                        发明的领域
总体上本发明涉及用于治疗青光眼的眼植入物。
                        发明的背景
青光眼是一种具有如下特征的眼状况:即眼睛的眼内压(int raocular pressure,IOP)升高到一异常水平。正常眼通过水状液于眼中的循环而保持一适宜的IOP。水状液从睫状体分泌出来,流经瞳孔进入眼球的前室,且通过小梁和Schlemm氏管(Canal ofSchlemm)透过到眼球之外。患青光眼时,水状液排泄通道受阻,水状液不能以足够的速率流出眼球,IOP升高,眼球变硬,视神经因为其从视网膜伸出的纤维受压而萎缩。具代表性的视神经病发展,导致视网膜中的神经节细胞的进行性死亡,视野受限,以致最终失明。这一疾病的后期的特征还包括显著的疼痛。
青光眼治疗,如果于患病早期即开始的话,是可以阻止眼功能的进一步恶化并保存大部分的眼功能的。青光眼治疗的目标就是把一特定的眼睛的IOP降低到一被认为对一特定的眼睛安全的水平,但却不致于低到导致眼功能失常或视网膜并发症的产生。
青光眼治疗方法的类型之一是透过外科手术,它为水状液流出眼球前房并进入结膜下空间提供了一条替代途径,从而降低了IOP,在全厚度手术中,贯穿边缘巩膜产生了一条瘘管,瘘管把眼球前房和结膜下空间直接连接起来。全厚度手术使得可对IOP进行持久的控制;然而,在术后期的前期眼球过量损失水状液经常导致低压症状。
在有保护的透过外科手术(小梁切除)中,穿过切开巩膜而产生的瘘管由一位于其上的部分厚度的缝合巩膜瓣片来保护。巩膜瓣片为眼球过度损失水状液提供附加的阻力,从而减少了在术后期的前期患低压症状的风险。然而,小梁切除,比起全厚度手术,可能导致最终的IOP较高,而且在后期透过失败的风险增大。
根据最近引用的一个方法,全厚度透过瘘管可由一钬激光探针来产生,这样外科手术所引起的外伤程度最轻。实施眼球后麻醉之后,在预切开部位期的巩膜切开部位之后大约12~15mm处切开一个结膜切口(大约1mm),然后把一激光探针通过结膜下空间插到角膜周围与巩膜连接部分上。然后,施加多个激光脉冲直到产生一全厚度瘘管。这一技术因为不易控制巩膜切口的尺寸而有时很快就导致低压症。另外,虹膜或早或脱垂到巩膜切口处将会迅速把瘘管堵住,最终导致外科手术失败。再者,虽然这一技术相对比较简单,但仍需使用眼球后麻醉来避免因施用激光而产生的疼痛。在临近已经受损的视神经的位置注射麻醉剂有时会引起进一步的视觉损伤。这一步骤,以及其他类型的青光眼透过外科手术的另一缺点是瘘管容易因为结疤而被封死。
为了克服透过外科手术的问题已经进行了各种尝试,例如,通过使用眼植入装置。典型的眼植入物使用排液管,以便保持在眼球上形成的开口的完整性,上述开口是用于降低IOP的。
通常的眼植入物有几个缺点。例如,植入物通常使用一阀构造,用于调节从眼球流出的水状液流量;此类阀构造的缺陷及/或失效可导致眼球过量损失水状液并可能导致低压症。随着时间推移,此类植入物也易于堵塞,要么是从内侧被组织,如虹膜,所堵塞,(此间虹膜被吸入入口处),要么就是从外侧被细胞增殖,如结疤,所堵塞。再者,通常的植入物插入手术很复杂,费用高,而且耗时长。
Donowitz等人的美国专利号3,788,327示明了一种植入物的已有技术,其利用一阀构造来调节从眼球流出的水状液流。如上所述,此类阀构造的缺陷及/或失效可导致眼球过量损失水状液,并可能导致低压症。另外,美国专利号3,788,327中所示的植入物的入口和出口都易于受堵—入口由虹膜所堵而出口由结疤所堵。最后,植入一根据美国专利号3,788,327的植入物可包括分离的步骤:即首先提供一用于容纳植入物的通道,并且/或者当植入物固定在位时把植入物缝合,这就增加了时间并可能增加手术的复杂性。
                        发明内容
本发明的目的是提供一改进的眼植入物,它被植入眼球用来治疗青光眼。

