Description
- Das Fachgebiet, auf das sich diese Erfindung bezieht, ist chirurgische Befestigungsvorrichtungen, insbesondere chirurgische Befestigungsvorrichtungen zum Befestigen von Gewebetransplantaten am Knochen.
- Stand der Technik
- Die Medizintechnik, die mit in vitro-Züchtung biologischen Gewebes bzw. Tissue Engineering in Zusammenhang steht, hat sich rapide weiterentwickelt. Insbesondere ist nun bekannt, wie man Zellen aus dem menschlichen Körper erntet, zum Beispiel Chondrozyten und Fibrochondrozyten aus dem Kniegelenk. Diese autologen Zellen werden dann auf einer bioabsorbierbaren Matrix in einer Laborumgebung gezüchtet. Die Matrix weist typischerweise eine Form auf, die im wesentlichen dem Gewebeabschnitt, der ersetzt werden muß, ähnlich ist. Nach einem ausreichenden Zeitraum in einem geeigneten Züchtungsmedium bei geeigneten Umweltbedingungen wachsen die geernteten Zellen auf der Matrix, um einen implantierbaren Gewebeabschnitt zu bilden, der im wesentlichen die gleiche physische Konfiguration aufweist, wie der Gewebeabschnitt, der beim Patienten ersetzt werden muß. Ein solches tissue-engineertes Konstrukt, das aus Zellen auf der Matrix (oder alternativ aus einer Matrix allein ohne Zellen besteht) besteht, wird dann mittels herkömmlich bekannter, chirurgischer Befestigungsmittel, einschließlich Nähte, Knochenhautabdeckungen oder Fibrinkleber, an der Knochenstelle befestigt.
- Die Vorteile von Tissue Engineering sind zahlreich, und kein geringer Vorteil ist beispielsweise, daß es nun möglich ist, einen Knorpel durch lebendes Knorpelgewebe zu ersetzen. Des Weiteren wird die Wahrscheinlichkeit der Abstoßung des Gewebeimplantats verringert, da das Knorpelgewebe, das invitro gezüchtet worden ist, mit dem autologen Knorpel des Patienten identisch ist.
- Obwohl bestehende Matrixbefestigungsvorrichtungen für ihre beabsichtigte Verwendung adäquat sind, gibt es auch einige Nachteile, die ihre Anwendung begleiten. Erstens sind diese Befestigungsvorrichtungen in dem Sinn generisch, daß sie nicht speziell für die Befestigung von Matrix am Knochen oder an weichem Gewebe ausgelegt sind, sondern für eine Vielzahl von chirurgischen Verfahren verwendet werden können. Andere Nachteile beinhalten die Schwierigkeit, viele dieser Vorrichtungen in einem minimal invasiven, arthroskopischen Verfahren zu verwenden. Weitere Nachteile beinhalten die Schwierigkeit und chirurgische Herausforderung, ein Stück Knochenhaut zur Verwendung als Knochenhautlappen zu ernten, die signifikante Patientenmorbidität, die mit einer solchen Ernte verbunden ist, sowie die Schwierigkeit, ein solches dünnes, nachgiebiges Material am umliegenden Gewebe anzunähen.
- Entsprechend gibt es in diesem Fachgebiet einen Bedarf nach neuen Befestigungsvorrichtungen, die eine Matrix tissue-engineerten Gewebes an einem Knochen oder an einer anderen Verankerungsstelle effektiv befestigen, so daß das Gewebe weiter im Körper des Patienten wachsen und sich neu bilden kann.
- Eine Befestigungsvorrichtung mit den Merkmalen des Oberbegriffs von Anspruch 1 ist in US-A-5643319 offenbart.
- Die Erfindung
- Daher ist es eine Aufgabe der Erfindung, eine Befestigungsvorrichtung bereitzustellen, die eine tissue-engineerte Matrix an einem Knochen oder an einer anderen Verankerungsstelle effektiv befestigt, wodurch ermöglicht wird, daß die implantierte Matrix an der Stelle bleibt, während das Gewebe weiter wächst und sich neu bildet.
