung der Gleichung W(Silikon-Feststoffgehalt)/N(THF) = 0,04 für eine 37,5%ige Heparinbeschichtungslösung verwendet. Schließlich wurde die Herstellervernetzungsmittellösung unter Verwendung einer Pasteur P-Pipette zugegeben. Die Menge an zugegebenem Vernetzungsmittel wurde gewählt, um das Profil der Freisetzungsrate zu bewirken. Typischerweise wurden fünf Tropfen der Vernetzungsmittellösung für jeweils fünf Gramm des Silikon-Xylol-Gemisches zugegeben. Das Vernetzungsmittel kann jedes geeignete und kompatible Mittel sein, einschließlich Platin und Peroxid basierende Materialien.
  • Die Lösung wurde unter Verwendung des Rührstabs gerührt, bis die Suspension homogen und milchartig war. Die Beschichtungslösung wurde dann in ein Farbglas in einem zur Auftragung durch Feinspritzen geeigneten Zustand übertragen.
  • Allgemeine Herstellung der Dexamethason-Beschichtungszusammensetzung
  • Silikon (35%ige Lösung wie oben) wurde in einem Becherglas auf einer Metler-Waage gewogen. Das Gewicht an Dexamethason in Form des freien Alkohols oder in Acetatform wurde berechnet, indem das Silikongewicht mit 0,35 und dem gewünschten Prozentsatz an Dexamethason (1 bis 40%) multipliziert wurde, und die erforderliche Menge wurde dann abgewogen. Beispiel: W(Silikon) = 5 g; für eine 10%ige Dexamethason-Beschichtung, W(Dex) = 5 × 0,35 × 0,1/0,9 = 0,194 g und THF, das in der berechneten Beschichtungslösung benötigt wird. W(Silikon-Feststoffgehalt)/V(THF) = 0,06 für eine 10%ige Dexamethason-Beschichtungslösung. Beispiel: W(Silikon) = 5 g; V(THF) = 5 × 0,35/0,06 = 29,17 ml. Das Dexamethason wurde in einem Becherglas auf einer analytischen Waage gewogen und die Hälfte der Gesamtmenge an THF wurde zugegeben. Die Lösung wurde gut gerührt, um die vollständige Auflösung des Dexamethasons sicherzustellen. Die gerührte DEX-THF-Lösung wurde dann in den Silikonbehälter übertragen. Das Becherglas wurde mit dem restlichen THF gewaschen und dies wurde zu dem Silikonbehälter übertragen. Das Vernetzungsmittel wurde unter Verwendung einer Pasteur-Pipette zugegeben. Typischerweise wurden fünf Tropfen Vernetzungsmittel für fünf Gramm Silikon verwendet.
  • Die Auftragung des Beschichtungsmaterials auf den Stent war für alle Materialien ziemlich ähnlich und dieselbe für die Heparin- und Dexamethasonsuspensionen, die wie in den obigen Beispielen hergestellt wurden. Die Suspension, die aufgetragen werden soll, wurde in eine Auftragungsvorrichtung übertragen, typischerweise ein Farbglas, das mit einer Sprühpistole verbunden ist, wie Badger Modell 150, geliefert mit einer Druckluftquelle durch einen Regulator (Norgren, 0 bis 160 psi). Sobald das Pistolengehäuse mit der Druckluftquelle stromabwärts des Regulators verbunden war, wurde die Luft aufgebracht. Der Druck wurde auf ungefähr 1 bis 1,7 atm (15–25 psi) eingestellt und der Düsenzustand durch Drücken des Auslösers kontrolliert.
  • Jedes geeignete Verfahren kann zur Befestigung des Stents beim Sprühen verwendet werden, wobei rotierende Halterungen erfolgreich im Labor verwendet wurden. Beide Enden des relaxierten Stents wurden an der Halterung durch zwei gefederte Halter, gewöhnlich Krokodilklemmen, befestigt, wobei der Abstand zwischen den Klemmen so eingestellt ist, dar., der Stent in einem relaxierten, ungestreckten Zustand bleibt. Der Rotor wurde dann eingeschaltet und die Rotationsgeschwindigkeit auf die gewünschte Beschichtungsgeschwindigkeit, beispielsweise etwa 40 U/min, eingestellt.
