- Technisches Gebiet
- Die vorliegende Erfindung ist allgemein auf eine operative Behandlung von Glaukomen und anderen Augenkrankheiten gerichtet und bezieht sich insbesondere auf eine aufpumpbare Vorrichtung zur Verwendung bei der ophthalmischen Operation, um den Schlemm'schen Kanal im Auge mechanisch zu erweitern und/oder Medikamente innerhalb des Schlemm'schen Kanals zur direkten Einwirkung auf den Kanal, das Trabekel-Netzwerk und benachbarte Gewebe zu infundieren.
- Hintergrund der Erfindung
- Ein Glaukom ist ein signifikantes häufiges Gesundheitsproblem, weil ein Glaukom eine Hauptursache für Blindheit ist. Die Blindheit, die aus einem Glaukom resultiert, involviert sowohl die zentrale als auch periphere Sicht und hat einen wesentlichen Einfluss auf die Fähigkeit eines Menschens, ein unabhängiges Leben zu führen.
- Ein Glaukom ist eine optische Neuropathie (eine Erkrankung des optischen Nervs), die gewöhnlich auftritt, wenn sich ein erhöhter Innenaugendruck einstellt. Der Druck innerhalb des Auges nimmt zu, und dies geht mit Änderungen im Erscheinungsbild ("cupping") und der Funktion ("blinde Flecken" im Blickfeld) des optischen Nervs einher. Wenn der Druck für eine ausreichend lange Zeitdauer hoch genug bleibt, kommt es zu einem totalen Sichtverlust. Hochdruck entwickelt sich im Auge wegen eines Ungleichgewichts der inneren Flüssigkeit.
- Das Auge ist eine hohle Struktur, die eine klare Flüssigkeit enthält "wässrige Flüssigkeit" genannt. Wässrige Flüssigkeit wird in der posterioren Kammer des Auges durch den Ziliarkörper mit einer Rate von etwa 2,5 Mikrolitern pro Minute erzeugt. Die Flüssigkeit, die mit einer ziemlich konstanten Rate gebildet wird, läuft dann um die Linse herum, durch die Pupillenöffnung in die Iris und in die anteriore Kammer des Auges. Sobald sich die Flüssigkeit in der anterioren Kammer befindet, fließt sie auf zwei verschiedenen Wegen aus dem Auge hinaus. In der "uveoscleralen" Route sickert die Flüssigkeit zwischen den Muskelfasern des Ciliarkörpers hindurch. Diese Route übernimmt zehn Prozent des wässrigen Ausflusses. Der primäre Weg für den wässrigen Ausfluss geht durch die "kanalikulare" Route, die das Trabekel-Netzwerk und den Schlemm'schen Kanal beinhaltet.
- Das Trabekel-Netzwerk und der Schlemm'sche Kanal sind an der Verbindung zwischen der Iris und der Sclera angeordnet. Diese Verbindung oder Ecke wird "der Winkel" genannt. Das Trabekel-Netzwerk ist eine keilförmige Struktur, die um den Umfang des Auges herum verläuft. Sie ist aus Kollagenbalken aufgebaut, die in einer dreidimensionalen siebartigen Struktur angeordnet sind. Die Balken sind mit einer Einzelschicht von Zellen bekleidet, die Trabekelzellen genannt werden. Die Zwischenräume zwischen den Kollagenbalken werden mit einer extrazellularen Substanz gefüllt, die von den Trabekelzellen erzeugt werden. Diese Zellen erzeugen auch Enzyme, die das extrazellulare Material zersetzen. Der Schlemm'sche Kanal ist dem Trabekel-Netzwerk benachbart. Die Außenwand des Trabekel-Netzwerks fällt mit der Innenwand des Schlemm'schen Kanals zusammen. Der Schlemm'sche Kanal ist eine rohrartige Struktur, die um den Umfang der Hornhaut herum verläuft. Bei erwachsenen Menschen nimmt man an, dass der Schlemm'sche Kanal durch Septen in eine Serie von autonomen blind endenden Kanälen unterteilt ist.
- Die wässrige Flüssigkeit läuft durch die Zwischenräume zwischen den Trabekelbalken über die Innenwand des Schlemm'schen Kanals in den Kanal hinein, durch eine Serie von Sammelkanälen, die den Schlemm'schen Kanal trainieren und in das episklerale venöse System hinein. In einer normalen Situation ist die wässrige Produktion gleich dem wässrigen Ausfluss, und der intraokulare Druck bleibt im Bereich von 15 bis 21 mm Hg ziemlich konstant. Bei einem Glaukom ist der Widerstand durch das kanalikulare Ausflusssystem abnormal hoch.