- Es ist eine weitere Aufgabe der Erfindung eine solche Vorrichtung zur Befestigung einer Matrix an einer Knochenstelle bereitzustellen, die mittels eines arthroskopischen Verfahrens oder eines offenen Verfahrens einfach installiert wird.
- Es ist eine weitere Aufgabe der Erfindung eine solche Vorrichtung zur Befestigung einer Matrix an einer Knochenstelle bereitzustellen, die keine Nähte oder Nahtknoten erfordert.
- Entsprechend wird eine Transplantatbefestigungsvorrichtung gemäß Anspruch 1 bereitgestellt. Die Transplantatbefestigungsvorrichtung weist ein erstes und zweites Implantationsglied auf. Die Glieder sind länglich und weisen bevorzugt eine zylindrische Konfiguration auf. Die Glieder weisen auch distale Enden, proximale Enden und longitudinale Achsen auf. Es gibt longitudinale Durchgänge, die sich durch die ganze Länge eines jeden Implantationsglieds erstrecken. Die Glieder weisen äußere Oberflächen auf. Die Implanta tionsglieder sind durch ein Rohrglied mit einem ersten und einem zweiten Ende und einem mittleren Abschnitt miteinander verbunden. Das erste Ende des Rohrglieds erstreckt sich von dem proximalen Ende des ersten Implantationsglieds, und das zweite Ende des Rohrglieds erstreckt sich von dem proximalen Ende des zweiten Implantationsglieds. Das Rohrglied ist vorzugsweise relativ starr und kann konfiguriert sein, um eine geometrische Form aufzuweisen, zum Beispiel eine umgekehrte "U"-Form. Jedoch kann das Rohrglied auch biegsam sein. Das Rohrglied hält die Implantationsglieder in einem relativ festen Abstand von einander. Der mittlere Abschnitt des Rohrglieds ist ausgelegt, um einen Abschnitt eines tissue-engineerten Matriximplantats aufzunehmen. In einer bevorzugten Ausführungsform weisen die Implantationsglieder eine Folge von sich von den äußeren Oberflächen der Implantationsglieder erstreckenden Erhöhungen auf, um bei der Verhinderung des Abgehens von einer Knochenstelle oder einer anderen Verankerungsstelle mitzuwirken, nachdem die Implantationsglieder in vorher erzeugte Bohrlöcher implantiert worden sind.
- Hierin wird auch ein Verfahren zur Verwendung der Transplantatbefestigungs-vorrichtung der Erfindung offenbart, um eine tissue engineertes Gewebe enthaltende Matrix an einem Knochen zu befestigen. Das Verfahren ist nicht Teil der Erfindung.
- Beschreibung bevorzugter Ausführungsbeispiele
- Diese und weitere Merkmale und Vorteile der vorliegenden Erfindung werden anhand der folgenden Beschreibung bevorzugter Ausführungsformen in Zusammen mit der beigefügten Zeichnung offensichtlicher. Hierbei zeigen:

12 eine Ansicht einer tissue-engineerten Matrix, die mit mehreren Transplantatbefestigungsvorrichtungen der vorliegenden Erfindung an einem Knochen befestigt ist. - Die Transplantatbefestigungsvorrichtungen der Erfindung können aus herkömmlichen biokompatiblen Materialien, einschließlich absorbierbaren und nicht absorbierbaren Materialien, sowie biologisch abbaubaren Materialien hergestellt werden. Die nicht absorbierbaren Materialien, die verwendet werden können, beinhalten herkömmliche biokompatible Materialien, wie beispielsweise rostfreier Stahl, Polyethylen, Teflon, Nitinol, nicht absorbierbare Polymere, andere biokompatible Metalle, Keramiken, Kombinationen davon und ähnliche. Die absorbierbaren Materialien, die verwendet werden können, um die Transplantatbefestigungsvorrichtungen der vorliegenden Erfindung herzustellen, beinhalten typischerweise jene in diesem Fachgebiet bekannten herkömmlichen bioabsorbierbaren oder bioresorbierbaren Materialien, die effektiv geformt oder bearbeitet werden können. Die bioabsorbierbaren und bioresorbierbaren Materialien beinhalten Polymilchsäure, Polydioxanon, Polycaprolacton, Polyglycolsäure, Polygalactsäure, andere bekannte biokompatible, bioabsorbierbare und bioresorbierbare Polymere, Keramiken, Komposite, Kombinationen davon und ähnliche sowie Äquivalente davon.