  • In Bezug auf den Stent, der in einer im Wesentlichen horizontalen Ebene rotiert, wurde die Sprühdüse so eingestellt, dass der Abstand von der Düse zu dem Stent etwa 2 bis 4 Inch betrug und die Zusammensetzung im Wesentlichen horizontal gesprüht wurde, wobei die Pistole entlang des Stents von dem distalen Ende des Stents zu dem proximalen Ende und dann von dem proximalen Ende zu dem distalen Ende in einer Gleitbewegung bei einer Geschwindigkeit gerichtet ist, so dass ein Sprühkreislauf in etwa drei Stentrotationen stattfindet. Typischerweise liegt eine Pause von weniger als einer Minute, normalerweise etwa eine halbe Minute, zwischen den Schichten. Normalerweise variiert die Anzahl an Beschichtungsschichten in Abhängigkeit von der speziellen Anwendung. Beispielsweise sind für ein Beschichtungsniveau von 3 bis 4 mg Heparin pro cm2 projizierte Flache 20 Kreislaufe der Beschichtungsauftragung erforderlich, und etwa 30 ml Lösung werden für einen Stent mit einem Durchmesser von 3,5 mm und einer Länge von 14,5 cm verbraucht.
  • Wenn gewünscht, kann die Drehgeschwindigkeit des Motors natürlich ebenso eingestellt werden wie die Viskosität der Zusammensetzung und die Fließgeschwindigkeit der Sprühdüse, um die geschichtete Struktur zu modifizieren. Im Allgemeinen sind mit den obigen Mischungen die besten Ergebnisse bei Drehgeschwindigkeiten in dem Bereich von 30 bis 50 U/min und mit einer Sprühdüsenfließgeschwindigkeit in dem Bereich von 4 bis 10 ml der Beschichtungszusammensetzung pro Minute in Abhängigkeit von der Stentgröße erhalten worden. Es ist anzunehmen, dass das Verfahren, von dem gezeigt wurde, dass es im Labor durchführbar ist, mittels einer ausgereifteren, computergesteuerten Beschichtungsvorrichtung erfolgreich automatisiert werden kann.
  • Mehrere aufgetragene Schichten bilden das, was man die Bindeschicht bei 18 nennt, und danach weitere Deckschichten, die von unterschiedlicher Zusammensetzung in Bezug auf das bioaktive Material sein können, die Matrixpolymermaterialien und Vernetzungsmittel werden beispielsweise als Deckschicht bei 20 aufgetragen. Die Auftragung der Deckschicht folgt derselben Beschichtungsverfahrensweise wie die Bindeschicht, wobei die Anzahl und Dicke der Schichten optional ist. Natürlich kann die Dicke jeder Schicht durch Modifizieren der Drehgeschwindigkeit des Stents und der Sprühbedingungen eingestellt werden. Im Allgemeinen wird die Gesamtbeschichtungsdicke durch die Anzahl an Sprühkreisläufen oder an dünnen Beschichtungen, die die Gesamtbeschichtung bilden, kontrolliert.
  • Wie bei 22 in 1 gezeigt, wird der beschichtete Stent danach einem Härtungsschritt unterzogen, in dem das Vorpolymer und die Vernetzungsmittel zusammenwirken, um eine gehartete Polymermatrix herzustellen, die die biologisch aktiven Spezies enthalt. Das Härtungsverfahren umfasst die Verdampfung des Lösungsmittels Xylol, THF usw. und das Härten und Vernetzen des Polymers. Bestimmte Silikonmaterialien können bei relativ niedrigen Temperaturen gehärtet werden (d. h. RT bis 50°C), was als Vulkanisationsverfahren bei Raumtemperatur (RTV) bekannt ist. Typischerweise sind in das Härtungsverfahren jedoch bei höheren Temperaturen härtende Materialien einbezogen, und die beschichteten Stents werden in einen Ofen bei ungefähr 90°C oder höher für ungefähr 16 Stunden gegeben. Die Temperatur kann auf bis zu 150°C für Dexamethason-enthaltende beschichtete Stents erhöht werden. Natürlich können die Zeit und Temperatur bei speziellen Silikonen, Vernetzungsmitteln und biologisch aktiven Spezies variieren.