- Bei einem primären Weitwinkel-Glaukom, das die häufigste Form eines Glaukoms ist, nimmt man an, dass der abnormale Widerstand entlang dem äußeren Aspekt des Trabekel-Netzwerks und der Innenwand des Schlemm'schen Kanals auftritt. Es wird angenommen, dass ein abnormaler Stoffwechsel der Trabekelzellen zu einem übermäßigen Aufbau von extrazellularen Materialien oder einem Aufbau von abnormal "steifen" Materialien in diesem Bereich führt. Das primäre Weitwinkel-Glaukom ist für angenähert fünfundachtzig Prozent aller Glaukome verantwortlich. Andere Formen von Glaukomen (wie etwa Engwinkel-Glaukom und sekundäre Glaukome) beinhalten auch einen verringerten Ausstrom durch den kanalikularen Weg, aber der erhöhte Widerstand hat andere Ursachen, wie etwa mechanische Blockage, entzündlicher Debris, zelluläre Blockage etc.
- Mit dem erhöhten Widerstand baut sich die wässrige Flüssigkeit auf, weil sie nicht schnell genug abfließen kann. Wenn sich die Flüssigkeit aufbaut, steigt der intraokulare Druck (IOP) innerhalb des Auges. Der erhöhte IOP kann die vaskuläre Versorgung der optischen Nerven beeinträchtigen, der das Sehen vom Auge zum Gehirn leitet. Einige optische Nerven scheinen auf IOP empfindlicher zu sein als andere Nerven. Während die Forschung Wege untersucht, um den Nerv vor einem erhöhten Druck zu schützen, ist der einzige derzeitig verfügbare therapeutische Ansatz beim Glaukom der, den intraokularen Druck zu senken.
- Die klinische Behandlung eines Glaukoms wird stufenweise angegangen. Häufig ist die Medikation die erste Behandlungsoption. Entweder topisch oder oral verabreicht, haben diese Medikationen die Wirkung, entweder die wässrige Produktion zu reduzieren, oder die Wirkung, den Ausstrom zu vergrößern. Gegenwärtig verfügbare Medikationen haben zahlreiche ernsthafte Nebenwirkungen, einschließlich: kongestiver Herzfehler, Atemnot, Überanspannung, Depressionen, Nierensteine, aplastische Anämie, sexuelle Disfunktion, und Tod. Ein Verständnis mit der Medikation ist ein Hauptproblem, mit Schätzungen, dass über die Hälfte von Glaukompatienten ihre richtigen Dosierungspläne nicht befolgen.
- Wenn die Medikation nicht in der Lage ist, den Druck adäquat zu senken, wird häufig eine Laser-Trabekuloplastik durchgeführt. Bei der Laser-Trabekuloplastik wird die thermische Energie von einem Laser auf eine Anzahl nicht durchgehender Flecken in dem trabekularen Netzwerk angewendet. Man nimmt an, dass die Laserenergie den Stoffwechsel der Trabekelzellen auf gewisse Weise stimuliert und das extrazelluläre Material in Trabekel-Netzwerk ändert. Bei angenähert achtzig Prozent der Patienten wird der wässrige Ausfluss verbessert und der IOP nimmt ab. Jedoch ist der Effekt häufig nicht langfristig, und fünfzig Prozent der Patienten entwickeln innerhalb von fünf Jahren einen erhöhten Druck. Die Laseroperation ist gewöhnlich nicht wiederholbar. Zusätzlich ist die Laser-Trabekuloplastik keine wirksame Behandlung für primäre Weitwinkel-Glaukome bei Patienten, die weniger als fünfzig Jahre alt sind, noch ist sie effizient für ein Engwinkel-Glaukom und viele sekundäre Glaukome.
- Wenn die Laser-Trabekuloplastik den Druck nicht weit genug senkt, wird eine Filteroperation durchgeführt. Mit der Filteroperation wird in der Sklera und dem Winkelbereich ein Loch hergestellt. Dieses Loch erlaubt, dass die wässrige Flüssigkeit das Auge durch eine alternative Route verlässt.