- Nun auf
1 und2 Bezug nehmend, wird eine bevorzugte Ausführungsform einer Transplantatbefestigungsvorrichtung10 der vorliegenden Erfindung dargestellt. Es wird gesehen, daß die Transplantatbefestigungsvorrichtung10 Implantationsglieder20 aufweist. Es wird gesehen, daß die Implantationsglieder20 längliche Glieder sind, die vorzugsweise eine im wesentlichen zylindrische Form aufweisen. Die Glieder20 können andere geometrische Formen aufweisen, einschließlich konisch, pyramidenförmig, mehreckig, würfelförmig, kugelförmig usw. Es wird gesehen, daß die Implantationsglieder20 distale Enden22 und proximale Enden24 aufweisen. Es wird gesehen, daß jedes Implantationsglied20 eine äußerliche Oberfläche28 und eine longitudinale Achse29 aufweist. Ferner wird gesehen, daß jedes Glied20 einen longitudinalen Durchgang35 aufweist, der sich hierdurch erstreckt. Ferner wird gesehen, daß die Implantationsglieder20 optionale stumpfkegelige Enden30 und proximale Endseitenoberflächen32 aufweisen. Obwohl bevorzugt wird, daß Endseitenoberflächen32 flach sind, kann Endseitenoberfläche32 auch abgewinkelt, konkav, konvex usw. sein. Es wird gesehen, daß Endseitenoberfläche32 eine mittlere kreisförmige Öffnung36 in Verbindung mit Durchgang35 aufweist. Vorzugsweise weist die mittlere Öffnung36 einen kreisförmigen Querschnitt auf, aber sie kann auch andere geometrische Querschnitte aufweisen, einschließlich elliptisch, mehreckig, quadratisch, rechteckig, Kombinationen davon und ähnliche. Ferner wird gesehen, daß Glieder20 distale Endseitenoberflächen mit kreisförmigen Öffnungen in Verbindung mit Durchgängen35 aufweisen. Wie bei dem optionalen stumpfkegeligen Ende30 gezeigt, weist die ringförmige Endseitenoberfläche eine De-Minimis-Dicke um die Öffnung herum auf, jedoch würde diese Dicke in Abwesenheit eines stumpfkegeligen Endes zunehmen. Ferner wird gesehen, daß sich von der Oberfläche28 des Glieds20 eine Folge von optionalen Vorsprüngen40 mit Gewebeeingriffskanten44 erstreckt. Ohne die Vorsprünge40 ist die Oberfläche28 des Glieds20 glatt. - Es wird gesehen, daß die Vorrichtung
10 ein Transplantatretentionsglied50 , das die Implantationsglieder20 verbindet, aufweist. Es wird gesehen, daß Retentionsglied50 ein rohrähnliches Glied mit einem ersten Ende52 , einem zweiten Ende54 und einem mittleren Abschnitt55 ist. Es wird gesehen, daß sich das erste Ende52 von der proximalen Endseitenoberfläche32 des ersten Glieds20 erstreckt, während gesehen wird, daß sich Ende54 von der proximalen Endseitenoberfläche32 des anderen Glieds20 nach oben erstreckt. Falls gewünscht, können sich die Enden54 und52 von Retentionsglied50 auch von irgendeinem Abschnitt der äußeren Oberfläche28 erstrecken oder daran angebracht sein. Es wird gesehen, daß das Verbindungsglied50 vorzugsweise in drei Segmente gebogen oder geformt ist, einschließlich oberes Segment55 und Beinsegmente56 . Es wird gesehen, daß das obere Segment 55 im wesentlichen senkrecht zu den Beinsegmenten56 ist. Obwohl bevorzugt wird, daß Verbindungsglied50 eine umgekehrte "U"-Konfiguration aufweist, kann das Verbindungsglied50 auch andere geometrische Konfigurationen aufweisen, einschließlich halbkreisförmig, gewölbt, bogenförmig, dreieckig, mehreckig, U-förmig und ähnliche sowie Kombinationen davon. Die Enden52 und54 des Verbindungsglieds50 können permanent an den Implantationsgliedern20 befestigt werden oder können auf herkömmliche Weise entfernbar hieran angebracht werden. Glied50 kann starr oder biegsam sein. Glied50 weist einen ausreichenden Oberflächenbereich auf, um eine tissue-engineerte Matrix effektiv auf einem Knochen oder einer anderen Körperoberfläche festzuhalten. Vorzugsweise weist Verbindungsglied50 einen kreisförmigen Querschnitt auf, kann aber auch andere geometrische Querschnitte aufweisen, einschließlich elliptisch, mehreckig, quadratisch, rechteckig, Kombinationen davon und ähnliche. Glied50 kann starr oder biegsam sein und kann eine einzige faserartige Struktur aufweisen oder mehrere verbundene Fasern oder Glieder aufweisen. - Nun auf
