Description
- Gebiet der Erfindung
- Die Erfindung betrifft allgemein Vorrichtungen und kann in Verfahren zum temporären Anordnen eines Filters in einem Blutgefäß angewendet werden. Insbesondere stellt die Erfindung ein Führungsdrahtsystem zum Einfangen embolischen Materials in einer Arterie oder Vene während eines endovaskulären Eingriffs bereit. Das System stellt auch einen Stützdraht zur Führung und/oder zum Austausch von "Over-the-wire"-Vorrichtungen, wie z. B. Angioplastie-, Atherektomie- oder Stententfaltungskatheter, an einen interessierenden Bereich im Gefäß bereit. Zoll-Einheiten werden verwendet, wobei 1 Zoll gleich 2,54 cm ist.
- Hintergrund der Erfindung
- Die Behandlung thrombotischer oder atherosklerotischer Läsionen in Blutgefäßen unter Verwendung einer endovaskulären Methode hat sich kürzlich als eine effektive und zuverlässige Alternative zum chirurgischen Eingriff bei ausgewählten Patienten erwiesen. Zum Beispiel sind die direktionale Atherektomie und die perkutane transluminale Koronarangioplastie (PTCA) mit oder ohne den Einsatz eines Stents bei der Behandlung von Patienten mit Koronarokklusion anwendbar. Atherektomie entfernt physikalisch Plaque durch Schneiden, Pulverisieren oder Schaben in atherosklerotischen Arterien unter Verwendung einer mittels Katheter zuführbaren Endarteriektomie-Vorrichtung. Angioplastie vergrößert den Lumendurchmesser eines stenotischen Gefäßes durch Ausübung einer mechanischen Kraft auf die Gefäßwände. Neben der Anwendung von Angioplastie, Stenting und/oder Atherektomie im Koronargefäßsystem haben sich diese endovaskulären Techniken auch bei der Behandlung weiterer vaskulärer Läsionen als nützlich erwiesen, bei z. B. Karotisarterienstenose, peripherer Arterienverschlußkrankheit (besonders der A orta, der Hüftarterie und der Oberschenkelarterie), Nierenarterienstenose, bewirkt durch Atherosklerose oder Muskelbindegewebserkrankung, Vena-cava-superior-Syndrom und Hüftvenenverschlußthrombose, die gegenüber Thrombolyse resistent ist.
- Es ist allgemein anerkannt, daß eine der Komplikationen, die mit endovaskulären Verfahren verbunden sind, die Verschiebung embolischen Materials ist, das während der Gefäßmanipulation entsteht, wodurch eine Okklusion der dahinterliegenden engeren Gefäße und eine Ischämie oder ein Infarkt des Organs, das durch das Gefäß versorgt wird, bewirkt wird. 1995 offenbarten Waksman et al., daß distale Embolisation nach direktionaler Atherektomie in Koronararterien und Saphenavenentransplantaten häufig ist. Siehe Waksman et al., American Heart Journal 129(3): 430–5 (1995), dessen Inhalt hierin durch Bezugnahme aufgenommen wird. Diese Studie stellte fest, daß distale Embolisation bei 28% (31 von 111) der Patienten eintrat, die sich einer Atherektomie unterzogen hatten. Im Januar 1999 offenbarten Jordan, Jr. et al., daß die Behandlung der Karotisstenose unter Verwendung perkutaner Angioplastie mit Stenting mit mehr als dem Achtfachen der Mikroembolie-Rate verbunden ist, die bei der Anwendung der Karotisendartiektomie auftrat. Siehe Jordan, Jr. et al., Cardiovascular surgery 7(1): 33–8 (1999), dessen Inhalt hierin durch Bezugnahme aufgenommen wird. Es hat sich gezeigt, daß Mikroembolie, wie durch transkraniale Doppler-Überwachung in dieser Studie festgestellt wurde, eine potentielle Ursache für Schlaganfall ist. Die embolischen Materialien weisen Calcium, Intimadebris, Atheroma-Plaque, Thromben und/oder Luft auf.
- Es gibt eine Anzahl von Vorrichtungen, die dafür ausgelegt sind, eine Blutfilterung zum Einfangen vaskulärer Emboli bereitzustellen. Die große Mehrzahl dieser Vorrichtungen ist für eine dauerhafte Anordnung in Venen ausgelegt, um eine Lungenembolie zu verhindern. Eine temporäre Venenfiltervorrichtung ist in Bajaj, US-Patent 5 053 008 offenbart. Die Vorrichtung von Bajaj ist ein intrakardialer Katheter zum temporären Anordnen im Truncus pulmonalis eines Patienten, der z. B. durch Hüftoperation, schwere Verletzung, schwere Abdominal- oder Beckenoperation oder Immobilisierung für eine Lungenembo lie prädisponiert ist. Die Vorrichtung von Bajaj weist einen Schirm auf, der aus einem Netzwerk hergestellt ist und der venöse Emboli auffängt, bevor sie die Lungen erreichen. Diese Vorrichtung ist zur venösen Filtration ausgelegt und wegen der hämodynamischen Unterschiede zwischen Arterien und Venen nicht zur arteriellen Anwendung geeignet.