- Die am häufigsten durchgeführte Filterprozedur ist Trabekulektomie. Bei der Trabekulektomie wird ein posteriorer Einschnitt in der Bindehaut durchgeführt, das transparente Gewebe, das die Sklera überdeckt. Die Bindehaut wird nach vorne gerollt, um die Sklera am Limbus freizulegen. Es wird eine Skleralklappe partieller Dicke hergestellt und halb dick in die Hornhaut eingeschnitten. Den Zugang in die anteriore Kammer liegt unter der Skleralklappe, und eine Sektion von tiefer Sklera und Trabekel-Netzwerk wird entnommen. Die Skleralklappe wird lose vor Ort zurückgenäht. Der Bindehauteinschnitt wird dicht verschlossen. Post-operativ fließt die wässrige Flüssigkeit durch das Loch unter der Skleralklappe und sammelt in einem erhöhten Raum unterhalb der Bindehaut. Die Flüssigkeit wird dann entweder durch die Blutgefäße in der Bindehaut absorbiert oder läuft über die Bindehaut in den Tränenfilm.
- Trabekulektomie geht mit vielen Problemen einher. Fibroblasten, die sich in der Episclera befinden, poliferieren und wandern und können die Skleralklappe vernarben. Es kann ein Fehler der Vernarbung auftreten, insbesondere bei Kindern und jungen Erwachsenen. Von Augen, die eine anfänglich erfolgreiche Trabekulektomie hatten, leiden innerhalb von drei bis fünf Jahren nach der Operation achtzig Prozent an fehlerhafter Vernarbung. Um Fibrose zu minimieren, wenden Operateure nun auf die Skleralklappe während der Operation Antifibrotische Mittel an, wie etwa Mitomycin C (MMC) und 5-Fluorouracil (5-FU). Die Verwendung dieser Mittel hat die Erfolgsrate der Trabeculectomie erhöht, hat jedoch auch das Auftreten von Hypotonie erhöht. Hypotonie ist ein Problem, das sich entwickelt, wenn das Wasser zu schnell aus dem Auge hinaus fließt. Der Augendruck fällt zu weit ab, gewöhnlich weniger als 6,0 mmHg); die Struktur des Auges kollabiert und die Sicht nimmt ab.
- Eine alternative operative Methode zur Glaucombehandlung kann spezifischer auf den Schlemm'schen Kanal ausgerichtet werden. Das
U.S. Patent Nr. 5,360,399 lehrt die Anordnung eines Kunststoff- oder Stahlrohr-Teils in dem Schlemm'schen Kanal, mit Injektion eines viskosen Materials durch Löcher in dem Rohr, um das trabekulare Netzwerk hydraulisch aufzutrennen. Jedoch gibt die '399 Vorrichtung nur wenig oder keine Option für die Distanz der Hydrodissektion innerhalb der Länge des Schlemm'schen Kanals, noch schlägt sie ein Mittel vor, um den Kanal zu erweitern, um die natürliche Drainage daraus zu erleichtern. - Die
U.S. 5,891,084 offenbart eine ophtalmische Kathetervorrichtung gemäß dem Oberbegriff von Anspruch 1. Diese Vorrichtung ist dazu ausgelegt, den Ballonkatheter zum episkleralen Raum des Auges zu führen, zur Anwendung eines Wirkstoffs oder Medikaments. DieUS-A-5486165 offenbart einen nicht erweiterbaren Katheter, der in dem Schlemm'schen Kanal einführbar ist, mit einer Mehrzahl von Fenstern innerhalb der Katheterwand, um den natürlichen Innenaugendruck einzuhalten. - Es gibt dann Bedarf nach einem System, das eine präzise Dilation und Expansion des Schlemm'schen Kanals entlang einem beliebigen Abschnitt davon erlauben würde. Es gibt Bedarf nach einer selektiven direkten Verabreichung therapeutischer Wirkstoffe in dem Schlemm'schen Kanal, die eine noch wirksame Kontrolle des Glaukoms mit weniger systemischen Komplikationen erlaubt als mit existierenden Alternativen zur Verabreichung von Medikamenten. Zusätzlich ist ein physiologischeres System erforderlich, um die Drainage der wässrigen Flüssigkeit in dem Schlemm'schen Kanal von dem anterioren Kammerwinkel her zu verbessern. Die Verbesserung des wässrigen Flusses direkt in dem Schlemm'schen Kanal würde die Vernarbung minimieren, da der Winkelbereich mit einer einzelnen Linie von nicht proliferierenden Trabekelzellen bevölkert ist. Die Verbesserung des wässrigen Flusses direkt in dem Schlemm'schen Kanal und auf natürliche Weise heraus in die Sammelkanäle würde Hypotonie minimieren, da der Kanal Teil des normalen Ausflusssystems ist und biologisch konstruiert ist, um das normale Volumen der wässrigen Flüssigkeit aufzunehmen. Die Verbesserung des wässrigen Flusses direkt in dem Schlemm'schen Kanal würde Komplikationen wie etwa Endophthalmitis, Hypotonie und Leckagen eliminieren.