3 –8 Bezug nehmend, wird die Verwendung der Transplantatbefestigungsvorrichtungen10 der vorliegenden Erfindung in einem chirurgischen Verfahren dargestellt. Zuerst auf3 Bezug nehmend, ist der erste Schritt vor Installation einer tissue-engineertes Gewebe enthaltenden Matrix mittels einer Transplantatbefestigungsvorrichtung10 der vorliegenden Erfindung, zwei Bohrlöcher200 in einen Knochen210 , zum Beispiel der Subchondralknochen im Kniegelenk, zu bohren oder zu "klopfen". Es wird gesehen, daß die Bohrlöcher200 zylindrische Löcher mit einem Boden208 und einer offenen Oberseite202 sowie Seitenwänden205 sind. Optional können die Bohrlöcher Knochentunnel mit einem kontinuierlichen Durchgang und keinem Boden oder einem offenen Boden sein. Es wird insbesondere bevorzugt, die Löcher in den Knochen mittels eines Instruments400 , wie in10 dargestellt, zu klopfen, welches einen proximalen Abschnitt aufweist, der herkömmlich in diesem Fachgebiet als "Slap-Hammer"-Abschnitt bezeichnet wird. Der Begriff "Klopfen", wie hier verwendet, ist definiert ein Verfahren zu bedeuten, bei dem die distalen, spitz zulaufenden Spitzen420 , die sich von dem distalen Ende415 des Schafts405 des Instruments400 erstrecken, über einer Knochenstelle angeordnet sind, und das proximale Ende410 des Instruments400 mit einem gleitbaren Hammergriff (des "Slap-Hammers") geklopft oder geschlagen wird, der auf dem Schaft zwischen dem proximalen Ende410 und einem proximalen Anschlag gleitet, um die Knochenbohrlöcher200 zu bilden. Das distale Ende des Schafts ist mit einem proximalen Ende verbunden. Der proximale Anschlag ist an dem proximalen Ende angebracht. Es wird gesehen, daß der Hammergriff einen Greifabschnitt, Manschetten und einen longitudinalen Durchgang aufweist. Fachleute werden anerkennen, daß ein ähnliches zugespitztes Instrument verwendet werden kann, um die Bohrlöcher in einen Knochen hinein zu "klopfen", das heißt jedes Instrument mit einem nagelähnlichen, distalen Ende. Des Weiteren, obwohl nicht bevorzugt, kann ein Knochenbohrloch nach dem anderen hinein "geklopft" werden. Wenn der Chirurg entscheidet, die Bohrlöcher in einen Knochen zu bohren, kann jede herkömmliche chirurgische Bohrvorrichtung verwendet werden. Nachdem die Bohrlöcher200 im Knochen210 gebildet worden sind, wird die tissue-engineertes Gewebe enthaltende Matrix220 vom Chirurgen, wie in4 gesehen, auf der Knochenoberfläche201 plaziert. Als nächstes wird die Transplantat-befestigungsvorrichtung10 auf dem Einführungsinstrument250 angebracht. Es wird gesehen, daß Einführungsinstrument250 , wie in11 dargestellt, ein längliches Rohr260 mit einem proximalen Ende262 und einem distalen Ende264 ist. An dem distalen Ende264 des Rohrs260 ist der Tiefenanschlag angebracht. Es wird gesehen, daß der Tiefenanschlag290 ein im wesentlichen rechteckiges Glied ist, das senkrecht zu der longitudinalen Achse251 des Rohrs260 angebracht ist. Es wird gesehen, daß Tiefenanschlag290 einen Boden aufweist. Vom Boden292 des Plattenglieds290 erstreckt sich distal ein Paar paralleler, von einander beabstandeter Anbringspitzen270 . Es wird gesehen, daß die Anbringspitzen270 im wesentlichen rohrähnliche Glieder mit distalen, spitz zulaufenden Spitzen277 und proximalen Enden272 sind. Es wird gesehen, daß die Spitzen270 einen ersten