在一本发明的改进的植入物的实施例中,提供了一眼内植入物用于被植入于眼球之中。这一植入物包括一个管,管有一入口端,一出口端,以及一介于其间的管通道,以允许水状液流出眼球,还有一在管的出口端连接到管上的带凸缘的碟片。管通道的横截面积足够的小,以抑制水状液流出管通道。管通道的横截面应小的足以在IOP低于阈值时防止水状液流出。

为了防止植入物阻塞,管在其入口端设置有一斜角表面,它在植入物被插入时是朝离虹膜的。另外,可沿着管设置有一个或多个圆周孔,即使在轴向入口受阻时也允许水状液流入管通道。
为了防止在出口端发生阻塞,带凸缘的碟片可具有一个如上所述的外缘,它把结膜提离管通道的轴向出口,以允许水状液外流。为了以上目的,也可在带凸缘的碟片上设置一个或多个内柱(它可设计成内缘的形式)。通过把植入物植入到结膜下并离开结膜中的插入切缝一段距离可进一步避免阻塞,因为切缝的愈合并不会导致结疤的组织形成于植入物的轴向开口区域。
植入物的其他特征可便利植入操作。例如,植入物可以有一个或多个固位突起部(如,刺铁状,凸缘状,或板状)。固位突起部可以是刚硬的,或者它也可以由弹性材料做成,以使其在穿过巩膜时紧靠着管向内弯曲。或者,固位突起部也可设计成开始时就相对平坦地靠着管子以使穿过巩膜更加容易并且防止巩膜被撕裂,此时还设计有一构造,用于在植入物已被植入眼球之后使固位突起部向外伸展。例如,固位突起部可由一单独的膨胀工具来使其向外伸展,或者可由一种形状记忆材料,如PMMA或镍,构造而成,这样当其受到眼球热量作用时即向外伸。一个或多个根据本发明的固位突起部已足以可靠地把植入物锚定于眼球中而不需缝合,节省了时间和费用。
通过在植入物上设置一个或多个标记物也可以便利植入操作,上述标记物在穿过巩膜时透过角膜可以看得到。例如,如上所述的一个圆周孔即可充当一标记物;或者,标记物可以是其他合适的看得见的结构,如管子上的刮痕或颜色标记。标记物的可视性使医生可以知道标记物已经穿过巩膜,这标志着植入物已经到位。
植入操作可以通过利用一根据本发明的植入装置来实施,上述植入装置包括一手柄和一杆状器械,例如一针或探针,用于承载植入物并把其插入眼球。植入装置有一尖端,用于插入植入体的管通道之中,以及一合适的固位机构,用于防止植入物在植入操作中向植入装置上移动。同时也可构造固位结构用于防止植入物在植入操作中转动,从而确保植入物在眼球中的适当的定位。植入装置另外也可有一合适的膨胀工具,用于在植入体的一个或多个突起部刚刚穿过巩膜时即使其向外伸展。
在一根据本发明的把一植入物植入眼中的改进的方法的实施例中,在结膜的一部分中切开一个小的切缝,这一切缝通常是离开预定的植入位点一段距离。由于植入体本身就很小,切缝也可以切得很小,例如其长度可以是大约2mm或更小。切缝的小尺寸以及把其定位于离开植入位点一段距离,比如大约10mm,的位置可以帮助防止巩膜切开部位的污染并减少感染的风险。
植入物通过上述切缝被置入并被导至植入位点,然入在植入位点处被插入巩膜之中。可以用植入物的管的一针状尖端来穿刺巩膜,上述尖端是由如上所述的管的入口端的斜角表面所形成的;或者也可以由承载着植入物的植入装置的针的尖端来穿刺巩膜。这样,不需任何单独的穿刺步骤即可把植入物直接插入眼球,从而节省了费用和时间。
根据本发明的眼内植入物,植入装置以及植入方法具有全厚度瘘管的优点,而却避免了标准小梁切除的局限性。根据本发明的植入物可以很小且不需外科手术即可植入。不需外科手术室或住院,从而节省了费用。植入操作具有最低限度的侵害性,简单,快速、仅需局部麻醉。不需进行眼球后麻醉,这样就避免了对视神经的医源性损伤。不用施行虹膜切开术,这样使水状液流得到保护,晶状体的营养不受影响,而且因为虹膜切开术而导致发展起来的白内障发生的可能性也降低。