- Zusammenfassung der Erfindung
- Die vorliegende Erfindung sieht eine ophtalmische Kathetervorrichtung vor, die einen proximalen Abschnitt, der von Verwender manuell steuerbar ist, einen distalen Abschnitt und ein Aufpumpzufuhrlumen, das sich von dem proximalen Abschnitt zum distalen Abschnitt erstreckt, aufweist, worin der distale Abschnitt des Aufpumpzufuhrlumens dehnbar ist, um einen Teil des Auges radial zu erweitern, wenn er aufgepumpt wird, worin der distale Abschnitt der Kathetervorrichtung einen Außendurchmesser von etwa 0,1 bis 0,5 mm und eine Länge von etwa 1,0 bis 20,0 mm aufweist, und dass der dehnbare Abschnitt in der Lage ist, den Durchmesser des Schlemm'schen Kanals vorübergehend zu erweitern oder einen Abschnitt des Kanals zu blockieren.
- An dieser Stelle kann das aufpumpbares Element dazu verwendet werden, den Ausfluss durch den Kanal vorübergehend zu verschließen, während physiologisches Material durch ein anderes Lumen der Vorrichtung injiziert wird, um hierdurch den Kanal zu erweitern und Stenosebereiche innerhalb des Kanals zu expandieren. Das aufgepumpte Element kann abgelassen und entfernt werden, nachdem die gewünschte Expansion erreicht ist, oder die Vorrichtung kann mit der expandierten aufpumpbaren Komponente herausgezogen werden, um den Durchgang innerhalb des Schlemm'schen Kanals mechanisch zu erweitern.
- Die vorliegende Erfindung kann auch dazu benutzt werden, verschiedene Medikamente direkt in den Schlemm'schen Kanal zu injizieren. Solche Medikamente können Antifibrotika, Antibiotika und andere Medikamente, die direkte Effekte innerhalb der internen Strukturen des Schlemm'schen Kanals, des Trabekel-Netzwerks und anderer Gewebe des Auges haben können, enthalten, sind aber hierauf nicht beschränkt. Die vorliegende Erfindung kann auch angewendet werden, um verschiedene Stents oder Shunts direkt im Schlemm'schen Kanal zu entfalten, um dazu beizutragen, nach dem Entfernen der aufpumpbaren Vorrichtung den Kanal durchgängig zu halten.
- Die hierin beschriebene erfindungsgemäße Vorrichtung erleichtert daher den normalen physiologischen Weg für die Drainage der wässrigen Flüssigkeit von der anterioren Kammer zu dem Schlemm'schen Kanal und den Austritt zu den Sammelkanälen, anstatt die Sklera oder eine andere anatomische Stelle kurzzuschließen, wie dies bei den meisten herkömmlichen Vorrichtungen getan wird. Darüber hinaus bietet die vorliegende Erfindung einen Mechanismus für die Verabreichung von Vorrichtungen oder Medikamenten direkt in den Schlemm'schen Kanal und die benachbarte ophthalmische Anatomie.

- Die vorliegende Erfindung sieht Kathetervorrichtungen für die Erweiterung des Schlemm'schen Kanals des Auges vor, durch mechanisches Dehnen eines Abschnitts, wenn sie aufgepumpt sind. Die aufpumpbaren Kathetervorrichtungen können auch dazu benutzt werden, eine Leitung vorzusehen, die in der Lage ist, therapeutische und/oder expansive Medikamente, die dort hindurch in den Schlemm'schen Kanal injiziert werden, zu verabreichen. Die aufpumpbaren Kathetervorrichtungen können auch dazu benutzt werden, Mechanismen für Stents, Shunts und dergleichen in den Schlemm'schen Kanal zu verabreichen, um den Kanal durchgängig zu halten, um die natürliche Drainage von wässriger Flüssigkeit zu erleichtern. Ferner können optische Fasern, Kameras, Temperatursensoren, Drucksensoren und eine beliebige andere besondere oder geeignete nützliche Vorrichtung durch die vorliegende Erfindung in den Schlemm'schen Kanal verabreicht werden.