Abschnitt und einen distalen Abschnitt aufweisen. Es wird gesehen, daß der Abschnitt eine größere Querschnittsabmessung aufweist als der distale Abschnitt, so daß der ganze Abschnitt275 in die Durchgänge35 der Glieder20 einführbar ist, während der proximale Abschnitt hierin nicht einführbar ist. Ferner wird gesehen, daß Instrument250 einen "Slap-Hammer-Abschnitt" aufweist, der aus einem sich von dem proximalen Ende262 erstreckenden proximalen Schaft und einem gleitbaren Hammergriff (der "Slap-Hammer") besteht, der auf dem Schaft zwischen dem proximalen Ende262 und dem proximalen Anschlag gleitbar ist. Es wird gesehen, daß das Hammergriffglied einen Greifabschnitt, Endmanschetten und einen longitudinalen Durchgang aufweist. Die Transplantatbefestigungsvorrichtung10 wird an dem Einführungsinstrument250 angebracht, indem die Implantationsglieder20 derart auf die Spitzen270 geschoben werden, daß die distalen Abschnitte der Glieder270 in den longitudinalen Durchgängen35 der Glieder20 aufgenommen sind, und distale Punkte277 über das Ende der distalen Endseitenoberflächen37 hinausragen. Dann, wie in5 und6 gesehen, wird das Instrument250 derart gehandhabt, daß die Transplantatbefestigungsvorrichtung10 durch Matrix220 und in den Knochen210 eingeführt wird, indem die auf den Spitzen270 angebrachten Implantationsglieder20 derart in die Bohrlöcher200 bewegt werden, daß die Glieder20 in den Bohrlöchern200 aufgenommen sind und derart, daß der Gewebe aufnehmende Abschnitt55 des Retentionsglieds50 die Matrix220 derart aufnimmt, daß die Matrix220 fest mit der Oberfläche201 des Knochens210 in Eingriff ist. Falls gewünscht, können vor Einführung der Vorrichtung10 Löcher in die Matrix220 geschnitten werden. Dann wird das Einführungsinstrument250 , wie in7 gesehen, proximal zurückgezogen, was dazu führt, daß die Spitzen270 aus den Durchgängen35 der Implantationsglieder20 zurückgezogen werden, wodurch die Transplantatbefestigungsvorrichtung10 in den Knochenbohrlöchern aufgenommen zurückgelassen wird und was dazu führt, daß die Matrix220 in Eingriff mit der Oberfläche201 des Knochens210 bleibt. Der "Slap-Hammer" Abschnitt des Instruments250 kann bei der Entfernung der Spitzen mitwirken. Eine Querschnittsansicht, welche die Vorrichtung10 im Knochen210 aufgenommen darstellt, während sie die Matrix220 auf der Knochenoberfläche201 hält, wird in8 gesehen. -
12 stellt eine Matrix220 dar, die an der Knochenoberfläche201 des Knochens210 mit mehreren Befestigungsvorrichtungen der Erfindung angebracht ist, die installiert sind, um die Matrix220 zu befestigen. Die Anzahl, anatomische Stelle und Ausrichtung der Befestigungsvorrichtungen10 , die notwendig sind, um eine ausreichend wirksame Befestigung bereitzustellen, variiert mit der Größe und Art des Implantats oder der Matrix, dem Gewebetyp, dem Alter des Patienten, der Größe des Patientendefekts, der Größe der Befestigungsvorrichtungen, dem Konstruktionsmaterial der Befestigungsvorrichtungen, der Belastung auf dem Gewebe an der Reparaturstelle usw. - Fachleute werden anerkennen, daß die Größe der Befestigungsvorrichtungen der Erfindung gemäß einer Anzahl von Variablen variiert, einschließlich der speziellen Ausführung der Vorrichtung, der Konstruktionsmaterialien, der speziellen Anwendung der Vorrichtungen, der Art des chirurgischen Verfahrens usw. Ebenso variiert die Größe der Matrizen, die mit diesen Vorrichtungen befestigt werden, ähnlich. Die Figuren, die Teil dieser Patentschrift sind, sind rein schematisch und veranschaulichen die Vorrichtung und das Verfahren der vorliegenden Erfindung; die tatsächlichen Abmessungen der Vorrichtungen und Matrizen können in der Praxis variieren.