- Es gibt sehr wenige intravaskuläre Vorrichtungen, die für arterielle Anwendung ausgelegt sind. Arterien sind viel flexibler und elastischer als Venen, und in den Arterien pulsiert der Blutfluß mit großen Druckunterschieden zwischen dem systolischen und diastolischen Fluß. Diese Druckunterschiede bewirken, daß sich die Arterienwände ausdehnen und zusammenziehen. Die Blutflußraten in den Arterien variieren von etwa 1 bis etwa 5 l/min. Das US-Patent 4 873 978 von Ginsburg offenbart ein arterielles Filtersystem, das einen Katheter mit einer Siebvorrichtung an seinem distalen Ende aufweist. Diese Vorrichtung wird in Fließrichtung hinter bzw. stromabwärts der Behandlungsstelle in das Blutgefäß eingeführt, und nach der Behandlung wird das Sieb um die eingefangenen Emboli herum zusammengeklappt und aus dem Körper entfernt. Die Vorrichtung von Ginsburg kann Flußraten von 5 l/min nicht standhalten. Sie ist nur für kleine Arterien ausgelegt und kann daher keine Emboli einfangen, die in alle Körperteile unterwegs sind. DE 34 17 738 , offenbart ebenfalls weitere arterielle Filter mit einem Faltgestängesystem, das das Filter vom zusammengeklappten in den expandierten Zustand umwandelt. - Am distalen Ende von Führungsdrähten angeordnete Filter wurden für eine intravaskuläre Blutfiltration vorgeschlagen. Die meisten dieser Vorrichtungen weisen ein Filter auf, das an einem Führungsdraht angebracht ist und über Streben oder einen vorgeformten Korb, der sich im Gefäß entfaltet, mechanisch betätigt wird. Diese Filter sind typischerweise Netz-"Fallschirme", die am Schaft des Drahts am distalen Ende und an Drahtstreben angebracht sind, die sich an ihrem proximalen Ende in einer radialen Richtung nach außen erstrecken. Die radialen Streben öffnen das proximale Ende des Filters zur Wand des Gefäßes. Blut, das durch das Gefäß fließt, wird durch das Netz gedrückt, wodurch embolisches Material im Filter eingefangen wird. Diese Vorrichtungen sind selbstlenkend und können intravaskulär angeordnet werden. Ein mit den gegenwärtigen Vorrichtungen verbundener Hauptnachteil ist jedoch, daß sich die Lenkbarkeit des Führungsdrahts im Vergleich zu den herkömmlichen Führungsdrähten durch die Filtergröße ändern kann. Der Führungsdraht kann sich biegen, knicken und/oder sich im Gefäß schlängeln, was das Einfügen des Filters durch eine komplexe vaskuläre Läsion schwierig macht.
- Während endovaskulärer Eingriffe ist es nicht ungewöhnlich, eine endovaskuläre Vorrichtung über den Führungsdraht gegen eine andere auszutauschen. Die Position des Führungsdrahts bleibt jedoch während des Austauschs der Vorrichtungen oft nicht erhalten oder wird beeinträchtigt. Zum Beispiel ist es während einer koronaren Revaskularisation oft erforderlich, einen Führungskatheter gegen einen anderen Führungskatheter mit anderen Eigenschaften, z. B. einem größeren Führungsdurchmesser, um eine spezielle angioplastische Vorrichtung zuzuführen, einem kleineren Führungsdurchmesser, um tiefe Intubation und/oder Druckdämpfung zu verhindern, einer anderen Führungsform oder einen Führungskatheter mit Seitenlöchern auszutauschen. Es ist bekannt, daß es wenige Eingriffsmanöver gibt, die so herausfordernd sind wie der Versuch, den distalen Führungsdrahtzugriff zu erhalten und gleichzeitig zu versuchen, einen Führungskatheter gegen einen anderen auszutauschen, ohne die Position des Führungsdrahts zu beeinträchtigen.
- Was gebraucht wird, sind einfache und sichere Blutfilter- und Führungsdrahtsysteme, die temporär in den Arterien und Venen angeordnet werden können, um distale Embolisation bei endovaskulären Eingriffen zu verhindern, und die verwendet werden können, um verschiedene Instrumente an einen interessierenden Bereich zu führen und/oder auszutauschen, ohne die Position des Filters oder Führungsdrahts zu beeinträchtigen. Bestehende Vorrichtungen sind für diesen Zweck unzureichend.
