Abstract

There is described a punching or cutting biopsy cannula with a means for coagulation at the distal end. An electrical connection can be effected on the cannula which is insulated or electrically conducting on the distal end only, or else on a special obturator which can be pushed into the cannula. The obturator is electrically insulted from the cannula and is electrically conducting on its distal end only. Cannula and obturator have a handle.

Description

Stanz- bzw. Punching and cutting biopsy cannulas are used in medicine for obtaining samples from various organs. You are there characterized in that they mostly consist of two parts, first, the actual metal cannula and one made of art fabric existing handle. An obturator can be inserted into the cannula be introduced, the distal, d. H. far end one Has a T-shaped recess in the cannula end. The summit itself is ground to make it easier for the cannula to penetrate the mesh. The ground cutting device distally has T- Form, whereby the tissue penetrates into the cavity of the obturator and then abge with the cutting cannula that can be pushed over it is cut and remains inside the cannula.

A similar principle is used by the cutting biopsy cannula, the un a rotating cylinder out of the tissue can cut, which then be inside the cannula finds. The punch biopsy cannula (e.g. Tru-Cut ®) can be split into two ver different functions are used:

The cannula and obturator are inserted into the tissue to be biopsied inserted. The cannula is then withdrawn so that only the T-shaped end with the tip in it tissue to be biopsied. Then the in the end, a sharp cannula is pushed over the obturator and thereby cut out a cylindrical piece of tissue that now located inside the cannula. By pulling out the obturator the piece of tissue can also be removed. Then the entire cannula removed from the body.
    2. The punch biopsy cannula is pushed together with the optorator directly in front of the tissue to be biopsied. The cutting tip is then used to push the obturator out of the cannula. The ground cannula tip can then again be pushed over the obturator, whereby the tissue cylinder is cut out and inserted into the cannula. All parts of the punching or cutting biopsy cannula can be sterilized. The disadvantages of the previously known designs are:
    • Since both the obturator and the cannula are electrically conductive, there is no possibility of electrical coagulation after the biopsy. Biopsies often lead to bleeding from the wound bed, depending on the degree of blood flow to the tissue to be biopsied (e.g. liver or prostate).
    • The cannula is then removed from the body, the bleeding usually coming to a standstill by folding the layers of tissue together. However, if larger vessels are hit, or if the patient has an additional coagulation disorder, there may be a larger bleeding in the tissue and the possibility of surgical exposure.

Allgemeines Ziel der vorliegenden Erfindung ist es, eine Schneid- oder Stanzbiopsiekanüle zu schaffen, dieThe general aim of the present invention is to provide a cutting or to create a punch biopsy cannula that

The previous properties of the punching or suction biopsy can len maintains, d. H. from a plastic handle as well a sharp cannula end for cutting out tissue zy alleviate is created.
The possibility of coagulation in the cavity after the cutting has been created.

Die erfindungsgemäß ausgebildete Stanz- oder Schneidbiopsiekanüle besteht aus zwei Teilen:The punching or cutting biopsy cannula designed according to the invention consists of two parts:

A cannula that has a plastic handle. The canoe le is sharpened diagonally sharply at the far end of the handle.
    • a) Die Kanüle selbst ist elektrisch isoliert (z.
    2. An obturator, which has a plastic part on the handle, which fits into the handle of the cannula and has an axial sliding with locking option, as well as a handle-remote end, which has a ground point for penetrating the tissue, and shortly after Tip has a T-shaped cut out recess for receiving the punch cylinder. There are now basically two options for the electrical connection option for coagulation:
    • a) The cannula itself is electrically insulated (e.g. covered with Teflon) and only has a conductive piece at the end of the cannula that is far from the handle. The piece corresponds exactly to the length of the punch cylinder to be cut out later. In the vicinity of the handle there is a recess in the cannula to take on an electrical connection, the z. B. can be done by a crocodile clip. The cannula can be isolated from the tissue by simply covering it with an electrically non-conductive material (e.g. Teflon). The area must be conductive again in the area of the connection point for the electrical cable.
    • b) Through the non-insulated cannula, a special obturator is inserted with a handle like the cutting obturator. The obturator itself is isolated from the cannula, but only the sharpened end is electrically conductive from the length of the cylinder cut out. At this obturator an electrical shear connection can also z. B. conditions with a crocodile clip.

For coagulation, the patient still needs to be grounded with a Metal plate, e.g. B. on the back or on the thigh.

The invention is described below using an example with a drawing explained:

Fig. 1 shows the first possibility of a coagulation punch biopsy cannula. The obturator is in the cannula. The cannula end of the length of the cylinder to be cut out is electrically conductive, the cannula itself is insulated.

Fig. 2 zeigt die Stanzbiopsiekanüle mit vorgeschobenem Obturator. Fig. 2 shows the punch biopsy cannula with the obturator advanced. The tissue now pushes into the T-shaped ground end of the obturator and is cut out by pushing the cannula in the form of a punch cylinder.

Fig. 3 shows the cannula with the electrically conductive Ka nulenende and the electrical connection. The end of the cannula is now in the cavity of the punched cylinder. Coagulation can take place in this area.

FIGS. 4 and 5 show a further possibility in principle a Koagulationsstanzbiopsiekanüle. As before, the cannula consists of electrically conductive material with a plastic handle. After the biopsy procedure, a special obturator, which has a round cross section with a ground tip, is inserted into the cannula. The surface of the obturator is not electrically conductive and is therefore insulated from the cannula. The obturator is only electrically conductive at its di stal end in the length of the cylinder to be cut out. An electrical connection can be made at the end near the handle. After completing the biopsy, the obturator is now inserted into the cannula, the cannula is pulled back to the stop, ie until the electrically conductive part of the obturator is fully exposed, and then coagulation via the obturator. The cannula and obturator can then be removed together.

Figs. 1 to 5 show basic ways of Koa gulationsmöglichkeit with a punch biopsy cannula. The same can be done on the cutting biopsy cannula, which is basically similar in construction. Other forms and possibilities of the electrical connection or the special insulation of the obturator against the cannula or the cannula against the tissue are in principle conceivable without exceeding the scope of the invention. The punching or suction biopsy cannula can be used for all types of biopsy and offers the additional possibility of coagulation at the biopsy site.