  • Die Stents, die in der beschriebenen Weise beschichtet und gehärtet wurden, müssen vor dem Verpacken für die zukünftige Implantation sterilisiert werden. Zur Sterilisation ist Gammastrahlung ein bevorzugtes Verfahren, speziell für Heparin-enthaltende Beschichtungen; jedoch ist herausgefunden worden, dass Stents, die gemäß dem erfindungsgemäßen Verfahren beschichtet und gehärtet und der Gammastrahlung unterzogen wurden, zu langsam sein können, um ihre Ursprungsstellung wiederzugewinnen, wenn sie einer vaskularen oder anderen Hohlraumstelle unter Verwendung eines Katheters zugeführt werden, sofern nicht ein Vorbehandlungsschritt bei 24 zunächst auf den beschichteten, gehärteten Stent angewendet wird.
  • Der Vorbehandlungsschritt umfasst eine Argonplasmabehandlung der beschichteten, gehärteten Stents in der unerzwungenen Konfiguration. Gemäß dieser Verfahrensweise werden die Stents in eine Kammer eines Plasmaoberflächenbehandlungssystems, wie ein Plasma Science 350 (Himont/Plasma Science, Foster City, CA), gegeben. Das System ist mit einer Reaktorkammer und einem Hochfrequenz-Festkörper-Generator, der bei 13,56 mHz und 0 bis 500 Watt Leistungsabgabe arbeitet, und mit einem Mikroprozessor-gesteuerten System und einer vollständigen Vakuumpumpen-Baueinheit ausgestattet. Die Reaktionskammer enthalt ein freies Arbeitsvolumen von 42,55 cm (16,75 Inch) mal 34,3 cm (13,5 Inch) mal 44,45 cm (17,5 Inch) in der Tiefe.
  • Bei dem Plasmaverfahren werden unerzwungene beschichtete Stents in eine Reaktorkammer gegeben und das System mit Stickstoff gespült und ein Vakuum auf 20 bis 50 m Torr angelegt. Danach wird Inertgas (Argon, Helium oder Gemisch von diesen) zu der Reaktionskammer für die Plasmabehandlung zugegeben. Ein stark bevorzugtes Betriebsverfahren besteht in der Verwendung von Argongas, das mit einem Leistungsbereich von 200 bis 400 Watt, einer Fließgeschwindigkeit von 150 bis 650 Standard-ml pro Minute, die zu 100 bis 450 m Torr äquivalent ist, und einer Exponierungszeit von 30 Sekunden bis etwa 5 Minuten eingesetzt wird. Die Stents können direkt nach der Plasmabehandlung entfernt werden oder verbleiben in der Argonatmosphäre für einen zusätzlichen Zeitraum, typischerweise fünf Minuten.
  • Danach, wie bei 26 gezeigt, werden die Stents der Gammasterilisation bei 2,5 bis 3,5 Mrad unterzogen. Die Bestrahlung kann mit dem Stent in entweder dem radial unerzwungenen Status oder in dem radial erzwungenen Status durchgeführt werden.
  • In Bezug auf das Antikoagulationsmittel Heparin beträgt der Prozentsatz in der Bindeschicht nominal etwa 20 bis 50% und der der Deckschicht etwa 0 bis 30% aktives Material. Das Beschichtungsdickenverhältnis der Deckschicht zu der Bindeschicht variiert von etwa 1:10 bis 1:2 und liegt bevorzugt in dem Bereich von etwa 1:6 bis 1:3.