- Das folgende Beispiel veranschaulicht die Prinzipien und Anwendung der vorliegenden Erfindung, ist aber nicht hierauf beschränkt.
- Beispiel
- Sechs Schafe wurden für ein chirurgisches Verfahren mittels standardmäßiger, aspetischer, chirurgischer Methoden, einschließlich der Verwendung voll sterilisierter Instrumente und Vorrichtungen, sowie herkömmlicher Anästhesieverfahren und -protokolle, vorbereitet. Der Chirurg erzeugte dann Knorpeldefekte mit einem Durchmesser von 7 mm (Knorpel von voller Stärke) auf einem Gewicht tragenden Bereich des medialen Femurkondylus und in dem Sulcus trochlearis in dem rechten Knie bei jedem der sechs ausgewachsenen Schafe. Die Defekte wurden mittels eines Spezialbohrers mit Tiefenanschlag erzeugt, um eine Subchondralknochenexposition oder penetration zu verhindern. Der Untergrund sämtlicher Defekte wurde dann mit einem Spezialmikrohackwerkzeug mikrofrakturiert, um einen Eintritt für Zellenwanderung bereitzustellen. Die Versuchstiere wurden dann in drei Gruppen von jeweils zwei Versuchstieren eingeteilt:
- Gruppe 1: Defekt gefüllt mit einer Kollagenmatrix, die mit der Transplantatbefestigungsvorrichtung der vorliegenden Erfindung befestigt wurde.
- Gruppe 2: Defekt gefüllt mit einer Kollagenmatrix, die mit einer 9–0 absorbierbaren VicrylTM-Naht (unterbrochene Nahtstichtechnik, ungefähr 12 Fäden pro Matrix) befestigt wurde.
- Gruppe 3: nicht gefüllter Defekt (Kontrollgruppe).
- Beide Defekte in einem gegebenen Knie erhielten die gleiche Behandlung oder dienten als Kontrollen.
- Bei den zwei Schafen in Gruppe 1 wurde, nachdem ein Defekt erzeugt und mikrofrakturiert worden war, ein Stanzwerkzeug
400 verwendet, um die zwei erforderlichen Bohrlöcher in dem Subchondralknochen zu erzeugen, um eine Transplantatbefestigungsvorrichtung der vorliegenden Erfindung aufzunehmen. Nur eine Polydioxanon-Vorrichtung (4 mm Abstand von Spitze zu Spitze) wurde verwendet, um jede Matrix anzubringen. Um die Bohrlöcher zu erzeugen, wurde das Stanzwerkzeug im Defekt zentriert, in der Sagittalebene ausgerichtet, und wiederholt mit einem Slap-Hammer getroffen oder "geklopft", bis es mehrere Millimeter in den Subchondralknochen eindrang. Als nächstes wurde eine kreisförmige Kollagenmatrix mit einem Durchmesser von 7 mm, die mit physiologischer Kochsalzlösung durchtränkt wurde, in dem Defekt plaziert und dann trocken getupft, um überschüssige Kochsalzlösung zu entfernen. Als das Einführungswerkzeug250 mit der Transplantatbefestigungsvorrichtung10 der vorliegenden Erfindung beladen wurde, wurden die Vorrichtung und das Einführungswerkzeug über der Matrix zentriert und in der Sagittalebene ausgerichtet. Der Chirurg lokalisierte dann die vorher erzeugten Bohrlöcher, indem die distalen Spitzen des Einführungswerkzeugs langsam durch die Matrix vorwärts bewegt wurden. Sobald der Chirurg die Löcher mit den Einführungswerkzeugspitzen lokalisierte, wurde ein Hammer verwendet, um das Einführungswerkzeug (und die Implantationsglieder20 der Befestigungsvorrichtung10 ) vollständig durch die Matrix und in den Subchondralknochen vorwärts zu bewegen. Das Einführungswerkzeug wies einen Tiefenanschlag auf, um zu verhindern, daß die Implantationsglieder20 zu tief eingeführt werden, wodurch die ordnungsgemäße Plazierung der Implantationsglieder durch die