- FR-A-2 666 980 offenbart ein perkutanes Filtersystem, wo ein Führungsdraht durch einen Stützkatheter und durch die zylindrische Spitze eines expandierbaren Filters eingeführt wird.
- Zusammenfassung der Erfindung
- Die Erfindung stellt Vorrichtungen zum Einführen von endovaskulären Vorrichtungen, z. B. Führungskatheter, Atherektomie-Katheter, Angioplastie-Katheter, intravaskuläre Ultraschall-Katheter oder Stent-Entfaltungskatheter, und zum Schutz eines Patienten vor distaler Embolisation bei kardiovaskulären Eingriffen bereit. Insbesondere wird ein geführtes Filtersystem mit einem Stützdraht zum Einfangen embolischen Materials, das während des Eingriffs erzeugt wird, und zur Führung oder zum Austausch von Vorrichtungen an einen interessierenden Bereich in einer Arterie oder Vene offenbart.
- In einer Ausführungsform weist das Filtersystem einen Führungsdraht und einen Stützdraht mit einem an einem distalen Bereich des Stützdrahts angeordneten expandierbaren Filter auf, z. B. einem Fallschirm, einem Korb oder einer Spirale. Der Stützdraht ist zum perkutanen Einführen in eine Arterie oder Vene angepaßt und ist dafür angepaßt, ein endovaskuläres Instrument aufzunehmen. Der distale Bereich des Stützdrahts weist eine Drahtführung auf, die mit dem Führungsdraht gleitfähig in Eingriff ist. Die Drahtführung weist einen Ring mit einer Öffnung auf, die dafür angepaßt ist, den Führungsdraht aufzunehmen.
- In einer weiteren Ausführungsform weist das Filter einen Expansionsrahmen und ein Netz auf, das über dem Rahmen angeordnet ist. Das Filter kann in einem zusammengeklappten Zustand angeordnet sein, um das Eintreten in ein Gefäß zu ermöglichen, und in einem vergrößerten Zustand, um embolisches Material im Gefäß einzufangen. In bestimmten Ausführungsformen weist der Rahmen mehrere Streben auf, die an einem ersten Ende mit dem Führungsdraht verbunden sind, und die Streben expandieren an einem zweiten Ende radial nach außen. Die Bauart und Verwendung der Expansionseinrichtung und der zugehörigen Filternetze wurden in früheren Publikationen einschließlich Barbut et al., US-Patent 5 662 671, eingehend beschrieben.
- Die Erfindung kann in Verfahren verwendet werden, die den Einsatz eines perkutanen medizinischen Instruments während eines endovaskulären Eingriffs einschließen, um Plaque und/oder Thromben aus den Koronararterien, der Aorta, Arteria carotis communis, Arteria carotis externa und interna, des Truncus brachiocephalicus, der Arteria cerebri media, Arteria basilaris, Arteria subclavia, Arteria brachialis, Arteria axillaris, Arteria iliaca, Arteria renalis, Arteria femoralis, Arteria poplitea, Arteria coeliaca, Arteria mesenterica superior, Arteria mesenterica inferior, Arteria tibialis anterior, Arteria tibialis posterior und allen weiteren Arterien, die sauerstoffreiches Blut führen, zu entfernen. Die Verfahren schließen auch die Vorbeugung gegen distale Embolisation während eines endovaskulären Eingriffs ein, um Thromben und/oder Fremdkörper im venösen Kreislauf, einschließlich der Vena cava superior, Vena cava inferior, Vena jugularis externa und interna, Vena brachiocephalica, Arteria pulmonalis, Vena subclavia, Vena brachialis, Vena axillaris, Vena iliaca, Vena renalis, Vena femoralis, Vena profunda femoris, Vena saphena magna, Vena portae, Vena lienalis, Vena hepatica und Vena azygos, zu entfernen.
- In einem ersten Verfahren zur Verwendung des geführten Filtersystems wird das distale Ende des Führungsdrahts perkutan durch eine Arterie oder Vene eingeführt und in einen interessierenden Bereich oder über diesen hinaus, typischerweise eine stenotische Läsion, die durch Aufbau von atherosklerotischer Plaque und/oder Thromben verursacht ist, vorgeschoben. In einem zusammengeklappten Zustand werden das Filter und der distale Bereich des Stützdrahts über den Führungsdraht vorgeschoben, während die Drahtführung des Stützdrahts mit dem Führungsdraht in Eingriff ist, d. h. wie ein Monorail-Katheter, der mit einem Führungsdraht in Eingriff ist. Das Filter wird in Fließrichtung hinter bzw. stromabwärts der vaskulären Okklusion expandiert, und der Führungsdraht wird herausgezogen und aus dem Körper entfernt. Der distale Bereich einer endovaskulären Vorrichtung, z. B. ein Atherektomie-, Stent-Entfaltungs- oder Angioplastie-Katheter, wird über den Stützdraht eingeführt und zum interessierenden Bereich vorgeschoben. Nachdem die stenotische Läsion entfernt oder durch die endovaskuläre Vorrichtung anderweitig behandelt worden ist und ein angemessener Lumendurchmesser geschaffen worden ist, wird das Filter zusammengeklappt und gemeinsam mit dem eingefange nen embolischen Debris aus dem Gefäß durch Herausziehen des Stützdrahts entfernt.