- Die Erfindung sieht eine Kathetervorrichtung vor, die einen proximalen Abschnitt aufweist, der von einem Verwender manuell steuerbar ist, sowie einen distalen Abschnitt, der zum umfangsmäßigen Einsetzen in einen Abschnitt des Schlemm'schen Kanals gestaltet und bemessen ist. Die Vorrichtung umfasst ferner ein Aufpumpzufuhrlumen, das sich von dem proximalen Abschnitt zum distalen Abschnitt erstreckt. Der distale Abschnitt des Aufpumpzufuhrlumens kann aus einem elastischen expandierbaren Material konstruiert sein, um einen Abschnitt des Schlemm'schen Kanals radial zu erweitern, wenn er aufgepumpt ist. Das Aufpumpen kann durch Gas oder Flüssigkeitsinjektion in den proximalen Abschnitt des Aufpumpzufuhrlumens erreicht werden und kann auf Volumen und Druck sorgfältig überwacht werden, um das distale Lumen der Kathetervorrichtung auf den gewünschten Betrag akkurat zu dehnen. Daher kann sich der distale Abschnitt des Aufpumplumens zwischen einer ersten Einführposition und einer zweiten Aufpumpposition bewegen, wenn es über den proximalen Abschnitt aufgepumpt wird.
- Es ist bislang nicht bestimmt worden, dass der Schlemm'sche Kanal bei normalen Menschen über seinen gesamten Umfang durchgängig ist, im Gegensatz zur Unterteilung durch Septen in eine Vielzahl blind endender Kanäle. Die Erfindung nutzt diese neue Kenntnis für den Zugang zu dem Schlemm'schen Kanal und dazu, den Kanal mit den vorliegenden Vorrichtungen durchgängig zu halten.
- Der proximale Abschnitt der Kathetervorrichtung ist ausgestaltet, um einen Verbinder zum Anbringen an ein Aufpumpmittel aufzunehmen, wie etwa eine Injektionsspritze oder Pumpe. Die Vorrichtung kann auch ein Führungslumen aufweisen, das sich vom proximalen Abschnitt zum distalen Abschnitt erstreckt, worin das Führungslumen einen lenkbaren Führungsdraht enthält, um die Kathetervorrichtung in eine gewünschte Länge des Schlemm'schen Kanals zu führen.
- In einigen Ausführungen können durch die Kathetervorrichtung medizinische Zusammensetzungen zur Ablagerung im Schlemm'schen Kanal verabreicht werden. Dies kann durch Nutzung eines einzigen Lumens mit einer oder mehreren Öffnungen oder Fenstern im distalen Abschnitt davon erreicht werden. In einigen Fällen kann die Verabreichung eines Medikaments selbst ein inertes Material wie etwa ein Gel sein, um den Kanal ausreichend zu dehnen und zu erweitern, um hierdurch einen therapeutischen Effekt zu erzielen. Alternativ kann die Kathetervorrichtung ein separates Medikamentenverabreichungslumen und -aufpumplumen aufweisen, wie oben beschrieben. In einigen Ausführungen ist das Aufpumplumen konzentrisch an einer Stelle entlang dem distalen Abschnitt angeordnet, um einen Verschluss des Schlemm'schen Kanals entweder distal oder proximal des injizierten Materials vorzusehen.
- Die vorliegende Erfindung ist auf Vorrichtungen zum Erweitern des Schlemm'schen Kanals und/oder zum Verabreichen topisch aktiver Medikamente direkt in den Schlemm'schen Kanal unter Verwendung einer aufpumpbaren Vorrichtung gerichtet, die operativ in zumindest einem Abschnitt des Schlemm'schen Kanals eingesetzt wird. Der Abschnitt der Vorrichtung, der sich umfangsmäßig in dem Schlemm'schen Kanal erstreckt, kann aus einem flexiblen biologisch inerten Material hergestellt sein. Der distale Abschnitt der Kathetervorrichtung hat einen Durchmesser, der angenähert gleich jenem des Schlemm'schen Kanals eines menschlichen Auges ist. Der Außendurchmesser des distalen Abschnitts beträgt zwischen etwa 0,1 und 0,5 mm, oder bevorzugt etwa 0,3 mm. Die Länge des distalen Abschnitts beträgt zwischen etwa 1,0 und 20 mm, bevorzugt etwa 10 mm. Der distale Abschnitt der Kathetervorrichtung kann eine vorgeformte Krümmung mit einem Radius haben, der angenähert gleich jenem des Schlemm'schen Kanals ist. Der Radius des distalen Abschnitts kann zwischen etwa 3 und 10 mm, oder bevorzugt etwa 6 mm betragen.