  • Die Unterdrückung der Explosionswirkung ermöglicht auch eine Verringerung der Arzneimittelladung oder mit anderen Worten eine Verringerung der Beschichtungsdicke, da der Arzt eine Bolus-Injektion von Antithrombozyten-/Antikoagulations-Arzneimitteln dem Patienten während des Einsetzens des Stents verabreichen wird. Infolgedessen kann das Arzneimittel, das in dem Stent eingebettet ist, vollständig ohne Verschwendung verwendet werden. Die Abstimmung der Freisetzung am ersten Tag, aber Maximierung der Freisetzung am zweiten Tag und am dritten Tag an der dünnsten möglichen Beschichtungskonfiguration wird die akute oder subakute Thrombose verringern.
  • 4 stellt die allgemeine Wirkung der Arzneimittelladung für Beschichtungen mit ähnlicher Dicke dar. Die anfängliche Elutionsrate erhöht sich mit der Arzneimittelladung, wie in 5 gezeigt. Die Freisetzungsrate erhöht sich ebenso mit der Dicke der Beschichtung bei derselben Ladung, aber ist gewöhnlich umgekehrt proportional zu der Dicke der Deckschicht, wie durch dieselbe Arzneimitelladung und ähnliche Bindeschichtdicke in 6 gezeigt.
  • Die Wirkung der durchschnittlichen Teilchengröße wird in den 7 bis 11 dargestellt, wobei Beschichtungsschichten mit einer durchschnittlichen Beschichtungsdicke von etwa 25 Mikron (μm), hergestellt und sterilisiert wie oben, mit den verteilten Heparinteilchen (37,5% Heparin) mit mehreren unterschiedlichen durchschnittlichen Teilchengrößen bereitgestellt wurden. 7 zeigt Diagramme von Elutionskinetiken für vier unterschiedliche Größen von eingebetteten Heparinteilchen. Die Freisetzung fand in Phosphatpuffer (pH 7,4) bei 37°C statt. Die Freisetzungsrate unter Verwendung kleinerer Teilchen, speziell mit einer durchschnittlichen Größe von 4 bis 6 μm, verringert merklich die anfängliche Rate oder Explosionswirkung, und danach verringert sich die Elutionsrate mit der Zeit langsamer. Durchschnittliche Teilchengrößen über etwa 15 mm führen zu anfänglichen Freisetzungsraten, die der Bolus-Elution nahe kommen. Dies ist natürlich weniger wünschenswert, sowohl aus dem Gesichtspunkt, dass ein unnötiger anfänglicher Überschuss vorhanden ist, als auch wegen der frühzeitigen Entziehung des abgegebenen Arzneimittelmaterials aus der Beschichtung.
  • Wie in den mikrophotographischen Aufnahmen der 8 bis 11 gezeigt wird, veränderte sich, wenn sich die durchschnittliche Teilchengröße erhöhte, außerdem auch die Morphologie der Beschichtungsoberflache. Beschichtungen, die größere Teilchen enthalten (9 bis 11), weisen sehr raue und unregelmäßige Oberflächeneigenschaften auf. Diese Oberflächenunregelmäßigkeiten können thrombogener sein oder zeigen eine erhöhte Tendenz, eine Embolisierung zu verursachen, wenn der entsprechende Stent in ein Blutgefäß implantiert wird.
  • Folglich ist herausgefunden worden, dass die durchschnittliche Teilchengröße im Allgemeinen auf unter etwa 15 μm eingestellt werden sollte, um die Explosionswirkung zu verringern, und bevorzugt sollte sie weniger als oder gleich etwa 10 μm für beste Ergebnisse betragen. Die Größe von 4 bis 6 μm war im Labor ziemlich erfolgreich. Elutionskinetiken können durch eine Kombination der Veränderung der Teilchengröße und der Veränderung der Ladung oder Konzentration des verteilten Arzneimittelmaterials eingestellt werden.