- In einem weiteren Verfahren wird, nachdem der Führungsdraht und der Stützdraht mit dem expandierten Filter in einem Gefäß distal zum interessierenden Bereich positioniert worden sind, die endovaskuläre Vorrichtung sowohl über den Führungsdraht als auch den Stützdraht eingeführt, um sich im interessierenden Bereich zu positionieren. Bei bestimmten kardiovaskulären Eingriffen, insbesondere bei koronarer Revaskularisation, ist der Austausch endovaskulärer Instrumente und Katheter notwendig und ist schwer zu verwirklichen, da die anfängliche Führungsdrahtpositionierung im interessierenden Bereich oft nicht erhalten bleibt, wenn die erste Vorrichtung herausgezogen wird. Unter Verwendung des geführten Filtersystems werden sowohl der Führungsdraht als auch der Stützdraht zum interessierenden Bereich distal vorgeschoben. Wenn die Position des Führungsdrahts während des Herausziehens der ersten Vorrichtung nicht erhalten bleibt, kann die zweite Vorrichtung, die im Austausch eingeführt werden muß, über den Stützdraht vorgeschoben werden, um im interessierenden Bereich positioniert zu werden.
- Es ist verständlich, daß es verschiedene Vorteile bei der Verwendung der hierin offenbarten Vorrichtungen zum Einfangen und Entfernen des embolischen Debris bei endovaskulären Eingriffen gibt. Das geführte Filtersystem (1) ist beispielsweise insbesondere zur temporären Blutfiltration in einem beliebigen Gefäß gut geeignet, um embolischen Debris einzufangen, wodurch neurologische, kognitive und kardiale Komplikationen, die mit distaler Embolisation verbunden sind, minimiert werden, (2) kann hohem arteriellen Blutfluß über einen ausgedehnten Zeitraum standhalten, (3) weist ein Netz auf, das ausreichend porös ist, um adäquaten Blutfluß in einem Blutgefäß während des Einfangens von Emboli zu ermöglichen, (4) kann verwendet werden, um einen endovaskulären Katheter zu einem interessierenden Bereich im Blutgefäß zu führen, (5) kann verwendet werden, um medizinische Instrumente auszutauschen, ohne die Position des Führungsdrahts zu beeinträchtigen, und (6) kann bei erwachsenen und nichterwachsenen Patienten verwendet werden.

- In einer ersten Ausführungsform wird ein Filtersystem zur temporären Anordnung in einem Gefäß, entweder einer Arterie oder einer Vene, bereitgestellt, wie in
1A ,1B ,1C und1D dargestellt. Das Filtersystem weist einen Stützdraht10 mit einem proximalen Ende, einem distalen Bereich11 und einem expandierbaren Filter20 auf, das am distalen Bereich angeordnet ist. Das Filter weist einen Expansionsrahmen22 und ein Netz25 auf, das mit den Streben28 des Expansionsrahmens ultraschallverschweißt oder verklebt ist. Antikoagulationsmittel, z. B. Heparin und Heparinoide, können auf das Netz25 angewendet werden, um die Thrombenbildung auf dem Netz zu verringern. Das Filter kann wie in1A gezeigt zusammengeklappt sein, um das Einführen in ein Gefäß zu ermöglichen, und danach wie in1B gezeigt expandiert werden. Eine Drahtführung26 ist im distalen Bereich11 des Stützdrahts vorhanden. Die Drahtführung kann im Filter (wie in1B und1C gezeigt) oder an jeder anderen geeigneten Position auf dem Stützdraht10 proximal zum Filter (wie in1E gezeigt) oder auf einer distalen Erweiterung des Stützdrahts angeordnet sein, die sich hinter dem Filter erstreckt (wie in1F gezeigt). In bestimmten Ausführungsformen kann der Stützdraht einen Ring aufweisen. Eine Schnittansicht des Stützdrahts durch die Schnittlinie C-C ist in1C dargestellt. Die Gestaltung und der Aufbau einer Vielzahl von Filtern zur Verwendung auf einem Führungsdraht sind in Tsugita et al., US-Patent 5 911 734, ausführlich beschrieben. - In einer weiteren Ausführungsform weist das Filter ferner eine Umhüllung bzw. Hülse auf, die das Filter bedeckt und vom Filter entfernbar ist, wobei die Umhüllung eine Öffnung in ihrem distalen Bereich hat, die dafür angepaßt ist, den Führungsdraht in der Art eines Schnellwechselkatheters aufzunehmen. In
