Minimally invasive surgery is through a diagnostic and therapeutic approach characterized in that the surgical site is no longer like that of the classic chir urgie is opened over a large area by a cut and there with little iatrogenic trauma. About punctual, small incisions or natural bodies  openings become endoscopic, elongated instruments in closed body cavities or in lumens of hollow organs brought in. For visual control of the operational process optics with a connected camera are used, which the Surgeons convey an image on a monitor.

Obvious advantages that the establishment of this Operati procedure in the field of classical surgery have in addition to minimizing the trauma and the resulting physical and psychological consequences for the patient, the reduction of postoperative Pain. The necessary in minimally invasive surgery against small, cosmetically better abdominal incisions low risk of morbidity for infections, suture dehis borders and subsequent scarring fractures In addition, the training of intra-abdominal guards solutions significantly and avoid intraoperative ab cooling the internal organs or reducing the core temperature of the patient. Less pulmonary complications and a shorter gastrointestinal impairment Functions can be determined. Ultimately it decreases the total load of the patient and in his direct The result is a much faster recovery and genes solution, combined with a reduction and cost reduction of the inpatient hospital stay.

Based on the establishment of the laparoscopic chole cystectomy is inevitably affecting endoscopic surgery to classic general surgery operations that from the esophagus to the rectum and from the abdominal wall to the Retroperitoneum range. Endoscopic procedures are performed in terdisciplinary, e.g. B. also in pediatric and thoracic surgery geology, urology and gynecology.

Although tremendous progress has been made in the past few years Development of complex surgical procedures in the field of Minimally invasive surgery was achieved, it turned out always out that the area of application of the endoskopi there are limits to surgery. A key problem in resection procedures, the removal of the resec tats from the abdominal cavity. By the trocar diameter predetermined size of the entrances, on average 3 to maxi times 20 mm, is the extractability of separated organs or Organ parts severely limited. With small and medium-sized inter ventions, such as appendectomy and cholecystectomy, can be a possible metric mismatch between tissue volume and abdominal wall incision through the use of recovery systems (Extraction bag, Bergetrokare, Spreizer) surgically overwhelm be done.

Tissue extraction systems of all kinds take part the operational-technical realization and feasibility minimally invasive interventions should not be underestimated importance. The tissue removal is here not a dedicated problem in general surgery. It exists rather an interdisciplinary development need for all Doscopically operative disciplines.

The technical teaching is described in US Pat. No. 5,176,687 the design of the insertion sleeve on the shape and size of the bag. The sack-like bag without a fixed one The outer contour is created using the instrument's equipment (parts the insertion sleeve) and corresponding aids (tabs) in the desired size unfolds.

A largely liquid and gas-tight extractor bags have a pro ximal and a distal opening, the distal opening the introduction of the resectate into the extractor bag is provided. With a pipe, its length and inside knives are chosen so that the filled with the resect Extractor bags at least largely retractable into the tube the resectate is removed as soon as the extractor bag is drawn into the pipe. There is a closure on the pipe Cover provided that ver the proximal end of the tube closes. A guide tube is inserted in the cover let that have an annular flange at one of its ends having. The flange diameter is smaller than the inside diameter of the tube, but larger than the proximal opening tion of the extraction bag so that the proximal opening can be tightly closed by the flange.

From US 4,611,594 a device is known which for Removal of stones from the kidney or bile is used. This device consists of a catheter in the form of a Tube through which a gripping device can be pushed with which the stones can be grasped. At the distal Elastic struts are attached to the end of the catheter form the shape of a basket and that with a plastic ma are covered with material. The basket formed by the struts can, for example, be axially surrounding the catheter  sliding tube can be opened or closed. Without the basket takes on a force due to the elasticity the original shape of the struts. This device is suitable for the extraction of small resectates. Volumi nasty organs or parts of organs cannot be separated with this here.

Furthermore, a device for ex traction of tissue in the course of laparoscopic opera become known in which a bag at the end of a tubular shaft is attached. The bag has one proximal and a distal opening, the distal Opening for introducing the tissue into the bag and the proximal opening encloses the tubular shaft. During the operation, the bag is inserted through a trocar introduced the patient's body. The one to be extracted Tissue is pulled through the tubular shaft and the proximal opening of the bag guided gripping instructions mentes pulled into the bag. The total is used for extraction device pulled out of the body. With this device only small pieces of tissue can also be extracted ren.

