Description

This invention relates to an electrosurgical device and in particular an electrosurgical tool for percutaneous removal of prostate tissue or other Tissue and a method for removing and removing Tissue from high frequency electrical energy.

Hintergrund der ErfindungBackground of the Invention

Benign prostatic hyperplasia (BPH) is one benign enlargement of the prostate gland that occurs in a decrease in urine flow through the urethra affects. Typically, BPH affects men in the Age over 45 and accelerates with that Age. BPH can cause symptoms of occlusion and irritation, their severity from a slight decrease in urine flow to for acute urinary retention is sufficient.

A common surgical procedure at BPH is Resection of the prostate (TURP) through the urethra, and this procedure removes parts of the prostate through the Urethra using a transurethral Resectoscopes and electrocautery. TURPs can be not always and can not perform without difficulty a longer hospital stay, blood loss that sometimes requires transfusions, Bring impotence and urinary incontinence. In addition absorption of the rinse fluid must be taken into account (TUR syndrome), which is a sharp increase in Heart strain.

There are various tools for implementation surgical cuts in body canals, e.g. B. for TURPs, known in the art. U.S. Patent 5,415,656 describes an electrosurgical device for the opening  or widening of a constriction within or in the Near a body channel. This device has one electrically conductive, deflectable wire on the with an electrical high frequency power source is connected. The disclosure of this patent does not contain any device which is the insertion of the instrument through the skin, d. H. a percutaneous application, make possible or would suggest. On the content of this patent hereby referred.

Patent 3,910,279 describes one electrosurgical instrument for resection of Body tissue. This instrument is for insertion through an existing body channel and into a body cavity designed. In contrast, this device is not for the suitable for percutaneous introduction into the tissue. The Description also does not deal with the need to aspirate fluid or irrigation fluid from this instrument feed. U.S. Patent 3,910,279 is also hereby incorporated by reference Referred.

Various attempts have been made to offer new therapies develop an alternative to TURP for the BPH enable. These therapies have become common less invasive options directed such. B. the Resection through the urethra, although others Resection procedures are known. The latter contain the Application of laser light, stents, the lowest temperature therapy, therapies with ultrasound or microwaves, with high frequency electrical current, the balloon dilatation, incisions through the urethra and drug therapies. Not all of this treatment methods have proven to be effective, and the ideal Treatment in a patient is not necessary the same for every patient.

The removal treatments offer advantages over TURPs, by reducing blood loss, the development of the  Avoid TUR syndrome and the occurrence of retrograde Reduce ejaculation by reducing the bladder neck and the Avoid the urethra completely. Another advantage of these ablative treatment methods is that the Patients usually follow their normal lifestyle can resume much faster than patients who have been subjected to a TURP.

Ablation therapies have advantages over amputations operations through the urethra since there are none Tissue scabbing through the urethra after surgery gives. The removal therapies also offer advantages versus standard hyperthermia procedures because it is has shown that with time in the prostate a noticeable lesion is created.

The decline in the current ablation amputation proceed as with all the non-invasive The procedure is that during the Surgery do not take tissue samples for pathology to let. Is even more critical with all the known ones slightly invasive therapies that release the occlusion not immediately and that often after surgical intervention a catheter must be inserted. Thus, there is no known device that one immediate postoperative recovery from BPH occlusion reached, tissue for pathological analysis Provides, and no post-operative catheter needs.

Kurzfassung der ErfindungSummary of the invention

A tissue ablation resection instrument like this described below can be used for operations on the Prostate can be used. This ablative resection instrument uses a wire loop for cutting and Cauterize tissue, which is then suctioned off opening is suctioned off. This device has advantages  in terms of reducing bleeding and likelihood TUR syndrome, at the same time achieves one fast recovery of the lock and supplies pathological specimens as from a TURP operation.

This invention is based on the idea that the fabric destroyed and immediately removed from the prostate and that there is only a minor violation of the the urethra surrounding the prostate arises. An advantage this product is also that it is the most beneficial Characteristics of the TURPs (these are the most immediate Lock retrieval and tissue samples) with removal and interstitial tissue resection (i.e. reduced bleeding and no injury to the Bladder neck and urethra and a quick Relaxation) combined in one device.

The resection instrument for removing tissue in Spaces can be designed as a disposable instrument and consist of a rigid plastic handle that with a semi-flexible shaft is connected, the one has sharp point. At the distal section of the shaft one or more cutting links can be varied stand out. These links can be electro surgical current or electrocautery current be fed. A suction / rinsing channel can be inside the Shaft may be provided to inject or fluid Remove tissue from the surgical site. For the surgeon the removal of tissue is important to drove the samples to get pathological examination.

The procedure for using the resection instrument to remove tissue in between spaces Ultrasound observation through the rectum or a Require observation through the urethra. As soon as that Resection instrument for removing tissue in Gaps may have been introduced into the tissue the active element at the distal end of the shaft  be moved outwards. After feeding in The resection cutting edge can be turned, and thereby cutting off wedge-shaped tissue parts, which then be sucked through the suction channel. The active link can be expanded bit by bit and at progressing surgery, ever larger lumps of tissue remove. The use of electrosurgical energy causes the remaining tissue to coagulate. This will encapsulate the surgical wound bleeding is restricted by itself.

A preferred one is detailed below Embodiment of the invention with reference to the drawing described.

As shown in FIG. 1, this invention is an electrosurgical device 10 for resection of tissue within a body. The device 10 is designed for percutaneous introduction into the body and can be used for operations on the prostate gland. This device is particularly suitable for the treatment of benign prostatic hyperplasia (BPH).