                      附图的详细描述
图1所示的是一眼内植入物30,它是根据本发明的一个实施例构造的,被植入于一眼球10之中。植入物30包括一针状管32及一碟片34。带凸缘碟片34的平面和管32形成一角度,这一角度对应于在巩膜12的表面和植入物30插入轴线之间的夹角。在角膜16邻近的切口区域。植入体30被插入眼球10的巩膜12之中,并突出进入毗临虹膜22的前房20。植入物30被插入以使带凸缘的碟片34被安置于位于结膜18之下的巩膜12的一个表面之上。植入物30可被安置于Tenon氏囊体(未示出)之上或之下。具有本领域的一般技术的人士都应理解,植入物30插入的具体位置并不仅局限于所示的,其他适合的位置,如位于虹膜22之后,也是可以的。

管通道38的横截面积小得足以抑制水状液流出管通道。例如,在一实施例中,圆筒形的管通道38的直径大概有300μm。通过为管通道设定一特定的内横截面积即可防止眼球中的水状液过量流失。
当IOP高于某一阈值,比如说大约5mm汞柱时,水状液通过轴向开口41以及一个或多个圆周孔42从眼球10的前房20中排出,通过管通道38并进入位于结膜18之下的空间。圆周孔42可以具有任何一种适合的形状;比如它们可以是圆形开口,其组合的横截面积与管通道38的横截面积相等。圆周孔42可防止管通道在其入口端受堵,因为即使虹膜22把轴向入口41堵住了,水状液仍能从圆周孔42流出。当轴向入口41受堵时,圆周孔还可在管通道38中产生一反向压力从而把受堵的轴向入口41疏通。圆周孔42的另外一个用途是被用于确定植入物30的恰当的插入深度,因为在插入操作中当植入物已穿过巩膜之后,上端的孔是看得见的,从而可以被当作一标记物。其他合适的标记物,如刮痕或颜色标记,也可被用于此目的。
管32的入口端40有一由斜角表面36形成的针状尖端,其斜角很尖锐,易于插入眼球。斜角表面36增加了轴向入口41的面积从而扩大了管通道38的入口。斜角表面36被设计成背离虹膜22,以减少轴向入口41受堵的可能性。因为带凸缘的碟片34被设计成紧贴着巩膜14而斜角表面36则被设计成背离虹膜22,所以斜角表面36所处的平面与碟片34所处的表面成一定角度。
管32可有一个或多个刺铁状的固位突起部,其与管32是一体成形的,用于在插入之后把植入物30固位于眼球10之中。或者,固位刺铁52也可被做成是连接到管32之上的单独的零件,连接可以用熔接,或者硬焊。固位刺铁52可以是刚硬的,也可以是柔韧的,这样它在穿过巩膜时可朝着管32弯曲,而在穿过巩膜之后刚向外回弹到其初始外形了。或者,固位刺铁52也可被设计成可进行塑性变形,当它位于眼球10中时,可由一单独的膨胀工具(如气球)来使其变形,或者固位刺铁52可由形状记忆合金构造而成,这样当合金是冷的时候它平靠着管32,而当它受到眼球10的热量作用时即膨胀到其最终外形。