1G ist der Stützdraht10 in ein Lumen51 eines Schnellwechselkatheters50 eingeführt. Der Katheter weist einen Seitenstutzen bzw. Seitenöffnung60 in seinem distalen Bereich auf, die dafür angepaßt ist, den Führungsdraht30 aufzunehmen. In1I weist der Katheter einen Spalt61 auf, der den Führungsdraht30 aufnimmt. In1J transportiert ein langgestrecktes Teil70 ein röhrenförmiges Segment75 mit einem Spalt77 an einem distalen Bereich des langgestreckten Teils70 . Das röhrenförmige Segment wirkt als eine Umhüllung für das Filter, während der Spalt den Führungsdraht aufnimmt. In1K transportiert das langgestreckte Teil70 ein erstes und zweites röhrenförmiges Segment75 und79 , die dafür angepaßt sind, das Filter bzw. den Führungsdraht aufzunehmen. - Bei Verwendung wird der Führungsdraht
30 zunächst in ein Gefäß eingeführt und zum interessierenden Bereich distal vorgeschoben. Der Katheter, der das Filter im Lumen51 transportiert, wird über den Führungsdraht eingeführt, wobei der Führungsdraht über die Seitenöffnung60 in Eingriff ist. Das Filter wird distal vorgeschoben, wobei es die atheromatöse Läsion100 passiert. Der Führungsdraht kann dann herausgezogen werden und der Katheter50 proximal gezogen werden, wobei der Katheter und das Filter im Gefäß eingeführt bleiben, wie in1H dargestellt. Der Katheter50 wird dann aus dem Gefäß entfernt. Der Expansionsrahmen22 wird expandiert, um embolische Materialien stromabwärts der atheromatösen Läsion einzufangen. Eine endovaskuläre Vorrichtung, wie z. B. ein Angioplastie-Katheter mit oder ohne Stent kann über den Stützdraht10 eingeführt werden, um sich nahe dem Atherom100 zu plazieren. Nachdem die vaskulären Eingriffe mit der/den endovaskulären Vorrichtung(en) durchgeführt worden sind, wird/werden die Vorrichtung(en) herausgezogen und aus dem Gefäß entfernt. Das Filter mit den eingefangenen Emboli wird dann zusammengeklappt und entfernt. - Das Filtersystem weist auch einen Führungsdraht
30 mit einem proximalen Ende und einem distalen Ende33 auf. Der Führungsdraht wird vom Stützdraht10 über die Drahtführung26 gleitfähig aufgenommen, wie in1D dargestellt. Das Filtersystem weist ferner endovaskuläre Vorrichtungen, wie z. B. Atherektomie-Katheter, endovaskuläre Bildgebungsvorrichtungen, Stent-Entfaltungskatheter, Angioplastie-Katheter, Drucküberwachungseinrichtungen, elektrophysiologische Katheter und Aspirationsapparate auf, die dafür angepaßt sind, den Führungsdraht30 und/oder Stützdraht10 in ihr Lumen aufzunehmen. - Verschiedene Bauarten des distalen Endes
33 des Führungsdrahts sind in2A ,2B und2C dargestellt. Das distale Ende33 kann eine im wesentlichen lineare Konfiguration relativ zum proximalen Ende des Führungsdrahts annehmen, wie in2A dargestellt. Alternativ kann das distale Ende33 eine angewinkelte Konfiguration relativ zum proximalen Ende des Führungsdrahts haben, wie in2A dargestellt. Das distale Ende33 kann wie ein Angelhaken geformt sein, wie in2C dargestellt. Der distale Bereich des Führungsdrahts kann aus einem flexiblen Material bestehen, um den Durchtritt durch einen interessierenden Bereich zu ermöglichen, und ist vorzugsweise mit einer atraumatischen Spitze ausgerüstet, wie sie in der Fachwelt bekannt ist. Die Ausführungsformen in2B und2C mit einer gekrümmten Gestaltung sind insbesondere nützlich, um Zugang zu einer komplexen Läsion in einem gewundenen Gefäß zu erlangen. -
3A und3B stellen alternative Ausführungsformen des expandierbaren Filters20 dar, das im distalen Bereich des Stützdrahts10 angeordnet ist. In3A weist das Filter20 einen Fallschirmrahmen und ein Netz25 auf, das mit den Streben28 verschweißt (z. B. Ultraschall oder Laser) oder verklebt ist. Die Drahtführung26 ist im distalen Bereich des Stützdrahts enthalten und steht distal vom Filter20 vor, um mit einem Führungsdraht in Eingriff zu treten. In3B weist das Filter20 zusammendrückbare Streben22 und ein Netz25 auf. In einem expandierten Zustand nimmt das Filter20 die Form eines Schneebesens an. - Wenn als Beispiel das hierin offenbarte Filtersystem für die Verwendung in der Aorta vorgesehen ist, wird die Netzfläche, die für die Vorrichtung erforderlich ist, nach der Bernoulli-Gleichung berechnet, wie in unseren früheren Anmeldungen einschließlich Barbut et al. beschrieben.