- Eine Ausführung der vorliegenden Erfindung ist in
1 dargestellt, worin die Shuntvorrichtung100 in Seitenansicht gezeigt ist. Die Shuntvorrichtung100 ist aus zwei Abschnitten aufgebaut, einem proximalen Abschnitt30 , der mit einem oder mehreren distalen Abschnitten40 verbunden ist. Der proximale Abschnitt30 ist rohrförmig, wobei er ein oder mehrere Lumen in entweder einer konzentrischen oder parallelen Innenkonfiguration enthält. Im Ausführungsbeispiel der vorliegenden Erfindung ist der proximale Abschnitt30 aus einem biologisch inerten flexiblen Material aufgebaut, wie etwa Silikon oder ähnlichen Polymeren. Alternative Materialien können dünnwandiges Teflon, Polyprophylen oder andere Polymere oder Kunststoffe enthalten, sind aber darauf nicht beschränkt. - An seinem proximalen Ende ist der proximale Abschnitt
30 an einem Verbinder25 angebracht, der eine Fluidverbindung zwischen einem oder mehreren Lumina innerhalb des proximalen Abschnitts30 und Standardverbindern zur medizinischen Spritzenkupplung vorsieht. In dem Ausführungsbeispiel der vorliegenden Erfindung stellt der Verbinder eine solche Verbindung mit separatem Subverbindern10 und20 her. Der Subverbinder10 ist am Ende mit einer Spritzenkupplung5 verbunden, die für eine Verbindung mit einer Spritze (nicht gezeigt) sorgt, die zum Injizieren von Materialien in eines oder mehrere Verbindungslumina innerhalb des proximalen Abschnitts30 verwendet wird. Der Subverbinder20 ist am Ende mit einer Spritzenkupplung15 verbunden, die eine Verbindung für eine Spritze (nicht gezeigt) vorsieht, die zum Injizieren von Materialien in eines oder mehrere Verbindungslumina innerhalb des proximalen Abschnitts30 verwendet wird. Der Subverbinder20 kann auch den Durchtritt eines Führungsdrahts71 mit einer stumpfen Spitze72 dort hindurch erlauben. - Der proximale Abschnitt
30 des Katheters ist mit dem distalen Abschnitt40 verbunden. Das Lumen oder die Lumina des proximalen Abschnitts30 sind jeweils mit einem entsprechenden Lumen innerhalb des distalen Abschnitts40 verbunden. Der distale Abschnitt40 ist so bemessen und ausgestaltet, dass er im Schlemm'schen Kanal im Auge aufgenommen wird. In einer alternativen Ausführung kann der distale Abschnitt40 eine durchgehende verjüngte Verlängerung des proximalen Abschnitts30 sein. Der distale Abschnitt endet in einer Spitze70 , die verjüngt und/oder stumpf sein kann, und offen oder geschlossen sein kann. In der in1 gezeigten Ausführung erstreckt sich die distale Spitze des Führungsdrahts72 etwas über die Spitze70 des distalen Abschnitts40 der Vorrichtung hinaus. In der in1 gezeigten Ausführung hat der distale Abschnitt eine konzentrische aufpumpbare Hülse50 . In einer alternativen Ausführung kann sich distal an die aufpumpbare Hülse50 der distale Abschnitt40 als Fenster der Katheter60 fortsetzen, der ein oder mehrere Fenster65 enthält. -
2 zeigt einen Querschnitt am Punkt A-A' durch die Ausführung der vorliegenden Erfindung, wie sie in1 angegeben ist. In dieser Ausführung wird ein konzentrischer Doppellumenkatheter vorgesehen, mit einem Außenrohr44 und einem Innenrohr48 . Das Außenrohrlumen45 ist zwischen den Wänden des Außenrohrs44 und des Innenrohrs48 definiert. Innerhalb des Innenrohrs48 befindet sich ein Innenrohrlumen49 . - Längsquerschnitt, der den terminalen Aspekt des distalen Abschnitts
40 in dieser Ausführung der vorliegenden Erfindung detailliert, ist in3 gezeigt. In dieser Ausführung schließt sich das Außenrohr44 an die aufpumppbare Hülse50 an, und das Außenrohr44 endet in einem verschlossenen Ende55 am distalen Ende der aufpumpbaren Hülse50 . Das Innenrohr48 erstreckt sich durch die Mitte der aufpumpbaren Hülse50 und schließt sich distal an die aufpumpbare Hülse50 an. In der vorliegenden Ausführung enthält der gefensterte Katheter60 eine Vielzahl von Fenstern65 und endet an einer stumpfen verschlossenen distalen Katheterspitze70 . - In einer alternativen Ausführung der vorliegenden Erfindung, wie in