碟片34的外缘46除了界定贮液池54之外,其另一作用是把结膜18提离轴向出口51以防止管通道38受堵。内柱48也有这一作用。
碟片34可以是椭圆形的,但并不局限于此。具有本领域的一般技术的人士应可理解:碟片34可以具有任何一种允许植入物适配到结膜18之下而又可调节IOP的外形。也可以改变带凸缘碟片34的尺寸及/或其外形以及/或者带凸缘碟片34与管32之间的夹角,以便对不同人的眼睛使用不同的植入物。
图8至10示明了一植入装置60及一把眼内植入物30插入眼球的方法。植入物30首先被附接到植入装置60上。植入装置60有一手柄62及一合适的杆状器械64,如一针或探针。杆状器械64有一尖端70,用以穿入植入物30的管通道,还有一固位结构,用于防止植入物在植入操作中移到植入装置之上,例如可以是一抵挡表面68,其角度总体上与碟片34的斜角相对应。这一构型也可防止植入物在植入装置上转动,从而确保了植入物在眼球中的恰当定位。固位构造也可包括一个或多个突起部,用于在外缘之内及/或在带凸缘碟片34之上的内柱之间延伸。在另一替代实施例中,固位构造可以是杆状器械的尖端,其构造成可通过摩擦适配而配合到植入物的管通道的内部,从而阻止植入物在植入操作中移到植入装置之上。

为了把植入物30插入眼球10,在结膜18的一部分中切开一小切口26,切口26正常是离开遮盖住预期植入位点的结膜18的一部分28一段距离的。离开植入位点的一小切口,如离开植入位点大约5~15mm的一个1~2mm的切口,可减小水状液从切口流出结膜的可能性,减小感染的可能性,减小在植入物的轴向出口结疤的可能性,并便利了闭合和愈合。
通过植入装置60把植入物30送过切缝26,沿着结膜18的下面把其送到于巩膜14中的植入位点。图9仅以简图示明植入物的推进情形;应该理解的是:在实践中,植入物大致上是沿着巩膜的表面被从切口导至植入位点的,这样植入体的纵长轴总体上就平行于巩膜的表面。到达植入位点后,植入物被倾斜以刺入巩膜。由植入物30的斜角表面36形成的针状尖端的锐角确保了植入物30可容易地进入巩膜14。针状尖端刺穿巩膜14进入眼球10的前房20,而碟片34则被推至贴靠于巩膜14之上。
当植入物30已处于应当的位置,如图10中所示,固位刺铁(或刺铁)52把植入物30锚定于眼球10之中并防止植入物30在植入装置60被抽出时滑出来。固位刺铁52也可防止植入物30在到位之后滑出来。
具有本技术一般技术的人士应可理解,植入物的插入并不限制于上面所述的方法,其可用本领域已知的几种方法的任意一种插入。植入装置可包括一“内部”或“外部”针。也可用一本技术所公知的直的或绕曲的导向线,用于把植入装置导至其准确的位置。为了使插入容易,可以振动植入装置,或者可以在植入装置上涂一层润滑剂,如医用膏或胶。另外,在植入物被植入之后,应在植入位点加一种适合的抑制纤维化的化合物(如5Fu,丝裂霉素)。
图11所示的是根据本发明而构造的眼内植入物130的一替代实施例。植入物130包括一附接于一椭圆形碟片134上的管132。管132有一入口端140,一出口端150,以及一管通道138,管通道138则有一轴向入口141,一轴向出口151,以及圆周孔142,圆周孔142用于把水状液从眼球10的前房20排到结膜18之下的空间。
管132的远端152有一部分圆锥的外形。在管132的外圆周上形成有许多固位突起部,其形式是固位凸缘158,固位凸缘158大致平行于碟片134,用以把植入物130锚定于眼球中。