- In einer Ausführungsform des geführten Filtersystems, die in der Aorta anzuwenden ist, ist ein Netz mit Abmessungen in folgenden Bereichen erwünscht: die Netzfläche ist 0,004 bis 5 Quadratzoll, besonders bevorzugt 0,007 bis 4 Quadratzoll, besonders bevorzugt 0,010 bis 3 Quadratzoll, besonders bevorzugt 0,015 bis 2 Quadratzoll, besonders bevorzugt 0,020 bis 1 Quadratzoll, besonders bevorzugt 0,025 bis 0,076 Quadratzoll; die Netzdicke ist 60 bis 280 μm, besonders bevorzugt 70 bis 270 μm, besonders bevorzugt 80 bis 260 μm, besonders bevorzugt 90 bis 250 μm, besonders bevorzugt 100 bis 250 μm, besonders bevorzugt 120 bis 230 μm, besonders bevorzugt 140 bis 210 μm; der Fadendurchmesser ist 30 bis 145 μm, besonders bevorzugt 40 bis 135 μm, besonders bevorzugt 50 bis 125 μm, besonders bevorzugt 60 bis 115 μm, besonders bevorzugt 70 bis 105 μm, und die Porengröße ist 500 μm oder weniger, besonders bevorzugt 400 μm oder weniger, besonders bevorzugt 300 μm oder weniger, besonders bevorzugt 200 μm oder weniger, besonders bevorzugt 100 μm oder weniger, besonders bevorzugt 50 μm oder weniger und normalerweise größer als mindestens ein rotes Blutkörperchen. In einer bevorzugten Ausführungsform der Erfindung ist die Netzfläche 2 bis 8 Quadratzoll, die Netzdicke ist 60 bis 200 μm, der Fadendurchmesser ist 30 bis 100 μm und die Porengröße ist 50 bis 300 μm. In einer ferner bevorzugten Ausführungsform der Erfindung ist die Netzfläche 3 bis 5 Quadratzoll, die Netzdicke ist 60 bis 150 μm, der Fadendurchmesser ist 50 bis 80 μm und die Porengröße ist 100 bis 250 μm. In weiteren Ausführungsformen weist das Filter eine dünne lasergetrennte Schicht mit Löchern auf, um den Blutfluß zu ermöglichen. Typische Abmessungen weisen eine Porengröße von 20 bis 500 μm, eine Dicke von 0,0005 bis 0,003 Zoll und eine Fläche auf, die annähernd die gleiche ist wie für oben beschriebene Netze.
- In weiteren Ausführungsformen weist das Filter eine dünne lasergetrennte Schicht mit Löchern auf, um den Blutfluß zu ermöglichen. Typische Abmessungen weisen eine Porengröße von 20 bis 500 μm, eine Dicke von 0,0005 bis 0,003 Zoll und eine Fläche auf, die annähernd die gleiche ist wie für oben beschriebene Netze.
- Wenn geeignete physikalische Merkmale bestimmt sind, kann unter den bekannten Standardnetzen ein geeignetes Netz gefunden werden. Zum Beispiel können Polyester-Netze verwendet werden, wie z. B. von Saati Corporations und Tetko Inc. hergestellte Netze. Diese sind in Bahnform erhältlich und können einfach geschnitten und zu einer gewünschten Form ausgebildet werden. In einer bevorzugten Ausführungsform ist das Netz zu einer Kegelform verschweißt (z. B. Ultraschall oder Laser) oder vernäht. Andere bekannte Netze, die die gewünschten physikalischen Merkmale haben, sind auch geeignet. Antikoagulationsmittel, z. B. Heparin und Heparinoide, können auf das Netz aufgebracht werden, um die Möglichkeiten der Blutgerinnung am Netz zu verringern. Andere Antikoagulationsmittel als Heparinoide können auch verwendet werden, z. B. monoklonale Antikörper wie etwa ReoPro (Centocor). Das Antikoagulationsmittel kann auf das Netz gestrichen oder gesprüht werden. Ein chemisches Tauchbad mit dem Antikoagulationsmittel kann auch verwendet werden. Weitere bekannte Verfahren zum Aufbringen der Chemikalien auf das Netz können angewendet werden.