4 gezeigt, schließt sich die aufpumpbare Hülse50 als terminale Verlängerung des Innenrohrs48 an, und die Vorrichtung endet in einer stumpfen distalen Katheterspitze70 direkt distal der aufpumpbaren Hülse50 . In dieser Ausführung der vorliegenden Erfindung schließt sich das Außenrohr44 an den gefensterten Katheter60 terminal an, sodass die Fenster65 mit dem Außenlumen45 in Verbindung stehen. In dieser Ausführung wird die aufpumpbare Hülse50 durch das Innenlumen49 gesteuert, das in dauernder Verbindung mit dem Lumen der aufpumpbaren Hülse50 steht. Beliebige Materialien, die in dem Schlemm'schen Kanal injiziert werden sollen, werden durch das Außenlumen45 eingeführt und treten durch die Fenster65 in den Schlemm'schen Kanal proximal der aufpumpbaren Hülse50 hinein. - Die operative Anatomie, die für die vorliegende Erfindung relevant ist, ist in
5 dargestellt. Allgemein zeigt5 den Schlemm'schen Kanal110 und die Pupille120 , mit der anatomischen Beziehung dieser Struktur zu der anterioren Kammer135 , der Iris140 , der Hornhaut145 , dem Trabekel-Netzwerk150 und der Linse170 . - Die operative Anordnung und Funktionalität der vorliegenden Erfindung ist in den
6 –8 aus einer frontalen Perspektive gezeigt.6 zeigt die Verwendung einer Ausführung, wie jener, die in3 gezeigt ist, worin die aufpumpbare Hülse50 proximal des gefensterten Katheters60 ist. Der distale Abschnitt40 der Vorrichtung wird durch einen operativen Einschnitt105 in den Schlemm'schen Kanal110 eingefädelt, sodass die aufpumpbare Hülse50 vollständig im Kanal110 aufgenommen ist. - Der Führungsdraht
71 kann während der operativen Platzierung der Vorrichtung dazu benutzt werden, der Vorrichtung eine vorübergehende Steifigkeit zu verleihen, um deren Platzierung zu erleichtern. Sobald die Platzierung erreicht ist, kann der Führungsdraht71 herausgezogen werden, wobei die Vorrichtung100 in der gewünschten anatomischen Position verbleibt. - Sobald der distale Abschnitt
40 des Katheters zufriedenstellend im Schlemm'schen Kanal110 platziert ist, wird die aufpumpbare Hülse50 durch Injektion eines flüssigen Materials oder von Luft durch das Außenlumen45 des Katheters aufgepumpt. Das Aufpumpen der aufpumpbaren Hülse50 dient dazu, den Schlem'schen Kanal110 zu verschließen. Die anschließende Injektion eines gewünschten Materials durch das Innenlumen49 des Katheters wird durch den gefensterten Katheter60 hinausgedrückt, was zu einer örtlichen Dehnung des Schlemm'schen Kanals110 distal der aufpumpbaren Hülse50 führt. -
8 zeigt die Funktionalität einer Ausführung wie jener, wie zuvor in4 gezeigt ist, worin die aufpumpbare Hülse50 terminal angeordnet wird, und die Fenster65 in der Wand des Außenrohrs44 zur Katheterspitze hin liegen. Wenn die auffindbare Hülse50 durch Injektion in das Innenrohr48 aufgepumpt wird, wird der Schlemm'sche Kanal110 distal der aufgepumpten Hülse50 wirkungsvoll verschlossen. Eine anschließende Injektion eines gewünschten Materials durch das Außenlumen45 des Katheters wird durch den gefensterten Katheter60 hinausgedrückt, was zu einer örtlichen Dehnung des Schlemm'schen Kanals110 proximal der aufpumpbaren Hülse50 führt. - Die operative Prozedur, die zum Einführen der Vorrichtung erforderlich ist, erfordert einen Ansatz durch eine gewölbeartige Bindehautklappe. Dann wird eine Skleralklappe partieller Dicke hergestellt und halb dick in die klare Hornhaut eingeschnitten. Es wird ein radialer Einschnitt am Limbus unterhalb der Scleralklappe gemacht und vertieft, bis er posterior in den Schlemm'schen Kanal eintritt. Die anteriore Kammer kann mit der Injektion eines viskoelastischen und eines miotischen Wirkstoffs vertieft werden. Der distale Abschnitt der Kathetervorrichtung wird ergriffen und in den Schlemm'schen Kanal gefädelt. An der gewünschten Stelle wird die Kathetervorrichtung aufgepumpt, um den Schlemm'schen Kanal zu erweitern. Die Vorrichtung wird dann abgelassen und herausgezogen. Die Skleralklappe und die Bindehautwunde werden in herkömmlicher Weise verschlossen.