The interoperative use of a retrieval pouch has established itself as the standard technique for the extraction of gall bladders, for example. The advantages of using an extraction bag for cholecystectomy are as follows in an exemplary list:
Protection of the abdominal cavity during the extraction process against loss of gallstones through lesions of the gallbladder wall.
Protection of the abdominal wall and the abdominal cavity against infectious bile: by isolating the gallbladder in a bag, it can be ex-contaminated without contact with other parts of the abdominal cavity or the abdominal wall. With the use of an extraction bag, the resected gallbladder can be recovered without loss of bile.
The extraction process is facilitated by a reduced frictional resistance of the outer surface of the extraction bag. At the same time, the tear bag, which is firmer and easier to grip, enables increased power transmission to the gallbladder.
Protection of the abdominal wall from a tumor cell implantation of an unexpected gallbladder carcass.

The use of an extraction bag alternatively or as Supplement to the inclusion of the freely prepared appendix in the Trocar sleeve before extraction can contaminate the Prevent the abdominal wall or the surgical site with germs  and thus reduces the incidence of postoperative wounding infections.

Extraktionsbeutel bzw. Extraction bags or recovery systems can also be used e.g. B. for bowel resection, nephrectomy, splenectomy, liver surgery section, ovarrectomy, hysterectomy and adrenal rectomy be used.

The operative use of extraction bags from tear Most plastic sheeting has been especially useful for salvage established smaller pieces of tissue. Although different Offered to manufacturers, these bags have a similar one Configuration and roughly a similar functional Construction. Against the background of avoiding contamina abdominal cavity with infectious material, such as play with appendectomy or cholecystectomy, it will Resectate over the opening of the bag in its lumen taken and removed as a whole from the abdominal cavity. To full constant isolation of the tissue has changed this approach proven due to the positive clinical experience.

In the case of larger organ fragments, however, it appears repeatedly the problem that this is for extraction by the already opening from the trocar perforation in the Abdominal wall have too large a diameter, so that egg ne uncomplicated salvage due to the metric mismatch ratio is not feasible. That in the extraction bag resect can not or only partially pulled through the strong layers of the abdominal wall. Either it jams in front of the intra-abdominal side the abdominal wall and deforms through the outward facing tensile force to an even less favorable spherical or widened form, or the resect remains on the  narrowest point of the breakthrough created by the trocar stuck. This is especially the case with fabric pieces made from anato a suitable morphology for the extraction own, the use of an extraction bag for Er Ease of the mountain process itself, but not as regards Lich the isolating effect, because of the strong Defor Mation of the resectate when passing through a perforation to critically evaluate the scorch. So it would offer for example resections of the intestine in their physiological Tighten longitudinally by a smaller cross cutting diameter to a more favorable form of extraction aim. But this is while maintaining the premise insulation with the existing recovery systems possible. Another intraoperatively demanding step represents the absorption of the resectate in the lumen of the bag. This can be caused by oversized fabric parts or by a redu grace of space in the abdominal cavity can be complicated.

The tissue extraction known from US 5,176,687 direction has the disadvantage that the handling, d. H. the Ent fold and collapse the pouch inside the introducer sleeve is difficult. The well-known pouch is also required a large space requirement within the insertion sleeve.

Therefore, with the device to be created a Ber system, regardless of size a receptacle for resect to be removed, can be easily inserted into the salvage cavity, can be opened easily with limited space, even larger resections can be taken easily without increased effort Completely absorb handling and then use it safely can be closed.

A device that fully meets the above problem come solves, consists of a tissue extraction device with a bag that is different in terms of its intake volume enlarged funnel-shaped towards the distal end and then merges into a section that spans the axial length hen has a constant diameter, and accordion Ka-like or rollable on and / or around the outer circumference surface of the instrument guide sleeve can be applied.