- Bei Verwendung wird, wie in
4A dargestellt, der Führungsdraht30 perkutan durch eine periphere Arterie oder Vene eingeführt und typischerweise in der Richtung des Blutflusses vorgeschoben. Der Führungsdraht30 kann jedoch in einer Richtung entgegen dem Blutfluß eingeführt und vorgeschoben werden, z. B. rückläufig durch die Aorta descendens, um die Koronararterie zu erreichen. Das distale Ende33 des Führungsdrahts wird durch die Okklusionsläsion100 geführt, typischerweise eine Atheroma-Plaque, und distal zu der Okklusion positioniert. Der Stützdraht10 von1A wird über das proximale Ende des Führungsdrahts30 durch die Drahtführung26 einge führt und distal vorgeschoben, bis das Filter20 zur Plaque100 distal positioniert ist, wie in4B dargestellt. Wenn man die Drahtführung26 mit dem Führungsdraht in Eingriff treten läßt, können das Filter und der Stützdraht intravaskulär leicht gelenkt werden, um den interessierenden Bereich zu erreichen. Das Filter20 wird expandiert, um embolisches Material, wie z. B. Calcium, Thromben, Plaque und/oder Gewebedebris, einzufangen. Der Führungsdraht30 wird dann herausgezogen und läßt den Stützdraht10 in einer entsprechenden Position, um eine endovaskuläre Vorrichtung zur Plaque100 zu führen. - Perkutane transluminale Angioplastie ist bei der Behandlung arterieller Stenosen sowie okklusiver venöser Thrombose, die gegenüber Thrombolyse resistent ist, bisher erfolgreich gewesen. Siehe American Heart Journal, 125(2Pt1): 362–6 (1993). Der Angioplastie-Katheter
40 , an dessen distalen Bereich ein Angioplastie-Ballon42 angeordnet ist, wird über den Stützdraht10 eingeführt, wie in4C dargestellt. In einem entleerten Zustand wird der Angioplastie-Ballon über den Stützdraht10 in eine Position nahe der Plaque100 vorgeschoben. Die Atheroma-Plaque wird durch Aufblasen des Ballons42 zusammengedrückt, wodurch die Stenose in dem Gefäß aufgeweitet wird. In bestimmten Ausführungsformen weist der Angioplastie-Katheter eine Infusionsöffnung44 proximal und einen Perfusionsöffnung45 distal zum Ballon42 auf. Die Infusionsöffnung44 kann verwendet werden, um pharmazeutische Wirkstoffe, z. B. t-PA, Adenosin oder Nitroglyzerin, durch das Katheterlumen (nicht dargestellt) zu verabreichen. Ein sauerstoffreiches Medium oder sauerstoffreiches Blut kann durch die Öffnung45 infundiert werden, um die Durchblutung distaler Organe während der Angioplastie aufrechtzuerhalten. In bestimmten Ausführungsformen steht ein Stent mit dem Angioplastie-Ballon in enger Verbindung. Der Stent ist typischerweise auf dem Ballon gefaltet und ist bei Anwendung einer radialen, nach außen gerichteten Kraft aus dem Inneren des Stents zu einer kontrollierten radialen Expansion im interessierenden Bereich fähig. Die Bauart des Kathetersystems, das einen Stent trägt, ist in Jang et al., US-Patent 5 749 848, ausführlich beschrieben. - Der Angioplastie-Katheter oder ein anderes endovaskuläres Instrument wird nach Beendigung der Angioplastie aus dem Gefäß herausgezogen. Embolisches Material, das während der Angioplastie entstand, wird vom Filter
20 eingefangen und gehalten. Das Filter wird dann zusammengezogen und mit dem eingefangenen embolischen Material aus dem Gefäß herausgezogen und aus dem Körper des Patienten entfernt. - Alternativ können, nachdem das Filter
20 positioniert und distal zur Plaque100 expandiert worden ist, der Führungsdraht30 und der Stützdraht10 im Gefäß an der Plaque100 verbleiben, wie in4D dargestellt. Der Angioplastie-Katheter40 wird dann sowohl über den Führungsdraht30 als auch den Stützdraht10 in eine Position nahe der Plaque100 geführt. Wenn beispielsweise eine Atherektomie-Vorrichtung erforderlich ist, um Plaque zu entfernen, die nach der Angioplastie verbleibt, wird der Angioplastie-Katheter40 mit oder ohne den Führungsdraht herausgezogen, und ein Atherektomie-Katheter wird über den Führungsdraht30 und/oder Stützdraht10 in eine Position nahe der Plaque geführt. Wenn auf diese Weise die Position des Führungsdrahts30 an