如图12中所示,碟片134的外形呈椭圆形。碟片134的较长轴尺寸大概是管132的直径的两倍,且碟片134相对于管132是呈偏心圆的方式设置的。椭圆外形和碟片134的设置方式允许植入物130有一广阔的锚定面积并使在椭圆的较长轴上的出口排液面积达到最大。椭圆的较短轴使得植入物130能在适配于巩膜18下的狭窄的空间之内。
图13所示的是一植入装置160以及一把眼内植入物130插入一眼球的方法。植入物130以可滑动的方式固定到植入装置160的针164之上,植入装置160与标准的医用注射器是相似的,有一附接于外壳62上的针164。针164的尖端174穿过植入物130,有一锐角使得尖端174大体上与植入物130的下部的角度相一致。
植入装置160的一前部表面被做成一抵靠表面,其角度与碟片134的角度相匹配,抵靠表面还进而包括一凹入部位172,用于在植入操作中把植入物130保持在位。植入装置160的外形以及带凸缘碟片134的成角度表面防止植入物130在植入操作中转动。
图13中的所示的植入装置160的使用方法与图8至10中所示的植入装置相类似。然而,在这一实施例中,针尖端174的锐角穿刺巩膜。植入装置130的成角度的入口端后随针尖端174穿过巩膜14,进入眼球的前房20。如图14中所示,固位凸缘158把植入物130锚定在位并防止植入物130在植入装置160被抽出时滑出。固位凸缘158的锚定功能也防止植入物130固定在位之后滑出。

根据本发明构造的眼内植入物可完全由任何一种合适的材料制造或覆盖,这些合适的材料可以是不锈钢,硅,金,镍,特氟隆(即Teflon,聚四氟乙烯),钼、PMMA,或其他合适的塑料或材料。植入物也可以涂覆肝素或其他合适的生物活性化合物。
根据本发明的眼内植入物可以根据下述工序进行生产。管可以用一标准的不锈钢皮下注射针头来形成。用一放电加工机(EDM)机器在临近针尖端处钻上小孔以形成圆周孔。在离开尖端一段与管的预期长度相应的距离处,针被以合适的角度切断,其切割角度与碟片预期的角度相应。针的侧边接着被从下部切入以形成一突起部,突起部随后可被向外弯曲以形成刺铁。
碟片可根据以下工序由一不锈钢板通过化学浸蚀获得。在一计算机辅助设计(CAD)系统上画出碟片的样式并用一激光绘图仪把它绘制到一透明胶片上。对碟片的上侧和下侧都进行绘图。上侧的绘图,例如,包括外缘和内柱;下侧的绘图,例如,包括碟片的基部。
在不锈钢板的两个表面分别胶粘上一层光敏抗蚀剂。然后把抗蚀剂暴露于紫外(UV)光下,UV光是透过其上有绘图的胶片照到光敏抗蚀剂的。由图形所阻挡的板的区域并不暴光。暴露于UV光的光敏抗蚀剂随后用化学方法除去。
接着使用一浸蚀化学试剂把不锈钢板浸蚀,这样化学试剂就会把板上光敏抗蚀剂已被除去的区域腐蚀掉。对浸蚀进行时间控制以便使化学试剂仅把材料腐蚀到一预定的深度。
通过使用一张包括外缘和内柱的上侧的绘图,在金属板上表面的化学试剂把碟片外侧,贮液池中包括内柱之间及碟片中央(其用于接纳管)的材料腐蚀掉。因为对浸蚀是控制时间的,化学试剂仅把金属板顶部的材料腐蚀到厚度的一部分。通过使用一张包括碟片基部的下侧的图,在金属板下表面的化学试剂把碟片外侧及碟片中央(其用于接纳管)的材料腐蚀掉。化学试剂把金属板底侧的材料腐蚀至厚度的一部分。因为上侧和底侧的作用,碟片外侧以及用于接纳管的碟片的中央的材料在浸蚀过程中完全被腐蚀到整个厚度。在侵蚀过程中可在碟片的外侧留下小的突起部以防止碟片从金属板上脱落下来。

Claims (1)

1.用于植入于眼球中的眼内植入物,包括:
一用于植入到眼球之中的管,包括一入口端,一出口端,以及一于入口端和出口端之间延伸的管通道,以允许水状液从眼球中流出;其中该管在入口端具有一斜角表面,
一于管的出口端与管相连的碟片,用于安置于眼球的表面;
和至少一固定凸起部,用于将植入物固定在眼球中,
其特征在于该管具有至少一个圆周孔,该圆周孔在紧靠于管的入口端处开口到管通道之中。