The device 10 has an elongated hollow shaft 11 with a proximal end 12 and a distal end 13 . The proximal end 12 of the shaft 11 is connected with a handle 14 to control elements for the surgeon. The distal end 13 of the shaft 11 is designed for contact with the tissue of a patient. The shaft 11 has a hollow interior 16 with a diameter that allows the passage of ablated tissue from the distal end 13 to the proximal end 12 . In one embodiment, the shaft 11 is a needle in the range 18 to 14 gauge (catheter scale).

There is a sharp tip 17 at the distal end 13 of the shaft 11 . The sharp tip 17 is used for percutaneous insertion of the device 10 . The sharp tip 17 enables the surgeon to pierce the distal end 13 into the prostate without passing it through a body canal. This feature distinguishes this invention 10 in part from blunt tip wire resection instruments.

In the hollow interior 16 of the shaft 11 there is an electrically conductive deflectable member 18 , as shown in FIGS . 4 and 5. In one embodiment, link 18 is a wire. In another embodiment, link 18 has two flat surfaces 19 and at least one sharp edge 20 . The link 18 makes it possible to cut mechanically with its sharp edge and at the same time to cauterize it with electrosurgical current. The flat surfaces 19 are designed so that the link 18 bends in a predetermined manner. In another embodiment, link 18 has a pre-set and permanent bend so that pulling or pushing link 18 results in a repeatable and predictable deflection. In another embodiment, the link 18 is made of super-elastic alloy. In yet another embodiment, link 18 is made from an alloy of nickel and titanium.

An electrically insulating material 21 is, as shown in FIGS. 4 and 5, point between the member 18 and the shaft 11 inserted. In one embodiment, the insulating material 21 lies on the link 18 so that no electrical current can flow between the link 18 and the shaft 11 . In another embodiment, the insulating material 21 lies on the shaft 11 . In a further embodiment, both the link 18 and the shaft 11 are provided with electrically insulating coatings.

An opening 22 is formed near the distal end 13 of the shaft 11 . In one embodiment, opening 22 is rectangular in shape. In another embodiment, opening 22 is oval in shape. The opening 22 is large enough that resected tissue pieces can be pulled through the opening 22 into the hollow interior 16 of the shaft 11 , as shown in FIG. 3.

A deflection control 23 is located on the handle 14 . The surgeon can operate the deflection controller 23 to deflect a portion of the link 18 outwardly relative to the opening 22 . In a preferred embodiment, the deflection control 23 has a ratchet mechanism 24 with which the position of the link 18 can be incrementally adjusted in a known incremental deflection in front of the opening 22 .

A suction source 25 is connected to the proximal end 12 , as shown in FIG. 2. It serves to suck liquid and tissue from the opening 22 inwards. The suction effect must be strong enough so that resected tissue can be pulled through the opening 22 , through the shaft 11 and out of the proximal end 12 of the device 10 . The tissue and fluids are then collected in a standard collector for later pathological analysis.

A flushing fluid source 26 is connected to the proximal end. The flushing fluid can be passed through the shaft 11 and out of the opening 22 . In the preferred embodiment, a suction / rinsing switch 27 attached in the vicinity of the proximal end 12 of the device 10 serves to select the suction or rinsing mode in the shaft 11 . The switch 27 is connected to a simple trumpet valve 28 , with which the accesses to the suction source or to the flushing fluid source 26 can be opened and closed, as shown in FIG. 2.

In a preferred embodiment, the suction / rinsing switch 27 is a rocker switch with three positions. The rocker switch 27 is biased so that there is no suction at the opening and no flushing fluid flows out. When the rocker switch 27 is pressed in a first direction, the flushing fluid source 25 is connected to the internal channel 16 and thereby allows flushing fluid to flow through the opening 22 . If the switch 27 is pressed in a second direction, the suction source 25 is connected to the internal channel 16 and thus suction is carried out at the opening 22 .

A rotary control 29 is provided near the proximal end 12 of the shaft 11 and thus provides the surgeon with a mechanism for rotating the shaft 11 relative to the tissue. When link 18 is deflected from opening 22 , twisting shaft 11 causes link 18 to ablate tissue. In one embodiment, the rotary control 29 can be a button rigidly attached to the shaft 11 . In the preferred embodiment, the rotary control 29 is a lever which has a joint 30 which allows a reciprocating movement in the direction of the shaft 11 . A rack 31 and a bevel gear 32 connects the lever to the shaft 11 so that the pivoting movement on the lever causes the shaft 11 to rotate, as shown in FIG . An electrical high-frequency current source 33 is connected to the link 18 . The high frequency electrical power source 33 may be any commercially available electrosurgical generator, such as the Force FX electrosurgical generator manufactured by Valleylab, Inc. of Boulder, CO. An electrical switch 34 connects the high frequency electrical power source 33 to the link 18 . A standard switch suitable for medical use can be used for this.

A method for using the electrosurgical resection instrument 10 when removing tissue in the inter-body spaces is also described below. The following steps are carried out: The device 10 is inserted into the tissue by pushing it through the tissue with the sharp tip 17 ; then link 18 is deflected so that it projects through opening 22 ; electrosurgical current is then passed through link 18 , shaft 11 rotated so that the projecting part of link 18 resects tissue from the body; the resected tissue is aspirated through the shaft 11 and finally the tissue is rinsed by means of rinsing fluid which flows through the shaft 11 and out of the opening 22 .