With such a tissue extraction device the bag without any special precautions over a set Trocar, which in any case with minimally invasive surgery pla is decorated, insert into the intended body cavity. At this movement process slides a smooth outer surface the insertion sleeve on the inner surface of the trocar into the predetermined body cavity. The one for the inclusion of the Re sektats certain bag lies protected between the in instrument guide sleeve and the insertion sleeve, so that the Bags during the insertion process in the predetermined basket per cavity not with objects moving relative to each other that comes into contact. The insertion sleeve is completely in inserted a trocar known per se, the gap Ver. gas-tight between insertion sleeve and trocar if necessary closes a surgical instrument by the In instrument guide sleeve pushed so that the prepared Resect can be held in place and held firmly. If necessary, there is also a movable surgical instruct ment gastight against the instrument guide sleeve ten.

During the entire recovery process, the resectate is removed from the surgical instrument. After grasping the resec tats with the instrument, the much longer instruct  ment guide sleeve relative to the surgical instrument and relative to the insertion sleeve so far into the body cavity one moved so that the between insertion sleeve and Instru ment guide sleeve folded and / or rolled up bags completely over the free end of the insertion sleeve and over the free end of the trocar protrudes. In this process not only the protection of the bag by the insertion sleeve given, but the bag unfolds at least to Part automatically. With special configurations of the Beu the folds that do not open automatically about aids, for example about one over two Ten trocar inserted pliers, loosened so the bag can release its entire size. With a first proxi paint end is the pouch at the distal end of the instruments guide sleeve fixed in such a way that the lumen of the In instrument guide sleeve for the use of a surgical Instruments, such as a gripper, not affected is pregnant and the bag with its second free di stale end over the resectate holding surgical instrument with aids such as another gripper, can be pulled. The size of the bag and therefore that Recovery lumens for a resect can meet medical requirements can be easily adjusted. Is the bag over the resect the bag is pulled up and completely separates the resect from adjacent tissue, so the bag is at the distal end sealed fluid-tight. This can be done via known ones Means (ligature loop) or be below written aids are made. Is the resect in the bag insulated, the trocar and the insertion sleeve are removed from the Perforation opening removed. It should be noted that the axial length of the instrument guide sleeve and the chirurgi instruments are much larger than the axial length of the placed trocar and the insertion sleeve used. The  Resectate is now removed using the instruments guide sleeve and the surgical instrument through the from Perforation point created through the trocar outwards in pulls the extracorporeal area. The bag is tearproof connected to the instrument guide sleeve so that the In instrument guide sleeve only together with the bag from the Body cavity can be pulled out. If necessary, can the surgical instrument right during the extraction process relative to the instrument guide sleeve even further to the outside to be pulled. Through that and through the compression that through fixation of the resect by the surgical instruct is accomplished, the resect to be removed lengthened and its outer diameter reduced in the bag. At The resect can be simplified for large resected specimens pull through a perforation constriction. With which he device according to the invention is the surgeon in the area minimally invasive surgery This is done with simple, clear movements procedures allowed to isolate a resectate and contact to remove without mination.

With the bag according to the invention, the resect is for example with a tear and gas-resistant polyurethane film envelops the entry of the deposited resectate into the Abdominal wall opening easier and getting caught in the Abdominal wall prevented. In the device according to the invention act the surgical instrument, for example a Gripping pliers and one on the instrument guide sleeve functional bag. Using the gripper the fixed resect is extracted during its extraction process Longitudinal axis tightened and thus prevents its obstructive Change of shape also when overcoming resistances during the extraction process on the bag and thus also on  the resect work (exert pressure). Especially before the additional use of a surgical In is partial struments during the extraction process of resectates in the bag, which have a high proportion of connective tissue, because in this If the resectate is firmly held with the chir urgent instrument is possible. Resectates with comparative wise high connective tissue levels arise with cholecystecto mien, appendectomies and intestinal resections. Furthermore in this context the anatomical elongated mor Resectate phology is positive during the extraction process be used. By removing the bag over that de resectate is pulled, the insertion process of the Resectate in the bag is significantly simplified and in terms of time shortened. To avoid contamination from leakage The germs from the ingested resect must be primary half-open bags can be closed. According to Resek fact recording secondary by an additive closure on free end in the edge area of the bag by means of a league Turschlinge take place or through already in the bag and in the Device integrated aids, the following be be written.

The collapsed pouch between the insertion sleeve and instrument guide sleeve folded particularly small or can be rolled. Also open up through the shaping folding techniques that allow the bag so that it is easiest in the body cavity and can be opened safely.