  • Was aus den bisher gewonnen Daten ersichtlich ist, ist jedenfalls, dass das erfindungsgemäße Verfahren die Modifikation der Arzneimittelelutionskinetiken ermöglicht, um die Notwendigkeiten der speziellen Stentanwendung zu erfüllen. In einer ähnlichen Weise können Stentbeschichtungen unter Verwendung einer Kombination aus zwei oder mehr Arzneimitteln hergestellt werden, wobei die Reihenfolge der Arzneimittelfreisetzung und deren Geschwindigkeit kontrolliert werden können. Beispielsweise können antiproliferative Arzneimitel in der Grundschicht und Antithrombotika in der Deckschicht kombiniert werden. In dieser Weise werden die Antithrombotika, beispielsweise Heparin, zunächst eluieren, gefolgt von antiproliferativen Arzneimitteln, beispielsweise Dexamethason, um die sichere Einkapselung des implantierten Stents besser zu ermöglichen.
  • Die Heparinkonzentrationsmessungen wurden unter Verwendung einer Standardkurve durchgeführt, die durch Komplexieren von Azurblau-A-Farbstoff mit verdünnten Lösungen aus Heparin hergestellt wurde. Sechzehn Standards wurden verwendet, um die Standardkurve in einer allgemein bekannten Weise zusammenzustellen.
  • Für den Elutionstest wurden die Stents in eine Phosphatpufferlösung bei pH 7,4 in einem Inkubator bei ungefähr 37°C eingetaucht. Regelmäßige Entnahmen von Lösungsproben wurden durchgeführt, um die Menge an eluiertem Heparin zu bestimmen. Nach jeder Probenentnahme wurde jeder Stent in Heparin-freie Pufferlösung gegeben.
  • Wie oben angemerkt, während die mögliche Ladung des elastomeren Materials mit Heparin variieren kann, kann in dem Fall von Silikonmaterialien Heparin 60% des Gesamtgewichtes der Schicht überschreiten. Jedoch liegt die Ladung, die im Allgemeinen am vorteilhaftesten verwendet wird, in dem Bereich von etwa 10% bis 45% des Gesamtgewichts der Schicht. Bei Dexamethason kann die Ladung bis zu 50% oder mehr des Gesamtgewichts der Schicht betragen, aber liegt bevorzugt in dem Bereich von etwa 0,4% bis 45%.
  • Es wird eingeschätzt, dass, der Mechanismus der Einführung der biologisch aktiven Spezies in eine dünne Oberflächenbeschichtungsstruktur, auftragbar auf einen Metallstent, ein wichtiger Aspekt der vorliegenden Erfindung ist. Die Notwendigkeit für relativ dickwandige Polymerelutionsstents oder irgendwelche Membrandeckschichten, die mit vielen vorherigen Arzneimittelelutionsvorrichtungen verbunden sind, ist ebenso überflüssig, wie es die Notwendigkeit für die Verwendung von biologisch abbaubaren oder reabsorbierbaren Vehikeln zum Tragen der biologisch aktiven Spezies ist. Die Techniken ermöglichen deutlich die Langzeitabgabe und minimieren die Interferenz mit den unabhängigen mechanischen oder therapeutischen Vorteilen des Stents selbst.
  • Beschichtungsmaterialien werden unter Berücksichtigung einer speziellen Beschichtungstechnik Beschichtungs-/Arzneimittel-Kombination und Arzneimittelinfusionsmechanismus gestaltet. Die Berücksichtigung der speziellen Form und des Mechanismus der Freisetzung der biologisch aktiven Spezies in der Beschichtung ermöglicht es, dass die Technik bessere Ergebnisse erzeugen kann. Auf diese Weise kann die Abgabe der biologisch aktiven Spezies aus der Beschichtungsstruktur so abgestimmt werden, dass sie an eine Vielzahl von Anwendungen angepasst werden kann.