Matrix sichergestellt wurde. Die Einführung war abgeschlossen, als das Verbindungsretentionsglied zwischen den zwei Implantationsgliedern anfänglich begann, die Kollagenmatrix zusammenzudrücken, wodurch die sichere Befestigung mit dem darunter liegenden Subchondralknochen angezeigt wurde. Nachdem die zwei Defekte in einem gegebenen Knie mit einer Matrix und einer Befestigungsvorrichtung repariert worden waren, wurde das Knie geschlossen und dem Schaf erlaubt, sich zu erholen. Es wurde von dem Chirurgen bemerkt, daß es ungefähr eine Minute dauerte, um eine Matrix mit einer Befestigungsvorrichtung der vorliegenden Erfindung (Gruppe 1) anzubringen im Vergleich zu den ungefähr 15 Minuten, um eine Matrix mit einer Naht allein und der erforderlichen Nahthandhabung und dem erforderlichen Knoten (Gruppe 2) anzubringen. - Zwei Wochen, nachdem die chirurgischen Eingriffe abgeschlossen worden waren, wurden die Kniegelenke chirurgisch zur Untersuchung geöffnet. Eine grobe makroskopische Beurteilung der Gelenke zeigte, daß alle vier Matrizen, die von der Transplantatbefestigungsvorrichtung der vorliegenden Erfindung gehalten wurden, völlig intakt waren. Jedoch waren alle vier Matrizen, die von Nähten alleine gehalten wurden, nur teilweise intakt, wobei im Durchschnitt ungefähr 30 % der Nähte auf jeder gegebenen Matrix gebrochen waren.
- Eine weitere Ausführungsform der Befestigungsvorrichtung der vorliegenden Erfindung, die mehrere Retentionsglieder aufweist, wird in
9 gesehen. Es wird gesehen, daß die Vorrichtung300 ein Paar Implantationsglieder310 aufweist. Die Implantationsglieder310 sind im wesentlichen zylindrische Glieder mit einer longitudinalen Achse311 , distale Enden314 und proximale Enden312 . Es wird gesehen, daß jedes Implantationsglied310 einen longitudinalen Durchgang320 aufweist. Es wird gesehen, daß die Glieder310 ein distales stumpfkegeliges Ende330 , eine äußere Oberfläche350 und Erhöhungen355 aufweisen, die sich von der Oberfläche350 erstrecken. Es wird gesehen, daß die Glieder310 durch ein Paar Retentionsglieder340 mit einem ersten und einem zweiten Ende342 bzw.344 verbunden sind. - Die Befestigungsvorrichtungen
10 und das Verfahren der Verwendung solcher Vorrichtungen der vorliegenden Erfindung weisen viele Vorteile auf. Die Vorteile beinhalten die Bereitstellung einer schnellen und routinenmäßigen Methode, um eine Matrix tissue-engineerten Gewebes oder eines anderen Gewebes zu befestigen. Die Befestigungsvorrichtungen10 können, da sie den Bedarf nach Nahtknoten beseitigen, in chirurgischen arthroskopischen Verfahren verwendet werden, die ein Minimum an chirurgischer Inzision erfordern und somit die Patientenmorbidität stark reduzieren. Zusätzlich ist bewiesen worden, daß die Befestigungsvorrichtungen10 eine ausgezeichnete Matrixbefestigung bereitstellen, ohne das umliegende, normale Knorpelgewebe zu beschädigen, im Gegensatz zur herkömmlichen Befestigung von Knorpeldefektmatrizen mit traditioneller Naht, die durch das umliegende Gewebe hindurch gehen muß (und somit beschädigt). - Obwohl die Erfindung in Bezug auf detaillierte Ausführungsformen gezeigt und beschrieben worden ist, werden Fachleute anerkennen, daß verschiedene Form- und Detailänderungen gemacht werden können, ohne vom Umfang der beanspruchten Erfindung abzuweichen.