The bag is between the insertion sleeve and the instruments accordion-like or rollable on the Outer peripheral surface of the instrument guide sleeve is attached arranges, so that the bag saves space between  these sleeves can be held while the bag is still in place not beyond the insertion sleeve into the body cavity is advanced. Is the bag around the instrument guide rolled up around, the bag unfolds when released from the insertion sleeve automatically, so as far as reeling is concerned.

In a further embodiment of the device according to the invention device, the insertion sleeve can be inserted into a trocar has essentially the axial length of the insertion sleeve. This has the advantage that the instrument guide sleeve even with the slightest axial displacements compared to the Insertion sleeve and the trocar the bag opposite the front direction can release.

In a preferred embodiment of the invention, the A an outer sleeve at the proximal end, the is larger than the lumen of the trocar. Also, the Instrument guide sleeve at the proximal end of an outer man cuff that is larger than the lumen of the insertion sleeve. This has the advantage that even with unintended movement conditions and relative displacements from the trocar to the insertion sleeve and / or from the insertion sleeve to the instrument guide sleeve it is excluded that these sleeves inadmissibly wide the trocar can be inserted.

In a further embodiment of the invention direction is the instrument guide sleeve opposite the Introducer sleeve or the introducer sleeve against the trocar gas tightly closed. This has the advantage that with the invent Intervention device can also be made interventions that require permanent space expansion during the procedure with an additional gas,  like creating carbon dioxide.

In a further preferred embodiment of the invention Introducer sleeve and / or the instrument guide sleeve made of egg or a stiff, tissue-compatible plastic material made of a metal. These materials can be rei Exercise-reducing surfaces are provided and also with Be may be sterilized again.

The bag used in the device according to the invention is made of a thin, tear-resistant, gas-tight and together layable material, for example made of a polyurethane film or from another tissue-compatible plastic film, manufactured. This ensures that even with increased Force applied to the bag containing a resect this does not tear and parts or the entire resect are uncovered can release after the bag is fluid-tight was closed. It also ensures that not parts of the bag after extraction in the body remain.

The bag is preferably at the distal end with a reinforced edge, preferably a duplicate. This has the advantage that the bag when pulled over the Re sektat can be easily grasped by aids and in ver at the end of the bag the loop is stabilized with which the Bag is closed. The noose is also attached to it prevented from slipping off the bag when increased pressure acts on the bag from the outside and / or inside.

The edge of the bag can be marked in color. The col Marking makes it easier for the surgeon to remove the pouch to handle indirectly.

In a further preferred embodiment of the invention according device is in the bag in the area of the edge a stable, bendable guide around its entire circumference guidewire incorporated over an in the axial direction stiff guide sleeve passed through the insertion sleeve is, the guide sleeve in the insertion sleeve and the Guide wire is slidable in the guide sleeve.

This has the advantage that the bag can be used without additional aids medium fully unfolded, opened, over the resect can be pushed and then sealed gas-tight. This Embodiment has compared to the previously described Embodiment two more, the entire handling united subject-specific configurations. About the guide sleeve the bag unfolds completely in the axial direction, and The distal opening of the bag is over the guide wire kept as open as possible. By means of the guide sleeve Bags are pushed over the resectate, and over the guide The wire is then sealed fluid-tight closed without any further trocar aid inserted into the body cavity for handling the bag Need to become.

In a further embodiment to the second embodiment example, the bag is shaped such that it ver closed second end of the bag by means of the guide wire in the distal end region of the bag in the direction of guide sleeve can be deflected. This has the advantage that in an extraction process, the maximum pressure that the Perforation constriction acts on the bag, not on the Closure point of the bag is passed on, but on a bag area is shifted to a completely ge  closed area represents. It is also for those too additional securing of the closure of the bag possible, the second free and closed end of the bag over the Insert the guide wire into the lumen of the insertion sleeve. Is the insertion sleeve has already been removed from the incision yourself an additional security if that first over the Guide wire sealed end of the bag through the same The incision is drawn back into the extracorporeal area becomes.

In a further embodiment of both the first embodiment Example as well as the second embodiment is between the surgical instrument and the instruments guide sleeve, from the proximal end of the instrument guide starting sleeve, a channel formed, which in the In inner area of the bag opens. This has the advantage that Liquid is also drained from the bag via this channel can be sucked or the bag evacuated if necessary can be. The suction or evacuation can also a corresponding instrument on the surgical In strument itself.