  • Während die obigen Beispiele Beschichtungen mit zwei unterschiedlichen Arzneimitteladungen oder Prozentsätzen an biologisch aktivem Material, das freigesetzt werden soll, darstellen, soll dies keineswegs die Erfindung einschränken, und es wird in Betracht gezogen, dass jede Anzahl von Schichten und Kombinationen von Ladungen eingesetzt werden können, um ein gewünschtes Freisetzungsprofil zu erreichen. Beispielsweise können graduelle Staffelungen und Veränderungen der Ladung der Schichten genutzt werden, wobei beispielsweise höhere Ladungen in den inneren Schichten verwendet werden. Es können ebenso Schichten verwendet werden, die überhaupt keine Arzneimittelladungen aufweisen. Beispielsweise kann ein pulsierendes Heparinfreisetzungssystem durch eine Beschichtung erreicht werden, wobei alternierende Schichten, die Heparin enthalten, zwischen unbeladenen Schichten aus Silikon oder anderen Materialien für einen Teil der Beschichtung angeordnet sind. Mit anderen Worten, die Erfindung ermöglicht zahllose Kombinationen, die zu einer sehr hohen Flexibilität im Hinblick auf die Kontrolle der Freisetzung von biologisch aktiven Materialien in Bezug auf einen implantierten Stent führen. Jede aufgetragene Schicht betragt typischerweise ungefähr 0,5 μm bis 15 μm in der Dicke. Die Gesamtzahl an gesprühten Schichten kann natürlich weitgehend variieren, von weniger als 10 bis mehr als 50 Schichten; gewöhnlich sind 20 bis 40 Schichten einbezogen. Die Gesamtdicke der Beschichtung kann ebenso weitgehend variieren, aber kann im Allgemeinen etwa 10 bis 200 μm betragen.
  • Während das Polymer der Beschichtung jedes kompatible biostabile, elastomere Material sein kann, das an dem Stentmaterial als eine dünne Schicht haften kann, sind hydrophobe Materialien bevorzugt, da herausgefunden worden ist, dass die Freisetzung der biologisch aktiven Spezies im Allgemeinen mit diesen Materialien vorhersehbarer gesteuert werden kann. Bevorzugte Materialien umfassen speziell Silikonkautschukelastomere und biostabile Polyurethane.

Claims (8)

  1. Beschichtete implantierbare Prothese mit einer äußeren Oberfläche, die mit einer Beschichtung umhüllt ist, umfassend: (a) eine erste Deckschicht, umfassend ein elastomeres Material, welches eine Menge an partikulärem biologisch aktivem Material einschließt; wobei die durchschnittliche Teilchengröße des biologisch aktiven Materials weniger als oder gleich etwa 15 μm beträgt, und (b) eine zweite Deckschicht, umfassend ein polymeres Material, das über der ersten Deckschicht angeordnet ist; wobei sich die Beschichtung haftend an die Prothese anpasst und wobei die Wirkstoffbeladung in der inneren Schicht höher ist.
  2. Vorrichtung nach Anspruch 1, wobei das elastomere Material ein hydrophobes elastomeres Material ist, und das partikuläre biologisch aktive Material in dem elastomeren Material für eine zeitlich abgestimmte Abgabe verteilt ist.
  3. Vorrichtung nach Anspruch 2, wobei das elastomere Material aus der Gruppe ausgewählt ist, bestehend aus Silikonen, Polyurethanen, Polyamidelastomeren, Ethylenvinylacetatcopolymeren, Polyolefinelastomeren, EPDM-Kautschuken und Kombinationen davon.
  4. Vorrichtung nach Anspruch 2 oder Anspruch 3, wobei das biologisch aktive Material Heparin umfasst.
  5. Vorrichtung nach einem der Ansprüche 2 bis 4, wobei das biologisch aktive Material eine durchschnittliche Teilchengröße von weniger als oder gleich etwa 10 μm aufweist.
  6. Vorrichtung nach einem vorhergehenden Anspruch, wobei die zweite Deckschicht eine nicht mit Medikamenten angereichene Silikondeckschicht ist.
  7. Vorrichtung nach Anspruch 6, wobei die Vorrichtung nur zwei Deckschichten enthält.
  8. Vorrichtung nach einem der Ansprüche 1 bis 5, wobei die zweite Deckschicht ein zweites biologisch aktives Material einschließt.