Claims (8)
- Transplantatbefestigungsvorrichtung (
10 ) mit: – einem ersten Implantationsglied (20 ) mit einer longitudinalen Achse (29 ), einem proximalen Ende (24 ), einem distalen Ende (22 ), einer äußeren Oberfläche (28 ) und einem longitudinalen Durchgang (35 ) hierdurch; – einem zweiten Implantationsglied (20 ) mit einer longitudinalen Achse (29 ), einem proximalen Ende (24 ), einem distalen Ende (22 ), einer äußeren Oberfläche (28 ) und einem longitudinalen Durchgang (35 ) hierdurch; – proximale Endseitenoberflächen (32 ) auf den proximalen Enden (24 ) des ersten und des zweiten Implantationsglieds (20 ), die die longitudinalen Durchgänge (35 ) umgeben; und – einem das erste und das zweite Implantationsglied (20 ) verbindenden Verbindungsglied (50 ) mit einem mittleren Abschnitt (55 ), einem sich vom ersten Implantationsglied (20 ) erstreckenden ersten Ende (52 ) und einem sich vom zweiten Implantationsglied (20 ) erstreckenden zweiten Ende (54 ); dadurch gekennzeichnet, daß das Verbindungsglied (50 ) ein Rohrglied ist, welches zu einer Konfiguration mit einem mittleren Abschnitt (55 ) und Beinsegmenten (56 ) geformt ist, wobei die Beinsegmente (56 ) im wesentlichen parallel zu den longitudinalen Achsen (29 ) der Implantationsglieder (20 ) sind und der mittlere Abschnitt (55 ) im wesentlichen senkrecht zu den Beinsegmenten (56 ) ist. - Transplantatbefestigungsvorrichtung nach Anspruch 1, dadurch gekennzeichnet, daß die Implantationsglieder eine Folge sich von deren äußeren Oberflächen erstreckender Erhöhungen aufweisen.
- Transplantatbefestigungsvorrichtung nach Anspruch 1 oder 2, gekennzeichnet durch ein stumpfkegeliges Ende, welches sich von dem distalen Ende (
22 ) des ersten Implantationsglieds (20 ) und dem distalen Ende (22 ) des zweiten Implantationsglieds (20 ) erstreckt. - Transplantatbefestigungsvorrichtung nach einem der vorangehenden Ansprüche, dadurch gekennzeichnet, daß die Implantationsglieder (
20 ) eine zylindrische Konfiguration aufweisen. - Kombination der Transplantatbefestigungsvorrichtung (
10 ) nach einem der vorangehenden Ansprüche und einem Einführungsgerät (400 ), welches ein Paar voneinander beabstandet angeordneter Spitzen (420 ) mit distalen Enden aufweist, so daß die Spitzen (420 ) in den Durchgängen (35 ) der Implantationsglieder (20 ) der Transplantatbefestigungsvorrichtung nach einem der vorangehenden Ansprüche so aufnehmbar sind, daß die distalen Enden der Spitzen sich über die distalen Enden (22 ) der Implantationsglieder (20 ) hinaus erstrecken. - Kombination nach Anspruch 5, dadurch gekennzeichnet, daß jede Spitze (
420 ) einen sich vom distalen Ende erstreckenden Punkt umfaßt. - Kombination nach Anspruch 5 oder Anspruch 6, dadurch gekennzeichnet, daß jede Spitze (
420 ) einen distalen Abschnitt mit einem Querschnitt und einen proximalen Abschnitt mit einem Querschnitt umfaßt, wobei die Fläche des Querschnitts des proximalen Abschnitts größer ist, als die Fläche des Querschnitts des distalen Abschnitts. - Kombination nach Anspruch 5, Anspruch 6 oder Anspruch 7, dadurch gekennzeichnet, daß jede Spitze (
420 ) einen kreisförmigen Querschnitt aufweist.