In addition to the tissue extraction device according to the invention device is a method for handling the invention Tissue extraction device presented in which the tissue extraction device in the first assignment of the Insertion sleeve to the instrument guide sleeve in a pla graced trocar is introduced that subsequently or at the same time the surgical instrument, preferably one Grasping forceps, through the instrument guide sleeve is pushed and the resect to be removed with the surgeon instrument is held that the In instrument guide sleeve so far out of the insertion sleeve  is pushed until the bag is released either automatically or unfolded with aids and with other aids about the resect to be removed come out that the bag is fluid-tight on the second free end is closed and that after that the Resek bags containing tat by means of the instrument guide sleeve se is pulled through the fuselage perforation, with to in front of the trocar and the insertion sleeve from the fuselage have been removed.

If the device is handled according to the method, the Pouch securely inserted into the intended body cavity and be unfolded there. The resect can be easily bring manipulations into the bag, and the bag can be closed fluid-tight. During the extraction process If necessary, the resected specimens can be lengthened and thus in Reduce cross-sectional diameter so that even larger Re sectates can be pulled through small incisions. Here it is of particular advantage that the entire extract a substantial part of the pulling force from the surgery instrument is exerted on the resect. The one individual work processes are clear and make it easier the surgeon, the flawless isolation and extraction of a resect.

If necessary, the contents of the bag can be vented and / or be freed of liquid before the bag passes through the Fuselage perforation site is pulled. This has the advantage that the cross-sectional diameter of the resect and bag reduced in size and thus with reduced effort a perforation constriction outwards in the extracorporeal Area can be dragged.

The individual figures of the drawing show the fiction moderate device highly schematic, and the individual off Designs on the device are not to scale understand. Also in the figures was a commercial standard cher Trocar indicated very schematically. The figures were  therefore kept schematized so that the fiction moderate features or the handling of a method in Ver binding with the tissue extraction device according to the invention tion can be clearly shown. In all the figures the same reference numerals for functionally identical features applies regardless of whether it is a changed end leadership example.

Fig. 1 shows with 10 a tissue extraction device according to the invention, as it is inserted into a trocar 11 , which is only indicated with simple lines. The trocar 11 is placed in an abdominal wall 12 , and the tissue extraction device 10 is inserted into the trocar 11 . The tissue extraction device 10 is composed of an insertion sleeve 13 and an instrument guide sleeve 14 , to which a bag 15 is attached. On the Instru ments guide sleeve 14 is attached to the open fene bag 15 at the distal end. In the instrument guide sleeve 14 , a surgical instrument 16 , for example a gripping or grasping forceps, is inserted, which can move grips 17 grippers 18 at the distal end of the surgical instrument 16 . Via the grippers 18 , a resect 19 can be gripped in a stable position.

On the insertion sleeve 13 , an outer sleeve 20 is ausgebil det, which forms a stop relative to the trocar 11 and causes the insertion sleeve 13 can only be moved in the axial direction towards the resectate 19 to a limited extent. On the instrument guide sleeve 14 at the proximal end of the instrument guide sleeve 14, an outer sleeve te 21 is formed, which forms a stop relative to the insertion sleeve 13 and prevents the instrumentation guide sleeve 14 from being inserted into the insertion sleeve 13 in an impermissible manner. The insertion sleeve 13 has ends 22 , 23 which delimit a lumen 24 of the insertion sleeve 13 , the lumen 24 being formed continuously in the insertion sleeve 13 . A lumen 25 is formed in the instrument guide sleeve 14 , through which the surgical instrument 16 can be pushed through the instrument guide sleeve 14 .

Around an outer peripheral surface 26 of the instrument guide sleeve 14 , the bag 15 is arranged folded and adjoins an inner peripheral surface 27 of the insertion sleeve 13 . In the form shown in Fig. 1, the Beu tel 15 is completely protected by the insertion sleeve 13 . With the surgical instrument 16 , the resect 19 can be kept summarized.