- Während die oben beschriebenen Ausführungen beispielhaft sind, befasst sich die Erfindung mit einer weiten Vielzahl von Formen und Konfigurationen des Katheters, um eine Fluidverbindung zwischen der anterioren Kammer und dem Schlemm'schen Kanal herzustellen. Die oben beschriebenen Ausführungen sollen daher den Umfang der Ansprüche nicht einschränken.
Claims (11)
- Ophthalmische Kathetervorrichtung, die einen proximalen Abschnitt (
30 ), der von Verwender manuell steuerbar ist, einen distalen Abschnitt (40 ) und ein Aufpumpzufuhrlumen (45 ), das sich von dem proximalen Abschnitt (30 ) zum distalen Abschnitt (40 ) erstreckt, aufweist, worin der distale Abschnitt (50 ) des Aufpumpzufuhrlumens (45 ) dehnbar ist, um einen Teil des Auges radial zu erweitern, wenn er aufgepumpt wird, dadurch gekennzeichnet, dass der distale Abschnitt (40 ) der Kathetervorrichtung einen Außendurchmesser von etwa 0,1 bis 0,5 mm und eine Länge von etwa 1,0 bis 20,0 mm aufweist, und dass der dehnbare Abschnitt in der Lage ist, den Durchmesser des Schlemm'schen Kanals (110 ) vorübergehend zu erweitern oder einen Abschnitt des Kanals (110 ) zu blockieren. - Kathetervorrichtung nach Anspruch 1, worin der distale Abschnitt (
40 ) eine vorgeformte Krümmung mit einem Radius hat, der dem Radius des Schlemm'schen Kanals (100 ) eines menschlichen Auges angenähert ist, wobei der Radius zwischen 3 mm und 10 mm beträgt. - Kathetervorrichtung nach Anspruch 1 oder 2, die ferner ein Führungslumen (
49 ) aufweist, das sich von dem proximalen Ende zum distalen Ende der Kathetervorrichtung erstreckt, worin das Führungslumen (49 ) einen lenkbaren Führungsdraht (71 ) enthält, um die Kathetervorrichtung in einen gewünschten Abschnitt des Schlemm'schen Kanals zu führen. - Kathetervorrichtung nach Anspruch 1 oder 2, die ferner ein Medikamentenverabreichungslumen (
49 ) aufweist, das sich vom proximalen Ende zum distalen Ende der Kathetervorrichtung erstreckt, worin das Medikamentenverabreichungslumen (49 ) am distalen Ende zumindest ein Fenster (65 ) aufweist, um Medikamente in den Schlemm'schen Kanal zu verabreichen. - Kathetervorrichtung nach Anspruch 1 oder 2, worin der distale Abschnitt (
40 ) einen Außendurchmesser von etwa 0,3 mm aufweist. - Kathetervorrichtung nach Anspruch 1 oder 2, worin der distale Abschnitt (
40 ) eine vorgeformte Krümmung mit einem Radius von etwa 6 mm aufweist. - Kathetervorrichtung nach Anspruch 1, die ferner ein Medikamentenverabreichungslumen (
49 ) aufweist, das sich vom proximalen Abschnitt (30 ) zum distalen Abschnitt (40 ) der Kathetervorrichtung erstreckt, worin das Medikamentenverabreichungslumen (49 ) am distalen Ende zumindest ein Fenster (65 ) aufweist, um Medikamente in den Schlemm'schen Kanal (110 ) zu verabreichen. - Kathetervorrichtung nach Anspruch 7, worin der distale Abschnitt (
40 ) eine vorgeformte Krümmung mit einem Radius aufweist, der dem Radius des Schlemm'schen Kanals (110 ) eines menschlichen Auges angenähert ist, wobei der Radius zwischen etwa 3 mm und 10 mm beträgt. - Kathetervorrichtung nach Anspruch 7 oder 8, die ferner ein Führungslumen (
49 ) aufweist, das sich vom proximalen Abschnitt (30 ) zum distalen Abschnitt (40 ) erstreckt, worin das Führungslumen (49 ) einen lenkbaren Führungsdraht (71 ) enthält, um die Kathetervorrichtung in einen gewünschten Abschnitt des Schlemm'schen Kanals (110 ) zu führen. - Kathetervorrichtung nach Anspruch 7, 8 oder 9, worin der distale Abschnitt (
40 ) der Kathetervorrichtung einen Außendurchmesser von etwa 0,3 mm aufweist. - Kathetervorrichtung nach Anspruch 7, 8 und 9, worin der distale Abschnitt (
40 ) der Kathetervorrichtung eine vorgeformte Krümmung mit einem Radius von etwa 6 mm aufweist.