der Plaque während des Entfernens des Angioplastie-Katheters40 nicht erhalten bleibt, ist der Stützdraht10 verfügbar, um eine weitere endovaskuläre Vorrichtung zum interessierenden Bereich zu führen. Dieses Verfahren ist insbesondere zum Austausch von Führungskathetern während koronarer Revaskularisation nützlich. - Die Länge des Führungsdrahts und des Stützdrahts ist im allgemeinen zwischen 30 und 300 Zentimetern, vorzugsweise annähernd zwischen 50 und 180 Zentimetern. Das Filter kann zu einem Außendurchmesser von mindestens 0,2 Zentimetern, besonders bevorzugt mindestens 0,5 Zentimeter, besonders bevorzugt mindestens 1,0 Zentimeter, besonders bevorzugt mindestens 1,5 Zentimeter, besonders bevorzugt mindestens 2,0 Zentimeter, besonders bevorzugt mindestens 2,5 Zentimeter, besonders bevorzugt mindestens 3,0 Zentimeter, besonders bevorzugt mindestens 3,5 Zentimeter, besonders bevorzugt mindestens 4,0 Zentimeter, besonders bevorzugt mindestens 4,5 Zentimeter, besonders bevorzugt mindestens 5,0 Zentimeter, expandieren. Diese Bereiche schließen geeignete Durchmesser für die Anwen dung sowohl bei Kindern als auch Erwachsenen ein. Die vorstehend genannten Bereiche sind ausschließlich zum Zweck der Darstellung typischer Vorrichtungsabmessungen beschrieben. Die tatsächlichen Abmessungen einer gemäß den Prinzipien der Erfindung aufgebauten Vorrichtung können offenkundig außerhalb der aufgeführten Bereiche variieren, ohne von diesen Basisprinzipien abzuweichen.
- Obwohl die vorstehend beschriebene Erfindung der Deutlichkeit und Verständlichkeit halber durch Darstellung und Beispiele mit gewisser Ausführlichkeit beschrieben worden ist, ist es jedoch offenkundig, daß bestimmte Änderungen und Modifikationen möglich sind, die noch in den Schutzbereich der beigefügten Ansprüche fallen. Außerdem wird verständlich, daß jegliche Merkmale, die für eine beliebige gegebene Ausführungsform beschrieben worden sind, mit jeder der anderen hierin beschriebenen Ausführungsformen kombiniert werden können.
Claims (14)
- Perkutanes Filtersystem mit: einem Führungsdraht (
30 ); einem Stützdraht (10 ) mit einem proximalen Ende, einem distalen Ende und einem distalen Bereich (11 ), wobei der Stützdraht (10 ) dafür angepaßt ist, ein perkutanes medizinisches Instrument aufzunehmen; einem expandierbaren Filter (20 ), das im distalen Bereich (11 ) des Stützdrahts (10 ) angeordnet ist; und gekennzeichnet durch eine Drahtführung (26 ) mit einem Ring, die im distalen Bereich des Stützdrahts angeordnet ist und für einen Gleiteingriff mit dem Führungsdraht angepaßt ist, wobei der Stützdraht dafür angepaßt ist, entlang des Führungsdrahts vorgeschoben zu werden. - System nach Anspruch 1, wobei das expandierbare Filter (
20 ) manuell aktiviert wird. - System nach Anspruch 1, wobei das expandierbare Filter (
20 ) selbstaktivierend ist. - System nach Anspruch 1, wobei die Drahtführung (
26 ) einen Ring mit einer Öffnung aufweist, die dafür angepaßt ist, den Führungsdraht (30 ) aufzunehmen. - System nach Anspruch 1, wobei das expandierbare Filter (
20 ) mehrere Streben (22 ;28 ) und ein Netz (25 ) aufweist, das um die Streben angeordnet ist. - System nach Anspruch 5, wobei das Netz (

25 ) ein dünner Film mit Löchern ist, die einen Fluidfluß ermöglichen. - System nach Anspruch 1, wobei das Filter zur Form eines Schneebesens expandiert.
- System nach Anspruch 1, ferner mit einem perkutanen medizinischen Instrument, das aus der Gruppe gewählt ist, die aus einem Angioplastie-Katheter (
40 ), einem Stent-Entfaltungskatheter, einem Atherektomie-Katheter, einem intravaskulären Ultraschall-Katheter und einem Aspirations-Katheter besteht. - System nach Anspruch 9, wobei der Stent-Entfaltungskatheter eine Hülle aufweist, die um einen selbstexpandierenden Stent angeordnet ist.
- System nach Anspruch 1, wobei die Drahtführung auf dem Stützdraht im Filter angeordnet ist.
- System nach Anspruch 1, wobei die Drahtführung auf dem Stützdraht distal zum Filter angeordnet ist.