Fig. 2 shows the tissue extraction device according to the invention of Fig. 1 in individual parts. The insertion sleeve 13 is made separately from plastic or metal, and it has the outer sleeve 20 at the proximal end. The dashed lines indicate the lumen 24 , which runs through the entire insertion sleeve 13 at open ends. With the reference numeral 28 , a seal is shown schematically, via which the instrument guide sleeve 14 can be gas-tightly sealed with respect to the insertion sleeve 13 . A breakthrough in the seal is indicated by dashed lines.

The instrument guide sleeve 14 has a lumen 25 which is formed with free open ends of the instrument guide sleeve 14 throughout the entire axial length of the instrument guide sleeve 14 . On the instrumentation guide sleeve 14 is formed on the distal end of the bag 15 , which is made of a thin tear-resistant tissue-compatible film. The film is also fluid tight. At the distal end, the bag 15 is provided with an edge 29 , which reinforces the bag at this end preferably via a duplication. The bag 15 is fully unfolded in the central representation in the axial as well as in the radial direction and shows that it is funnel-shaped out in a partial area. In the lower illustration, the bag 15 is partially folded, and the folds are never indicated 30 over a Faltli. The bag 15 is BEFE via a first open end 32 on the instrument guide sleeve 14 so that the lumen 35 is not impaired and remains open to the outside. A second free open end 33 is formed on the bag 15 at the distal end.

Fig. 3 shows the tissue extraction device according to the invention, as it is inserted into the trocar 11 and the abdomen wall 12 penetrates. The introducer sheath 13 is inserted up to the trocar 11 resting on outer sleeve 20 in the trocar. 11

The instrument guide sleeve 14 is displaced relative to the insertion sleeve 13 in the direction of the resect 19 so that the bag 15 is completely released with respect to its axial extension on the instrument guide sleeve 14 . The bag 15 lies folded on the instrument guide sleeve 14 in the radial direction, already partially opened. A fold line is indicated by reference numeral 30 . The bag 15 is partially open and the reinforced rim 29 limits the bag 15 in the distal direction. In the direction of arrow 34 , the instrument guide sleeve 14 was inserted into the insertion sleeve 13 until the outer sleeve 21 abuts the insertion sleeve 13 like a stop.

Fig. 4 shows the embodiment of FIG. 3 with a white surgical instrument, a grasping forceps 35 , by means of which the bag 15 is unfolded in the axial direction, as far as the grasping forceps 35 is moved in the direction of arrow 36 . The grasping forceps 35 hold the bag 15 on the reinforced edge 29 and thereby engage in the opening of the bag 15 at the second free end 33 . In this process, the resect 19 is held by the surgical instrument 16 unchanged in a stable position.

Fig. 5 shows an embodiment according to FIG. 4 with fully developed men bag 15 , which was pulled with the aid of the forceps 35 in the direction of arrow 36 over the resect 19 . The second free end 33 of the bag 15 is still completely open. The resect 19 is held within the bag 15 by the surgical instrument 16 . In addition, a known ligature loop 37 is indicated in FIG. 5, which is brought up to the bag 15 via additional aids, so that the bag 15 can be fluid-tightly closed in the loading area of its second free end 33 .

Fig. 6 is compared with FIG. 5, as the snare loop 37 has been pushed to the bag 15 via the edge 19. The ligature loop 37 is placed in the edge region of the bag 15 .

Fig. 7 is compared with Fig. 6 that can slide down from the Beu tel 15 the perfect closure of the bag 15 by means of the snare loop 37, the edge 29 forms a bead to the effect against external pressure to the bag 15, the snare loop 37 is not. The trocar 11 and the insertion sleeve 13 are pulled out in the direction of arrow 38 from the abdominal wall 12 and the instrument guide sleeve 14 , to which the bag 15 is attached, is now also in the direction of arrow 38 through the perforation in the abdominal wall 12 to the outside into the extracorporeal area gezo gen. The surgical instrument 16 keeps the Re sectate in the bag 19th The surgical instrument 16 and the instrument guide sleeve 14 are pulled together through the perforation point in the extracorporeal area.

Fig. 8 shows a second embodiment of a tissue extraction device 40 according to the invention which, in addition to the guide sleeve 13 and the instrument guide sleeve 14, has a guide wire 41 which leads to a guide sleeve 42 . The guide wire 41 is integrated in the bag 15 in the edge region of its distal end and keeps the second free end 33 open. In the figure, the bag 15 is fully deployed in both the axial and radial directions. The bag 15 is connected to the instrument guide sleeve 14 at the first open end 32 . The Instrumen tenführungshülse 14 is inserted into the lumen 24 , and the lumen 25 can accommodate a surgical instrument if necessary. The guide sleeve 42 is rigid in the axial direction so that it can move the bag 15 from the Instru ments guide sleeve 14 down.

Fig. 9 shows the tissue extraction device 40 according to the invention, as it is inserted into a trocar 11 . The tro kar 11 penetrates the abdominal wall 12 and thus allows access from the extracorporeal area to the intra-corporal area. In the trocar, the insertion sleeve 13 is inserted with the instrument guide sleeve 14 . The bag 15 is arranged between the insertion sleeve 13 and the instrument guide sleeve 14 folded. The guide sleeve 42 is in this position behind the bag 15 is arranged so that the outer diameter of the insertion sleeve 13 can be kept as small as possible. The resect 19 is gripped via the surgical instrument 16 . The guide wire 41 and the guide sleeve 42 protrude far beyond the Tro kar 11 in the extracorporeal area.

Fig. 10 shows the embodiment of Fig. 9, wherein the instrument guide sleeve 14 , the guide sleeve 42 and the guide wire 41 have been moved so far in the direction of arrow 43 that the bag 15 protrudes beyond the insertion sleeve 13 . The bag 25 is still partially folded. The surgical instrument holds the resectate 19 .

Fig. 11 shows the embodiment of FIG. 10 with the bag 15 fully unfolded, the guide sleeve 42 having been moved in the direction of arrow 44 . During this displacement, the bag 15 opens in the axial direction and is pushed over the resect 19 . By moving the guide wire 41 in the direction of arrow 44 , the bag 15 is completely opened and the reinforced edge 29 is stabilized.

FIG. 12 shows the embodiment of FIG. 11 with the bag 15 closed. The guide wire 41 was withdrawn in the direction of arrow 45 and thus the second free end 33 of the bag 15 was closed fluid-tight. About the guide sleeve 42 , the bag 15 can be kept taut and elongated in the axial direction if necessary. As a result of this elongation, an additional compression can be applied to the resectate in such a way that the bag and the resectate can be extracted with less effort. This compression advantageously reduces the cross-sectional diameter of the resect. The resectate 19 is now trapped in the bag 15 , and the surgical instrument 16 continues to hold the resectate 19 in the bag 15 .

Fig. 13 shows the embodiment of the tissue extraction device of Fig. 12 in a more advanced stage of the extraction process. The bag 15 was in the area of its distal end at the second free end 33 deflected so that the maximum pressure of the resectate in the bag does not press on the second free end 33 . The trocar and insertion sleeve have been withdrawn in the direction of arrow 46 so that they are outside the abdominal wall 12 . In order to complete the extraction process, the surgical instrument 16 with the instrument guide sleeve 14 and the bag 15 , the guide sleeve 42 and the guide wire 41 in the direction of arrow 47 through the abdominal wall 12 into the extracorporeal area must now be pulled out.

A tissue extraction device to facilitate the retrieval of resected 19 by a Rumpfperforationsstelle comprises an introducer sheath 13 and an instrument guide sleeve 14, wherein the gap between the introducer 13 and is disposed in strumentenführungshülse 14 a bag 15 folded up. Through the instrument guide sleeve 14 , a surgical instrument 16 is passed, which holds a Re sectate 19 during the entire extraction process. The instrument guide sleeve 14 can be axially ge move such that the Beu tel can be pushed from the introducer sheath 15 of the introducer sheath 13 genüber. 13 The bag 15 itself is attached with an open end to the instrument guide sleeve 14 and the other open second end 33 of the bag 15 can be pulled over a resect 19 ge. The bag 15, after the resected tissue is completely enclosed by the bag 15 19 fluid-tightly closed at the free second end of the 33rd Subsequently, the insertion sleeve 13 is removed from the fuselage perforation site and the instrument guide sleeve 14 with the bag 15 , the resect 19 and the surgical instrument 16 , which holds the re sectate 19 during the entire extraction process, are pulled richly through the fuselage perforation site into the extracorporeal area.
